buy Medigap. State agencies in Washingt<strong>on</strong>, Wisc<strong>on</strong>sin and Idaho already are operating<br />
successful educati<strong>on</strong> programs and other states are c<strong>on</strong>sidering this approach. The<br />
Department of Health and Human Services could provide Informati<strong>on</strong> to newly enrolled<br />
Medicare beneficiaries, Including comparis<strong>on</strong>s of coverage, loss ratios, and exclusi<strong>on</strong>s of<br />
private plans that meet the applicable state and federal standards. In additi<strong>on</strong>.<br />
beneficiary educati<strong>on</strong> could emphasize that neither Medicare nor Medigap are designed<br />
to cover l<strong>on</strong>g term care.<br />
Alternately, senior groups or Medicare c<strong>on</strong>tractors could provide educati<strong>on</strong>al outreach.<br />
These programs could inform beneficiaries about existing opti<strong>on</strong>s to minimize their<br />
expenses, such as HMOs/CMPs, and could help beneficiaries compare the value of<br />
Medigap policies. Secretary Bowen's report recommends educati<strong>on</strong> for l<strong>on</strong>g term care,<br />
but that also would be a relatively Inexpensive approach to Inform beneficiaries of the<br />
need for catastrophic coverage of acute-care expenses.<br />
L<strong>on</strong>g Term Care Catastrophic Protecti<strong>on</strong><br />
The lack of l<strong>on</strong>g term care protecti<strong>on</strong> is the largest catastrophic coverage gap for the<br />
elderly. While Medicare and Medigap provide the elderly with reas<strong>on</strong>able protecti<strong>on</strong><br />
from catastrophic acute-care expenses, l<strong>on</strong>g term care Is the elderly's largest single<br />
out-of-pocket health expense.<br />
The private sector is beginning to resp<strong>on</strong>d to this need. The Blue Cross and Blue Shield<br />
organizati<strong>on</strong> recently completed a major effort to determine the feasibility of l<strong>on</strong>g<br />
term care insurance. Though multiple impediments exist, we believe insurance is<br />
workable. Indeed, <strong>on</strong>e Blue Cross and Blue Shield Plan has begun to offer this coverage,<br />
and several other Plans have pilot programs.<br />
