Fiscal Year 2012-2013 Agency Goals<br />

It is my privilege to present CONCERN’s agency goals for FY2012-2013. The pages that follow<br />

detail agency goals developed in five critical areas of CONCERN’s operations:<br />

• Corporate<br />

• Counseling Services<br />

• Foster Care and Adoption<br />

• Maryland Programs<br />

• Residential Services<br />

These goals are a product of our strategic planning process, which brings input solicited from<br />

every CONCERN service site in order to identify and prioritize annual and long-term agency goals.<br />

For each goal area, there will be the name of the committee chair, the corporate consultant (if<br />

applicable), committee participants, long-term goals, and FY2012-2013 annual goals. Each<br />

committee meets regularly throughout the year and written quarterly progress reports are issued<br />

by each committee.<br />

The environment in which we currently offer our services continues to be a challenging one.<br />

The hard work, creative thinking, and level of commitment of those who have been involved<br />

in our strategic planning and goal setting processes should help us to remain optimistic about<br />

CONCERN’s continued ability to meet the needs of children, youth, and families in Pennsylvania<br />

and Maryland.<br />

Corporate Strategic Planning Committee<br />

Goals – Fiscal Year 2012-2013<br />

Long-Term Goal – CONCERN’s business model will be defined by specific strategic direction from the<br />

Corporate Office.<br />

Annual Goals<br />

1. The Corporate office will examine rebranding strategies to address CONCERN’s expanding program<br />

diversity.<br />

2. The Corporate office will implement a plan to insure CONCERN’s continuum of care is well defined<br />

and accessible to internal staff and external stakeholders.<br />

Long Term Goal – CONCERN will promote agency-wide understanding of quality assurance and<br />

quality improvement processes.<br />

Annual Goals<br />

1. A structured Quality Assurance Summary will be developed for each CONCERN division, with the<br />

subsequent goal of being able to identify procedures that could be generalized agency-wide.<br />

2. CONCERN will work with each CONCERN division to purposefully and efficiently use outcomes<br />

and program data to make informed decisions.<br />

Long-Term Goal – CONCERN will expand its fundraising efforts to reflect its geographic presence in the<br />

states of Pennsylvania and Maryland.<br />

1. CONCERN will develop a three to five-year plan to revamp CONCERN’s fundraising practice.<br />

CONCERN Counseling Services Strategic Plan<br />

Long-Term Goal – CONCERN Counseling Services will implement improvement projects to<br />

maximize program viability, quality and productivity.<br />

Annual Goals<br />

1. CONCERN Counseling Services will research the Rx (prescription) program for Credible and<br />

complete a written cost-benefit analysis.<br />

Projected Completion: December 31, 2012<br />

2. CONCERN Counseling Services will develop and implement a work plan for revising the CCS<br />

Program Manual.<br />

3. CONCERN Counseling Services will develop and implement advanced autism programming<br />

agency-wide.<br />

Projected Completion: June 30, 2013<br />

Long-Term Goal – CONCERN Counseling will provide a continuum of quality mental health services to<br />

meet the needs in the communities we serve.<br />

Annual Goals<br />

1. CONCERN Counseling Services will research, develop and implement a specialized counseling<br />

program to address the increasing mental health treatment needs of Pennsylvania’s growing elderly


population and their families.<br />

Lead Responsibility: North Central Region Director and Lewisburg Clinical Supervisor<br />

(Eastern Region Director to provide assistance where necessary)<br />

Projected Completion: June 30, 2013<br />

2. CONCERN will develop resources and programming to correspond with Pennsylvania’s Early<br />

Childhood Mental Health initiatives.<br />

Lead Responsibility: Directors of North Central and Eastern Region CCS Operations<br />

Projected Completion: June 30, 2012<br />

3. CONCERN will research replicating Summer Therapeutic Activities Programs (STAP) that are<br />

currently being provided in the North Central region and implement this program across applicable<br />

agency service areas.<br />

Lead Responsibility: Eastern and North Central Region Directors and Clinical Supervisors<br />

Projected Completion: June 30, 2013<br />

4. CONCERN Counseling Services will develop resources and programming to correspond with the<br />

Cultural Competency initiatives that are being presented by the Managed Care Organizations.<br />

Lead Responsibility: Eastern and North Central Region Directors, LVO Clinical<br />

Supervisor, Human Resources Department<br />

Long-Term Goal – CONCERN Counseling Service will develop private funded programs.<br />

Annual Goals<br />

1. CONCERN Counseling Services will review federal laws expanding mental health coverage through<br />

2014. Opportunities to expand and develop private funded programming will be documented in a<br />

written plan.<br />

2. CONCERN Counseling will develop and implement a plan to refine and market the Employee<br />

Assistance Program.<br />

Foster Care and Adoption Strategic Planning Committee<br />

Goals – Fiscal Year 2012-2013<br />

Committee Participants: Rick Alvarez, Pam Berg, Eda Cofield, Tonya Klemmer, Helene Knapp,<br />

Long-Term Goal - CONCERN will strive for a standard of excellence in placement services, working to<br />

become a provider of choice for placing agencies.<br />

Annual Goals<br />

1. CONCERN will adopt a strengths based approach in working with children and their families; staff<br />

and resource families will be trained to implement the approach.<br />

Lead Responsibility: Selected Workgroup/Director of Social Services/Quality Assurance<br />

and Training Administrators/Director of Adoption and Permanency<br />

Services<br />

Projected Completion: June 30, 2013<br />

2. CONCERN will conduct an agency wide internal and external recruitment campaign to increase the<br />

placement of children in the Medical Levels Foster Care Program by 20%.<br />

Lead Responsibility: Regional Directors/Development Department/Medical Services<br />

Coordinator<br />

Projected Completion: June 30, 2013<br />

Long-Term Goal - CONCERN will improve safety, permanency, and well being outcomes for children and<br />

youth in our care.<br />

Annual Goals<br />

1. CONCERN will offer enrichment trainings to prepare foster, adoptive and host home families<br />

for working with specialized populations thus increasing placements and decreasing placement<br />

disruptions.<br />

2. Through quality casework visits, CONCERN will enhance the well being and quality of life for<br />

children in our care by monitoring timely access to education, behavioral health, medical and dental<br />

care, as well as extra-curricular activities and community involvement.<br />

Projected Completion: December 31, 2012<br />

3. CONCERN will implement a system to ensure 100% compliance with completing medical/dental/<br />

EPSDT services for children in foster care according to state regulations.<br />

Services Coordinator<br />

Projected Completion: December 31, 2012<br />

Long-Term Goal - CONCERN will recruit and retain sufficient resource families to meet the diverse needs<br />

of children referred for placement services with particular emphasis on increasing the capacity to care for<br />

children with medical needs and keeping children in their home school districts.<br />

Annual Goals<br />

1. CONCERN will develop and implement new “word of mouth” strategies for effectively recruiting<br />

foster, adoptive and host home families.<br />

Long-Term Goal - CONCERN will ensure that all clients will receive sufficient support in reaching their<br />

permanency goal.<br />

Annual Goals<br />

1. CONCERN will research effective models for providing reunification services to children returning<br />

to their family.<br />

Projected Completion: June 30, 2013<br />

2. CONCERN will implement measures to ensure that staff and families consistently support life book<br />

work with our children.<br />

Long-Term Goal - CONCERN will utilize innovative approaches to create opportunities for the meaningful<br />

involvement of legal families in their children’s lives during placement.<br />

Annual Goals<br />

1. CONCERN will increase opportunities for legal families to participate with their children and foster<br />

families in medical/dental appointments, school, athletic and community functions.<br />

2. CONCERN will identify and adopt a training curriculum regarding quality visitation practices,<br />

including additional research regarding a visitation center.<br />

3. CONCERN will emphasize our work with fathers by exploring resources such as the Fatherhood<br />

Initiative and Inside Out Dads.<br />

4. CONCERN will develop expertise in working with children of incarcerated parents.<br />

Maryland Programs Strategic Planning Committee<br />

Committee Chair: Carrie Knebel<br />

Corporate Consultant: Mary Beth Hughes<br />

Goals – Fiscal Year 2012 – 2013<br />

Committee Participants: Bambi Harmon, Valerie Rheinheimer, John Green, Teeda Jagne,<br />

Dana Fay, Consuella North, Kelly Golden, Lolita Gleaton and<br />

Erik Johnson<br />

Long-Term Goal- CONCERN will expand its array of services to ensure the long-term health of the Maryland<br />

Regional Office.<br />

Annual Goals<br />

1. The Maryland Regional Office will complete and implement a program that will address job<br />

readiness and career development utilizing the Ansell-Casey Life Skills Assessment Tool. Grant<br />

opportunities will be explored simultaneously in an attempt to identify potential funding sources for<br />

a position to implement the program.<br />

Projected Completion: December 31, 2012<br />

2. The Maryland Regional Office will actively engage certified foster parents in recruiting and<br />

approving new foster parents.<br />

Lead Responsibility: Regional Director/ Assistant Regional Director/ Foster Home<br />

Projected Completion: June 30, 2013<br />

3. The Maryland Regional Office will explore providing a parenting program utilizing SPIN as a<br />

component of Family Preservation for identified counties in the state.<br />

Lead Responsibility: Regional Director/ Assistant Regional Director/ SPIN Video Home<br />

Trainer/individuals trained in SPIN Video Home Training<br />

Projected Completion: December 31, 2012<br />

4. The Maryland Regional Office will explore Human Trafficking as it relates to the youth that<br />

CONCERN provides therapeutic services or potentially could provide therapeutic services. The<br />

Maryland Regional Office will provide training to the staff and foster parents so that CONCERN<br />

will be able to therapeutically respond to these youth’s needs.<br />

Residential Placement Strategic Planning Committee<br />

Committee Chair: Kevin E. Stichter<br />

Goals – Fiscal Year 2012-2013<br />

Committee Participants: Bob Cohick, Greg Girolamo, Jane Longazel, Ginny Ysa<br />

Long-Term Goal – CONCERN residential programs will position themselves to maintain their long-term<br />

viability.<br />

Annual Goals<br />

1. Each CONCERN residential program will document if and how they address the criminogenic needs<br />

assessed in the Youth Level of Service/Case Management Inventory (YLS/CMI).<br />

2. CONCERN will create training for residential staff on the Juvenile Justice System Enhancement<br />

Strategy (JJSES).<br />
