role?<br />

The major <strong>health</strong> system constraints in general can be grouped into 2 parts:<br />

Dem<strong>and</strong> side where <strong>the</strong>re is lack <strong>of</strong> universal access to a service delivery infrastructure. Examples<br />

<strong>of</strong> dem<strong>and</strong> side barriers are (e.g. affordability, stigma) to accessing services, inadequate service<br />

delivery infrastructure, weak drug regulatory <strong>and</strong> supply systems <strong>and</strong> multiple donor interests which<br />

might not be in t<strong>and</strong>em with national interests<br />

The supply side <strong>of</strong> <strong>the</strong> constraint are human resources <strong>and</strong> <strong>the</strong> way <strong>the</strong> service delivery is<br />

traditionally organized.<br />

Streng<strong>the</strong>ning <strong>health</strong> systems as part <strong>of</strong> HIV <strong>and</strong> AIDS scale up <strong>plan</strong> should ensure that<br />

o wider benefits <strong>of</strong> <strong>the</strong> general <strong>health</strong> system are achieved by making possible <strong>the</strong> integration<br />

<strong>of</strong> HIV <strong>and</strong> AIDS intervention into existing <strong>health</strong> systems <strong>and</strong><br />

o specific scale up or expansion <strong>of</strong> HIV <strong>and</strong> AIDS programmes are realized through building<br />

<strong>the</strong> necessary new infrastructure<br />

The entry points for <strong>the</strong> integration <strong>of</strong> HIV <strong>and</strong> AIDS interventions can be at <strong>the</strong> point <strong>of</strong> service delivery, in<br />

<strong>the</strong> management <strong>of</strong> programmes at district or local level, in <strong>the</strong> financing, procurement <strong>of</strong> resources <strong>and</strong> in<br />

<strong>the</strong> monitoring <strong>of</strong> programmes at national level.<br />

Therefore <strong>the</strong> two aspects <strong>of</strong> <strong>health</strong> system streng<strong>the</strong>ning in terms <strong>of</strong> ensuring wider benefits while scaling<br />

up HIV <strong>and</strong> AIDS programmes will be addressed in this <strong>the</strong>matic section.<br />

Intervention Area 1: National Strategic Planning <strong>and</strong> Programme Management<br />

Preamble<br />

As indicated in <strong>the</strong> introduction, that HIV <strong>and</strong> AIDS control programmes have increased <strong>the</strong> burden in <strong>the</strong><br />

already overstretched <strong>health</strong> care delivery system. This calls for innovative <strong>and</strong> renewed thinking on <strong>health</strong><br />

systems <strong>and</strong> service delivery as well as infrastructure, human resources development <strong>and</strong> <strong>plan</strong>ning.<br />

This section explores how streng<strong>the</strong>ning <strong>health</strong> service delivery at all levels would be done through:<br />

Innovative <strong>and</strong> renewed thinking on <strong>the</strong> organization <strong>of</strong> <strong>the</strong> <strong>health</strong> services<br />

Joint program management in:<br />

o Planning, implementation <strong>and</strong> reporting <strong>of</strong> activities,<br />

o quality improvement <strong>and</strong> st<strong>and</strong>ard setting<br />

o resource mobilization, utilization <strong>and</strong> accountabilities<br />

Providing technical support to non <strong>health</strong> <strong>sector</strong> units<br />

Options for delivering HIV interventions at community <strong>and</strong> household levels through partnerships<br />

<strong>and</strong> involvement <strong>of</strong> community-based agents <strong>and</strong> PLHIV.<br />

Situation analysis<br />

Below are <strong>the</strong> achievements <strong>and</strong> challenges <strong>of</strong> <strong>the</strong> general <strong>health</strong> <strong>sector</strong> reform <strong>and</strong> <strong>the</strong> introduction <strong>of</strong><br />

specific HIV <strong>and</strong> AIDS scaling up programmes<br />

Achievements<br />

General <strong>health</strong> <strong>sector</strong> reform-Programme management at national level<br />

The Ministry <strong>of</strong> Health <strong>and</strong> Social Welfare has initiated a process for improving <strong>the</strong> following:<br />

o decentralization <strong>of</strong> services <strong>and</strong> <strong>the</strong> streng<strong>the</strong>ning <strong>of</strong> existing institutions <strong>and</strong> o<strong>the</strong>r<br />

management structures<br />
