required to complete the Official Withdrawal Form in the<br />

Office <strong>of</strong> Educational Services. Failure to complete this form<br />

may create difficulties in the student’s eligibility to receive a<br />

tuition credit, if any is appropriate; in clearing his or her<br />

financial record; and in having academic records reflect an<br />

<strong>of</strong>ficial withdrawal.<br />

Full-time students who withdraw the intention <strong>of</strong> returning<br />

to complete their degree program may be granted a leave <strong>of</strong><br />

designation may be granted only to those students who are<br />

A part-time degree-seeking student who withdraws from<br />

all <strong>of</strong> his or her courses is not required to submit a <strong>of</strong>ficial<br />

withdrawal form.
