Influences of vertebral segments upon the entire body processes
Organic disorders are preprogrammed when provoked by malaligment of vertebral segments which adversely
affects the bilaterally emerging nerves. The corresponding individual organs and/or groups of organs are supplied
by information, respectively controlled by their relevant nerve groupings. The bilateral openings in the vertebral
segment are called intervertebral foramen thru, which emerge the organ controlling nerve roots.
Malaligned vertebral segments consequently may cause an intravertebral prolaps eventually leading to nerve root
compression, leading finally to impaired reflexes and motor disturbances. Such nerve root compression will impair
the function of organs and can further lead to great pain, as in sciatica. Continued unilateral compression, as at
work may often lead to numbness, pain sensitivity, etc., caused by fatiguing nerves. The illustration and
accompanying list refer to possible functional impairments due to vertebral malaligment, prolaps, disc protrusions
and sequesters.
The following compiled experiences covering impairments and disorders are based upon patient examinations and
their questioning in the course of nearly 20 years. The results of investigations stem from different countries, from
China via Russia and from Europe to the US. A physician must be consulted for a precise and reliable diagnosis.
The human being spends about one third of his
life in bed. This time period should be utilized
intensively for the distributed relief of the spinal
column and improve discal metabolism.
Back pain very often is generated by incongruity
between physical performance requirements
and the available capability of the spinal column.
For this reason it is very important to reach the
neutral position of the spinal column in the
recumbent, lateral or prone position during
sleep.
The Detensor ® -Therapy-Systems are fabricated
with water processed open cell PE foam
The Detensor ® Method by Kienlein ® is a system
designed for the relief and traction (distension)
of the spinal column during recumbency as well
as in sitting. This is achieved without the usually
necessary strapping and ropes.
The Detensor ® Method by Kienlein ®
Functional Principals
possessing high elasticity and ventilatory
features. The special and patented configuration
of the ribbed surface laminations in connection
with the design constrained cavities offer an
ideal ventilatory system facilitated by the cotton
terry cloth located between the body and the
laminations. This prevents uncomfortable
perspiration or freezing.
The average life span of the product may reach
10 years. The Detensor ® -Therapy-System
(recumbency-orthosis-5% and recumbencyorthosis-18%)
has been in daily use for 16 hours
or more in clinics since 1982.
.
The Detensor ® system of ribbed laminations
possesses extremely high elasticity on the
surface. The adaptation of either the recumbent,
lateral or prone position are regarded by experts
as optimal since the ribbed laminations serving
as supports are immediately located below the
surface, i.e. under the body. The anatomically
correct support of the spinal column is thus
possible.
A heart shaped lamination is seen at point “S” of
the lumbar region. The laminations above this
point are inclined towards the head and below
towards the feet. This inclination produces a
component which redirects the body weight into
an opposingly acting traction force affecting the
distension of the spinal column. The highly
flexible surface of the long term traction system
for beds (recumbency-orthosis-5%) produces an
equal distribution of the body weight, reducing
subconscious turning of the body and
consequently leading to longer rest periods
during sleep.
Page4

A special lying-down procedure may produce an
approximate redirection of 18% of the body
weight into a traction force. An additional 2% of
traction may be obtained in the cervical region
by correct placement of the functional cervical
support (FCS)
The recumbency-orthosis-5% may also achieve
as much as 10% traction by following the special
lying-down procedure. Turning and rolling during
recumbency may create as much as 5% traction
force.
The core and the ribbed surface are custom
designed to different body weights in order to
fully utilize the inherent surface elasticity. The
recumbency-orthosis-18% is available in five
grades of hardness and the recumbencyorthosis-5%
in three different grades of
hardness.
Detensor ® Recumbency Orthosis 5 %
The Detensor ® recumbency-orthosis-5% by Kienlein for passive long term traction treatment of the spinal
column.
Distension (stretching) of the spinal column is
still a proven principle for the treatment of
vertebral disc disease to this date. Derived from
the knowledge of orthopaedic mechanics of the
spinal column it is evident that traction should
not be applied to the spinal region on a short
term but long term basis. Reason being that the
vertebral disc does not possess the capability for
short term fluid exchange. The fully flexible
surface of the recumbency orthoses promotes
the fluid exchange in the vertebral disc by its
special arrangement of the laminations, which
1.) Vertebral discs
produce the so-called peristaltic effect. The
undisturbed function of the vertebral disc
depends upon the capability to alternately
absorb fluid under tension and discharge it
under compression.
The Detensor ® -Therapy-System (recumbencyorthosis-5%
in combination with the
recumbency-orthosis18%) produced exceptional
results on patients following disc surgery.
Page5

Persisting muscular hypertonicity and the
associated restriction in range of motion
generates pain. The recumbency-orthosis-5% in
combination with the recumbency-orthosis-18%
prevent vascular flow restrictions by the
lamination technique and normalize the
Reports about the positive results achieved by
the application of the recumbency-orthosis-5%
in combination with the recumbency-orthosis-
18% as for instance:
Degenerative spinal diseases as osteoporosis,
osteoarthitis, disc prolaps and protrusions,
spinal trauma (vertebral fractures, contusions),
The Detensor ® recumbency-orthosis-5% was
used successfully in hospitals, rehabilitation
The benefits associated with traction upon the
entire organisms of the human body have been
known long before Hippocrates. A relaxed body
is more ready in the position to activate its
inherent healing capabilities, thus amongst
others reducing back pain.
As reported by our competent physicians, more
than 90% of the patients treated experienced a
continual and lasting improvement to their back
pain during the course of treatment.
The Detensor ® -Therapy-System (5% in
combination with 18%) was initially conceived
for the traction (distension) of the spinal column.
However, the daily utilization of the system
yielded some very positive side effects in the
course of years.
2.) Sciata or Lumbago
3.) Other spinal diseases
4.) Decubitus
Findings
intervertebral spaces, relieving the nerve roots
within the vertebral segment. Compression relief
simultaneously reduces wear on the vertebral
joints.
anomalies of the spinal column, (stenosis of the
spinal canal, scoliosis, etc.) and dysfunction of
the trunk musculature. Because of the wide
range of indications, only a few examples can be
referred to at this point.
centers and sanatoriums.
The continually acting laminations not only
generate the force of traction, but also lead to a
massage of the connective tissue based on its
inherent oscillations, improved orthomechanical
alignment, and deep sleep, just to mention a
few. Subcutaneous cardiovascular restrictions
can be avoided by the exceptionally high pinpoint
elasticity.
The range of application for the Detensor ® -
Therapy-System is not restricted since it is not
possible to hyper distend the spinal column. No
negative findings of the system have been
reported since its inception.
These experiences and the resulting findings
have established a secure position of the
Longterm-Traction-Therapy in the treatment of
back pain.
Page6

Necessitated by the innovative 3-layer
combination of the Detensor ® recumbencyorthosis-5%,
it is mandatory to place it on a rigid
support in order to facilitate the full development
of the inherent effects. Lattice bed frames
should be removed, since their deformation to
the orthosis may cause an increase in
discomfort. We suggest the use of rigid boards
of about 2 cm’s thick and 10-15 cm’s width,
Attention!
spaced at a distance of 2-3 cm’s in between.
Only extremely elastic bed sheets are to be
used.
The high elasticity of the orthosis also permits
any inclined positioning. If the underlying
structure lends itself to adjustment, then the
supporting boards may of course be used.
Detensor ® Recumbency Orthosis 18 %
The Detensor ® recumbency-orthosis-18% by Kienlein ® for intensive traction treatment of the spinal
column.
The smaller unit of the therapy system is the
Detensor ® recumbency-orthosis-18%. It is a very
important component of the Detensor ® therapy
method by Kienlein ® for long term traction of the
During the phase of acute pain, i.e., acute
sciatica:
In the first week: Every three hours – a
minimum of 45-60 minutes – three to four
times daily.
In the second week: Therapy twice to three
times daily.
In the third and fourth week: Twice daily at
approximately 12:00 to 1:00 p.m. and in the
evening prior to retiring.
Following thereafter: Therapy should be
performed for 45-60 minutes a day. The best
time being in the evening prior to retiring.
Application
spinal column since it is capable of producing
greater forces of traction.
The elasticity and regeneration of the resting
spinal column during the night can be
maintained until rising in the morning, due to the
absence of the gravitational loads. Detensor ®
therapy is consequently more effective in the
evening.
Use the recumbency-orthosis-18% only in the
recumbent position and on a solid base,
preferably on the floor.
Sleeping during the night or for longer time
periods on the recumbency-orthosis-18%
should definitely be avoided.
Page7

Always use the functional B.K. (below knee)
support (FUS) belonging to the recumbencyorthosis-18%
because others, especially hard
support may lead to the restriction of vascular
flow. It is not advisable to place cushions, etc.,
under the FUS.
Furthermore, it is highly advisable not to place
the FUS in the widely known 90/90 position
since this reduces the force of traction.
Supporting the legs in a lower position than on
the FUS increases the lumbar lordosis,
inevitably leading to discomfort.
Discomfortz may occur at the beginning of
traction therapy on the recumbency-orthosis-
18%, which as a rule will subside shortly.
Optimal ventilatory function of the orthosis is
provided by the spacing between the ribs.
Hence it is important to cover oneself with a
blanket or bed sheet.
Observing the positioning and rollback technique
described in the handbook of the “Detensor ® -
Method” contained in the shipment) may
generate traction forces of 18%-22% of the body
weight.
This traction acts continuously as long as
the prescribed recumbent position is
maintained.
Light and elastic clothing (no synthetics) as
for jogging should be worn. This contributes to
attain the maximally available traction force by
first sitting in the position according to the
illustration, followed by rolling back onto the
recumbency-orthosis-18%. The basic rule for
this orthosis is also never to sit up with a jerk.
It is most important for the patient to roll off in
order to get up.
Discomfort caused by reloading the spinal
column may occur for a short period but will
generally subside shortly thereafter.
The recumbency-orthosis-18% can be used
beneficially even in the absence of back pain.
Positioning your body on the recumbencyorthosis-18%
just for relaxation will generate a
feeling of well being within a few minutes. The
orthosis may be applied wherever deep
relaxation, warming up muscles, relaxing the
spinal column and rest periods are called for.
The heart shaped rib in the recumbencyorthosis-18%
is easily found by touch. This is
the correct point for the waist line region (“S” or
“C” – range between 2 nd and 4 th lumbar
vertebrae).
The recumbency-orthosis-18% consists of three parts:
1. Back support
2. Functional cervical support (FCS)
3. Functional lower leg support (FUS)
Important instruction for using the recumbency-orthosis-18%:
The recumbency-orthosis-18% should not be used postoperatively under 6 months and never without
supervision of a therapist!
Page8

The functional cervical support (FCS), as
illustrated, helps relieve muscular tension in the
cervical region and is thus applied successfully
to patients suffering from cervical migraines.
Important Instructions!
We strictly advise that the combination of the
Detensor ® -Long-Term-Traction-Therapy (recumbency
advised not to use the recumbency-TVspectacles
when corrective glasses are normally
worn during TV-viewing.
