Service description: This program is funded by home <strong>and</strong> community care<br />
program. HACC provides support <strong>to</strong> assist frail elderly <strong>and</strong> younger people with<br />
disabilities <strong>and</strong> their carers. The services provided depend on the needs of the<br />
client or carer ranging from assistance with daily living, general household tasks,<br />
transport, in home respite, personal care, social support, allied health including<br />
counselling, social work <strong>and</strong> client education <strong>and</strong> other tasks in response <strong>to</strong><br />
individual needs. Eligibility: persons in the target group at risk of premature or<br />
inappropriate placement in institutional care – screened using the HACC ONI Tool.<br />

Service description: Support <strong>to</strong> frail aged clients in their own homes, daily if<br />
required – including assistance such as personal care, housework, transport,<br />
shopping <strong>and</strong> bill paying <strong>and</strong> assistance with meals. An emergency call system is<br />
provided. Eligibility: is assessed by Aged Care Assessment Service – contact them<br />
directly on 4122 8733 if you need this level of support. Fees: 17.5% of basic<br />
pension with adjustments depending on client’s financial circumstances.<br />
