Service description: Assessment of people aged over 70 years and indigenous
people over 50 years for Australian Government funded places:
• Residential Aged Care (high and low care)
• Residential Respite Care (high and low care)
• Community Aged Care Packages (CACPS)
• Extended Aged Care at Home (EACH)
• Extended Aged Care at Home – Dementia
• Transition Care Program
Home visiting service – free of charge.
