Service description: This program is funded by home and community care
program. HACC provides support to assist frail elderly and younger people with
disabilities and their carers. The services provided depend on the needs of the
client or carer ranging from assistance with daily living, general household tasks,
transport, in home respite, personal care, social support, allied health including
counselling, social work and client education and other tasks in response to
individual needs. Eligibility: persons in the target group at risk of premature or
inappropriate placement in institutional care – screened using the HACC ONI Tool.

pension with adjustments depending on client’s financial circumstances.

Service description: The Meals on Wheels service provides a nutritious meal
delivered by volunteers to the person's home. Meals are provided to help the

screening and assessment service free of charge for women over 40 years of age
including information about breast cancer and breast screening; free breast
cancer screening; follow up of abnormalities found on the breastscreen;
counselling; referral treatment if breast cancer is detected and an invitation for
rescreening every two years. A doctor's referral is not necessary. Women aged
50-69 are especially encouraged to attend every two years as the biggest risk
factor for breast cancer is age with 70% of all breast cancer diagnosed in women
over the age of 50. Parking is available and this service is located next door to
the red cross blood service, access via queens park road. Tty and interpreter
services are available for women with special needs. A satellite service is available
at Hervey Bay and rural areas are serviced every two years by the breastscreen
Queensland Bundaberg service relocatable service.
