<strong>2012</strong> Iowa District Nomination Form<br />

Each year the Iowa Tennis Association honors the volunteers, associations and programs that have<br />

done an outstanding job to promote the sport of tennis. If you know of a person, an association or<br />

program that should be recognized, send in a nomination. A detailed description about the person,<br />

association or event, press releases, and photos will assist the awards <strong>com</strong>mittee. After giving out<br />

awards at the ITA level, we may submit up to 3 nominations per category to the <strong>USTA</strong>/Missouri<br />

Valley Awards Committee for consideration. If more than one nomination for a category is forwarded,<br />

please indicate the preference of rank first through third. Six of the Sectional awards will be forwarded<br />

to the United States Tennis Association to be considered for the national award, in the respective<br />

categories (denoted by an asterisk). (Please submit ITA Awards by August 25, <strong>2012</strong> Nomination forms<br />

can be downloaded from the Iowa District website, clicking on the Awards tab)<br />

COMMUNITY SERVICE EXCELLENCE (award for an individual only)<br />

Community service awards are given for recent significant contributions made to tennis development by<br />

individuals actively involved in teaching tennis through <strong>com</strong>munity centers, schools and parks, inner city or<br />

suburban programs.<br />

Criteria to consider when nominating are:<br />

A. Scope of program.<br />

B. Depth of involvement in <strong>com</strong>munity, i.e., impact on <strong>com</strong>munity’s overall recreational picture.<br />

C. New and innovative features resulting from candidate’s activities.<br />

D. Inspirational and motivational work with adult or juniors,<br />

E. Involvement of other recreation organization such as the YMCA/YWCA, Boy Scouts, Girl<br />

Scouts, etc., in the establishment of a <strong>com</strong>munity oriented program, association with the<br />

<strong>USTA</strong>/ Missouri Valley Section and / or District.<br />

DISTINGUISHED SERVICE (award for an individual only)<br />

Distinguished Service Award recognizes performance, dedication and ac<strong>com</strong>plishment of an individual’s effort<br />

in service to the ITA (Alice Eaton Distinguished Service Award) and forwarded to the <strong>USTA</strong> Missouri Valley<br />

Section.<br />

TENNIS FAMILY OF THE YEAR* (award for a family only)<br />

This award is made annually to the family who in recent years has done the most to promote amateur tennis<br />

– primarily on a volunteer basis. Ideally all members of the family should participate in some way, either as<br />

a player or by offering their services in promoting tennis by directing programs and tournaments and helping<br />

with Junior Development from the beginning stages through the college level.<br />

JUNIOR PLAYER(S) OF THE YEAR (1 female and 1 male)<br />

Junior Player(s) of the Year award is based on performance, current ac<strong>com</strong>plishment, sportsmanship and the<br />

promotion of tennis in general, raising the standard of play to its highest level.<br />

ADULT PLAYER OF THE YEAR<br />

Adult Player of the Year award is based on performance, current ac<strong>com</strong>plishment, sportsmanship and the<br />

promotion of tennis in general, raising the standard of play to its highest level.<br />

RECREATIONAL PLAYER(S) OF THE YEAR (1 adult, 1 junior)<br />

Recognition of an Adult player (4.0 and below) who participates in <strong>USTA</strong> League Tennis or Team Tennis<br />

(adult) on a regular basis AND recognition of a junior player who participates in Jr. Team Tennis (youth) on a<br />

regular basis. Both recipients should demonstrate good sportsmanship and a lover for the game.<br />

OUTSTANDING CONTRIBUTOR TO <strong>USTA</strong> YOUTH PROGRAMS<br />

(award for an individual only)<br />

This person(s) has made significant contributions to <strong>USTA</strong>/Missouri Valley/Iowa Tennis Association 1) junior<br />

recreational programs, including <strong>USTA</strong> School Tennis, NJTL, Jr. Team Tennis and junior novice events or<br />

2) adaptive/wheelchair/special populations programs. Efforts may include CTA development, fund raising,<br />

equipment repair and distribution, summer tennis, special events, pro assistance, and education.<br />

MEDIA EXCELLENCE AWARD* (award for an individual only)<br />

Part-time or full-time reporter, writer, editor, etc. who provides extensive media support for the sport of tennis<br />

and its related activities. Criteria include coverage of <strong>USTA</strong>/Missouri Valley/ITA programs and events,<br />

frequency and depth of coverage and overall involvement in the tennis field.
