Metaplastic breast carcinomas (MC)<br />

• ER/PR/HER2 “triple negative” (TN) (80% <strong>of</strong> cases)<br />

• 3-yr disease-free survival (DFS) 75.5%<br />

• 3-yr overall survival (OS) 86.3%<br />

• 3-year OS TNMC 93.4%; 93 4%; NTNMC 58.2% 58 2% (P = 00.007) 007)<br />

• With respect to DFS: no survival difference<br />

• Contrary to that for invasive ductal carcinoma<br />

Spindle-cell <strong>Carcinoma</strong><br />

<strong>of</strong> the <strong>Breast</strong><br />

• Most metaplastic cas clearly high-grade.<br />

• BUT, spindle-cell ca can be deceptively benign.<br />

• Misdiagnosed as nodular fasciitis, fibromatosis,<br />

granulation g tissue, , or squamous q metaplasia. p<br />

• Also can be misclassified as sarcoma.<br />

• YET, these spindle-cell cas show aggressive behavior<br />

like infiltrating duct carcinomas (NST).<br />

• 5-year survival for spindle-cell ca report at 64%.<br />

Spindle-Cell (Metaplastic)<br />

<strong>Breast</strong> <strong>Carcinoma</strong><br />

• A malignancy capable <strong>of</strong> causing death.<br />

• Emphasized in 1981 by Gersel & Katzenstein.<br />

• Confirmed by Wargotz Sneige & others<br />

• Confirmed by Wargotz, Sneige, & others.<br />

• May appear deceptively benign – fasciitis-like.<br />

• Transition from SCCa to spindle cells present.<br />

• SCCa may be very, well differentiated & cystic.<br />

• Capillary-like tracery (angioid areas).<br />

• Tracery & spindle cells positive for keratin.<br />
