Primary Malignant Tumors

This chapter focuses on primary malignant tumors of the

breast, including ductal, lobular, and special types of carcinomas

and sarcomas.

Ductal Carcinoma

This section focuses on the cytomorphology of in situ versus

invasive ductal carcinoma. It is emphasized that this represents

a difficult area in cytology, and there is significant controversy

as to whether it is possible to distinguish between

these two types of lesions using only cytomorphology.

Ductal Carcinoma In Situ

Clinical Features

• Accounts for 10%–20% of mammographically detected

breast carcinomas

• Arises from terminal duct lobular units

Cytomorphologic Characteristics (Figures 4.1 to 4.9)

• Smears are hypercellular, with sheets and cords of malignant

cells.

• Cells are crowded and enlarged. Nuclei are hyperchromatic

with occasional prominent nucleoli.
