Figure 5.10. Metastatic medullary thyroid carcinoma. An irregular

fragment of pleomorphic malignant cells with high nucleus to cytoplasm

ratios and prominent pleomorphism and anisonucleosis. A

rare intranuclear inclusion is seen toward the center. There is nothing

in this smear to suggest a medullary carcinoma diagnosis. Clinical

history and immunoperoxidase staining are crucial when dealing

with metastasis in the breast. (Smear, Diff-Quik.)
