Figure 5.6. Metastatic small cell carcinoma of the lung. Hypercellular

smear illustrating the characteristic neuroendocrine cytomorphology,

that is, small round oval nuclei, fine dusty chromatin, nuclear

molding, nuclear crush artifacts, and numerous karyorrhectic nuclei.

Distinction from a primary neuroendocrine carcinoma of the breast

is not possible on morphology alone. (Smear, Papanicolaou.)

Figure 5.7. Metastatic small cell carcinoma of the uterine cervix.

This patient in advanced clinical stage presented with a breast mass.

Cytomorphology as seen here looks concordant with a primary cervical

tumor. The smear is hypercellular with large nests of neuroendocrine

cells, depicting high-grade morphology. Note prominent

nuclear molding, lack of cytoplasm, and abundant karyorrhexis.

(Smear, Diff-Quik.)
