Secretory Carcinoma
Clinical Features
• First reported in children and called juvenile carcinoma
• Occurs in children, adolescents, and adults
• Occurs in both men and women
• Shows a tubuloalveolar pattern of growth
Cytomorphologic Characteristics
• Hypercellular smears
• Background mucinous material
• Cells with abundant cytoplasm and intracytoplasmic vacuoles
• “Globules” of secretory material
Pitfalls and Differential Diagnosis
• Fibrocystic changes, papillomatosis
• Lactational changes
Adenoid Cystic Carcinoma
Clinical Features
• Rare tumors with good prognosis
• Slowly enlarging breast mass
• Lobulated and well-defined by mammography
Cytomorphologic Characteristics (Figures 4.38 and 4.39)
• Smears with globules and/or cylinders of pale blue hyaline
material on Papanicolaou stain
• Bright magenta globules on Diff-Quik stain
• Small hyperchromatic cells with very little cytoplasm
surrounding the globules
• Inconspicuous nucleoli
