Hayata her zaman pozitif bakıp,
kendinize dikkat edenlerden olmanız dileğiyle…
Hepinize sağlıklı günler!
We are all aware that cataract is a very difficult disease that can be seen at any
age from birth. This disease, which hinders vision with the increase in blurring
and makes even daily work impossible, unfortunately becomes unmanageable
with the increasing age. Although cataract has witnessed a difficult process, you
do not need to lose your hopes for this disease because I have good news for you:
Turkish scientist has developed a new method to be used in cataract treatment!
Although the innovations that will make an impact in the medical world continue
without slowing down, I need not tell you how proud we were when we witnessed
the article “Turkish Scientist”. Lecturer Prof. Dr. Rıfat Rasier, Faculty of Medicine,
Istinye University developed a new method that enables the conversion of singlefocal
lenses inserted into the eye to multifocal years ago. In addition, he received
the best project award in ESCRS, one of the most respected scientific associations
in the field of eye in the world.
We covered important issues such as new cataract treatment, 5 signals of bone
tumor and 6 questions about obesity testing in our magazine of this month. Hope
that we have been able to convey each and every article we have carefully written
in a useful way. I wish you to be one of those who always look at life positively and
take care of yourself…
Have a healthy day ahead for you all!
Brother country Pakistan
Kardeş ülke Pakistan
Turkey-Pakistan relations were built on the basis of
friendship and brotherhood. Friendship and brotherhood
relations between the two countries are rooted in an
effective response between the peoples of both countries.
Although our brotherhood manifests itself most clearly
at every opportunity in international relations, when we
look at the commercial and economic dimensions of
the business, we see that it is not at the desired point.
As such, our business people have important duties
in bringing commercial and economic relations to the
point they deserve. As it is known, fairs,
congresses and bilateral meetings are
the most important opportunities for
increasing economic relations.
Robotic surgery in colon cancer gives successful results
Kolon kanserinde
robotik cerrahi
başarılı
sonuçlar veriyor
Colon cancer (Large Bowel Cancer) treatment, which is
one of the most common cancer types today, gives better
results thanks to advancing technology, laparoscopyrobotic
surgery and smart drugs. Expressing that the Özel
Sağlık Hospital has become an important center in the field
of robotic surgery, Assoc. Dr. Acar stated that colon and
rectum surgery performed with robot offers significant
advantages for both doctors and patients.
Provides significant advantage in the healing
process
Providing information about the operation, Assoc. Dr. Turan
Acar, M.D., General Surgeon and Colorectal Surgeon at
Özel Sağlık Hospital said, “With robotic surgery in colon and
rectal cancer, the operated area can be evaluated closely
and in detail in three dimensions. Thus, it provides great
convenience in protecting the main vessels and nerves, and
cleaning even the smallest lymph nodes due to cancer. With
the unlimited mobility of the robot arm in the abdomen
and its high sensitivity to millimetric movements, it
becomes easier to reach areas that are difficult to reach in
open surgery. In this way, the highest efficiency is obtained
from the surgical procedure performed. In robotic surgery,
all diseased tissues can be cleaned with four small holes
of eight millimeters without causing large wounds in the
patient’s abdomen. Recovery after surgery and returning
to normal life is faster and easier. Other advantages include
less blood loss and need for blood transfusions, lower
risk of infection and complications, early start of bowel
function, earlier initiation of oral food intake, and less pain.”
keynote speaker and İ.Ü. Cerrahpaşa Faculty Member and
Secretary General of UHD Prof. Dr. Haluk Özsarı attended
as moderator, was organized with the contributions
of the Association of Research-Based Pharmaceutical
“With the development of gene therapies and
new generation therapies, health expenditures
will become unaffordable”
Speaking at the 26th TÜSAP Meeting, he said that they
started two projects with the titles “Implementation of
Quality-Oriented Financing Methods” and “Application of
Cost-Effectiveness Analysis and Determination of National
Cost-Efficiency Threshold” for drugs, in order to encourage
value-based payment according to gains, SGK GSS General
Manager Prof. Dr. Gökhan Tuna Öztürk talked about the
targeted outputs with these projects. Noting that the
Quality-Oriented Financing Methods Implementation
project aims to monitor and evaluate the quality, quality,
and quality of the health services financed, Öztürk, General
Manager of GSS, said that with the project, reducing the
impact of health cases that constitute the burden of
disease on health expenditures, minimizing re-applications
for the same transactions, planning evidence-based and
patient-oriented pricing. and ensuring the distribution
of patient safety and satisfaction. Adding to his words
that the most important database in this project is
MEDULA, Öztürk also mentioned the National Registration
System and mentioned that data suitable for analysis are
evaluated. Noting that they analyzed the revision rates
according to the facility type, using the quality forms of
the Ministry of Health for knee arthroplasty procedures,
Öztürk said, “There are some hospitals that have revisions
in 8 out of 100 patients. Complications developed in 2
percent of the patients afterwards. embolism, pulmonary
embolism developed, we sent him to the intensive care
unit, he became normal DVT and it cost him to re-establish.
There are such hospitals that the revision rate is 1. For this
reason, the patient did not visit the hospital again for the
next 10 years. We paid the same amount to both, and even
paid more for those who developed complications. In fact,
this is a situation that is not believed to be very undesirable
and very fair in the world literature.”
Stating that the project of Implementation of Cost-
Effectiveness Analysis for Medicines and Determination
of the National Cost-Effectiveness Threshold is carried
out under the roof of TÜSEB and SGK plays a stakeholder
role here, Öztürk said, “When we buy a drug and make an
SGK GSS Genel Müdürü Prof. Dr. Gökhan Tuna Öztürk’ün
ana konuşmacı ve İ.Ü. Cerrahpaşa Öğretim Üyesi ve ÜHD
Genel Sekreteri Prof. Dr. Haluk Özsarı’nın moderatör
olarak yer aldığı 26. TÜSAP Vizyon Toplantısı, Araştırmacı
ile düzenlendi.
“Gen tedavileri ve yeni nesil tedavilerin
gelişmesiyle birlikte sağlık harcamaları
karşılanamayacak duruma gelinecek”
Emboli, pulmoner
emboli gelişmiş, yoğun bakıma göndermişiz, normal DVT
olmuş ve tekrar kuruma bir maliyeti olmuş. Öyle hastaneler
de var ki revizyon oranları binde 1. Bu sebepten dolayı hasta
sonraki 10 yıl boyunca tekrar hastaneye uğramamış. Her
Nisan - April 2022
12
evaluation, we only evaluate the budget burden. We also
evaluate the clinical benefits, but we never evaluate how
much these clinical benefits will bring us. For example,
there is an obvious cost-effectiveness difference between
a drug that will treat a 15-year-old child with a full cure, and
a drug that will treat an 80-year-old patient with a full cure,
even if the drugs are the same and the cost burden is the
same. For example, we do not evaluate them at all. These
economic gains of the treatment should not only reduce
the health expenses, but even the employee power and
return to the institution as a premium should be calculated.
Conversely, not only the budget burden of the drug, but
also the cost of the side effects it may cause should be
calculated, and for this, the economic analysis methods
need to be detailed. Together with TÜSEB, we wrote
our project outputs related to this.” Listing the targeted
outputs of this project as the creation of the Health
Technology Assessment (HTA) Guide, determining the
scope of the cost-effectiveness analysis, establishing the
national cost-effectiveness threshold, conducting sample
cost-effectiveness analyzes for the determined drugs/drug
groups, and participating in international HTA organizations
and/or collaborations, Öztürk added that the backbone of
the General Health System is the 4A section.
Stating that the employees and their dependents are
43 million in the system and that 79 percent of the GSS
premiums go to health expenditures, GSS General Manager
Öztürk continued his words as follows: “79 percent
normally seems like a good rate, but the interesting thing
about this statistic is that only 14 of these 43 million people
are paid. Millions of them go to the hospital. 29 million
people did not go to the hospital even once in a year.
Only 14 million people are enough to spend 79 percent
of the premium collected on health services. If 25 million
of 43 million, not 14 million, applied to health services,
the system would not be sustainable. In the next stage,
we think that drug expenditures will increase more with
the introduction of gene therapies and new generation
therapies, especially in rare diseases. In a few years, we
will not be able to meet the expenses of even 14 of these
43, rather than fear that 25 million of 43 million will go
to the hospital. That’s why I think these projects are very
important.”
“There are direct or indirect payments in health
services”
Expressing that there are direct or indirect payments in
health services, Deputy Minister of Health of the Republic
of Turkey Prof. Dr. Sabahattin Aydın said that there is no
such thing as “repayment” in the social security system and
that when a word is misused at first, it is now accepted as
such. Adding that newer, more up-to-date and promising
payment methods for the future have emerged as a result
of especially the rapid development of health informatics,
the detailed recording of the registration system and the
very thorough follow-up of the processes, Aydın said,
“When we say that, we immediately perceive the issue as
SUT-based payment to service providers.”
hastaneye gidiyor. 29 milyon kişi bir yıl içinde 1 kez bile
hastaneye gitmemiş. Sağlık hizmetlerinde toplanan
primin yüzde 79’unu harcamaya sadece 14 milyon kişi
yetiyor. 43 milyonun 14 milyonu değil de 25 milyonu
sağlık hizmetlerine başvursa sistemin sürdürülebilirliği
kalmıyor. Bir sonraki aşamada özellikle nadir hastalıklarda
gelişen gen tedavilerinin, yeni nesil tedavilerin devreye
Cataract, which occurs most often with
advancing age and the effect of sun rays, is
among the leading causes of blindness and visual
impairment in the world. Specialists working for
the prevention and treatment of this disease,
the treatment of which is surgery, continue to
develop new methods. One of these experts, Prof.
Dr. Rıfat Rasier, Lecturer, Faculty of Medicine,
Istinye University, years ago, developed a new
method for converting single-focal lenses
inserted into the eye to multifocal. Informing
on this new method, Prof. Dr. Rasier made
explanations about the causes, symptoms and
treatment methods of cataract.
Cataract is one of the most common causes of vision loss
and vision loss in the world. According to the World Health
Organization (WHO), cataracts are among the leading
lenses used in cataract surgery multifocal with a new laser
method he applied. The method received the best project
award from ESCRS, one of the most respected scientific
about this new method, Prof. Dr. Rasier also made
statements about the causes, symptoms and treatment
methods of cataracts.
A white image can be noticed in the pupil of a
person with cataracts
Prof. Dr. Rıfat Rasier gave the following information about
cataract, “Cataract is the most common cause of vision loss
in the world. In order for the image to be formed, the light
must first pass through the frontmost transparent layer
of the eye, which we call the cornea. Then this light passes
through another transparent tissue, the lens in the eye,
and reaches the retina. The lens is a transparent, convex
structure on both sides. It allows the light coming into
the eye to be refracted and focused on the visual center
of the image. The lens should be transparent for life, if it
loses its transparency at any time, this situation is called
cataract. It causes difficulty in seeing the person by causing
problems in reaching the retina and reducing the contrast.
The lens in the eye loses its transparency and takes on the
structure of frosted glass. Just as a person looks through
a frosted glass, he sees the image as hazy, and in a person
with cataracts, the image he normally sees becomes hazy,
icy, foggy. In advanced stages, a mature cataract can reduce
the person’s appearance to a level where only light can
be noticed. The person looking at the person with such
advanced cataract may notice a white image instead of
blackness in the pupil.”
Sunlight exposure and increasing age are some
important factors
Stating that there are many reasons that may cause the
lens in the eye to lose its transparency, Rasier continued his
words as follows,
“The most important of these is the progression of our
age. As we age, the water content of the lens decreases
and the amount of lens proteins increases. In this case, the
lens becomes rigid, its flexibility decreases, and as a result,
the transparency of the lens gradually decreases. Another
important reason is exposure to sunlight with increasing
age. The eye, which is exposed to the ultraviolet rays of
the sun without wearing sunglasses, actually turns its lens
from transparent to frosted glass as a defense mechanism
so that more damaging light does not come to the retina.
Because these harmful rays coming to the retina cause
yellow spot disease, which we will explain later. Trauma is
one of the causes of cataracts. When a blunt or sharp object
hits the eye from the outside, the lens inside the eye may
lose its transparency, either by displacing or not moving at
all. Among the rarer causes that can lead to the formation
of cataracts are the use of drugs containing cortisone.
When cortisone medication is used in the form of drops,
it causes cataracts, and when it is used orally in the form
of pills, it can lead to cataract formation. While hereditary
metabolic diseases cause congenital cataracts in newborn
babies, many systemic diseases such as diabetes and thyroid
disease can cause cataracts in adults. If the sugar level is
kept within normal limits in systemic diseases, especially
diabetes, cataract development slows down.”
There are many different types of cataract!
Prof. Dr. Rasier listed the many different types of cataracts
as follows:
• Age-related cataract: It is a type of cataract that occurs
with the loss of water content of the lens as age increases,
and the increase in the protein ratio in the lens. After
the age of 40, the probability of developing age-related
cataracts doubles in every 10-year period. While the
probability of developing cataract is 5 percent around the
age of 65, this rate increases to 50 percent at the age of 75.
• Congenital cataract: Newborn babies may develop
congenital cataracts due to infection, blows they received
during birth, or the inability of the baby’s lens to fully
develop.
• Traumatic (injury) cataract: It is a type of cataract that
develops as a result of penetrating or blunt blows.
• Cataracts developing due to a systemic cause: It is the
type of cataract that develops due to a disease such as
Sistemik
hastalıklardan özellikle şeker hastalığında şeker seviyesi
normal sınırlarda tutulur ise katarakt gelişimi yavaşlar.”
Kataraktın birçok farklı tipi var!
Prof. Dr. Rasier, kataraktın birçok farklı tipini şöyle sıraladı:
• Yaşa bağlı katarakt: Yaş arttıkça lensin su içeriğini
kaybetmesi, lensteki protein oranının artması ile ortaya
çıkan katarakt tipidir. 40 yaştan sonra her 10 yıllık periyotta
yaşa bağlı katarakt gelişme olasılığı iki kat artar. 65 yaş
civarı katarakt gelişme olasılığı yüzde 5 iken 75 yaşında bu
oran yüzde 50’ye yükselir.
• Doğumsal katarakt: Yeni doğan bebekler enfeksiyon,
doğum sırasında aldıkları darbe veya bebeğin lensinin
tam olarak gelişememesi nedeniyle doğuştan katarakt
geliştirebilirler.
diabetes, thyroid disease, develops as a result of exposure
to a toxic substance, develops as a result of exposure to
ultraviolet, or develops as a result of the use of drugs such
as cortisone and diuretics.
• In addition, smoking, air pollution and excessive alcohol
consumption are also causes that can accelerate the
development of cataracts.
Symptoms of cataract:
Prof. Dr. Rasier states that the diagnosis of cataract is
made by detecting a decrease in your vision level with
the examination performed by your ophthalmologist,
and when the lens is examined with a microscope, the
opacity of the lens and the decrease of the transparent
parts are observed. “Due to the loss of transparency of
the cataracted lens, the vision-related symptoms become
more and more pronounced and progress to a situation
that is more disturbing and interferes with daily activities,”
Rasier summarizes the symptoms of cataracts as follows:
• Foggy, hazy, dirty appearance as if looking through a
frosted glass
• Rapid change of glasses numbers due to change in lens
• Changes in color vision
• Myopia shift of the eye with the development of cataract
and therefore the decrease in the need for near glasses.
In general, patients who have started cataracts express
themselves that they have started to see their relatives
better.
• Scattering of lights especially at night
• Scattering in images during the day
• Double vision as if images are superimposed
Cataract treatment is surgery
Prof. Dr. Rasier said, “The treatment of cataract is surgery.
If the person’s vision level is very low, if the visual level is
affecting the person’s quality of life, or if the lens is very
hard during the examination, cataract surgery should be
performed. The first thing to do is to determine the level
of vision of the person with the correction of eyeglasses
numbers. If the image is low despite the glasses, surgery is
required to remove the lens that has lost its transparency.
The name of cataract surgery is phacoemulsification
surgery. For this surgery, the opaque lens is broken up
with sound waves called ultrasound. After the lens is
removed, an artificial lens is put into the eye. The lenses
placed in the eye can be single-focal (only near or only far
view) or multifocal (far-mid-near view) lenses in today’s
technology. The benefit of multifocal lenses for the
patient is that they provide intermediate and near vision
without distorting the distant view. Thus, the need to
use glasses is eliminated while the cataract is surgically
removed, causing a blurry image. The number of people
who need this treatment, anyone over the age of 40-
42 who wears eyeglasses may need this lens. However,
multifocal lenses are not recommended for those who
have no problems with distance vision, as they cause a
slight loss of contrast at distance.”
aktiviteleri engelleyecek duruma doğru ilerler” diyen Rasier
olmayanlara tavsiye edilmez.” Dedi.
Pandemic bring obesity out of control!
Pandemi obeziteyi 12’den vurdu!
Pandemi obeziteyi kontrolden çıkardı. Türkiye’de
obezite görülme oranı kadınlarda %40, erkeklerde
%25 sınırına yaklaştı. Obezitenin kanser kadar
tehlike saçtığını belirten uzmanlar, tedavinin
endokrinoloji, beslenme, psikiyatri ve estetik
cerrahi uzmanlarının gözetiminde yürütülmesi
gereken komplike bir süreç gerektirdiğine,
Post-Bariatrik Cerrahi operasyonların obezite
tedavisine son noktayı koyduğuna dikkat çekiyor.
Prof. Dr. Alper Çelik, konuyla ilgili olarak,
The pandemic has brought obesity out of
control. The prevalence of obesity in Turkey
has approached the limit of 40% for women
and 25% for men. Stating that obesity is as
dangerous as cancer, experts point out that the
treatment requires a complicated process that
must be carried out under the supervision of
endocrinology, nutrition, psychiatry and aesthetic
surgery specialists, and that Post-Bariatric
Surgery operations put the last point in the
treatment of obesity. Prof. Dr. Alper Çelik, on the
subject, said, “Obesity is at least as dangerous as
cancer.”
Türkiye’de her 3 kişiden 1’i obez. Obezite için genel cerrahi,
The pandemic has increased the tension of obesity in
Turkey. According to the report prepared by the TBMM
Committee on Obesity Fighting Methods and Measures,
in 2021, the rate of obesity in women reached the limit of
40% and 25% in men. In the report, it was stated that 34%
of the population was overweight, that is, at the obesity
limit, and it was determined that the obesity rate was
increasing from east to west. Turkey ranks 1st in Europe
and 4th in the world with these rates. Stating that obesity
brings psychological problems such as depression along
with many risky health problems, Prof. Dr. Alper Çelik,
Medical Director of Etiler Esthetique Center and Private
Etiler Hospital, said, “Increasing stress and anxiety in the
pandemic opened the door to eating disorders. Obesity,
as the disease of the age, poses as much danger as cancer
in Turkey as well as in the rest of the world. According to
the data of the World Health Organization, 1 out of every
3 people in Turkey is obese. There is a long treatment for
obesity, which must be carried out under the supervision
of specialists in general surgery, endocrinology, nutrition,
psychiatry and plastic surgery, and even cardiothoracic
specialists should be included if necessary.”
Skin recovers with post-bariatric surgery
operations
Noting that obesity treatment is a complex treatment
under the control of various specialists, starting with
obesity surgery and extending to the recovery of the
body with aesthetic operations, Prof. Dr. Alper Çelik
made the following assessment on the subject, “It is of
great importance that all stages of treatment are carried
out at the right time. This process carries a great risk
for those who have diabetes, high blood pressure and
anemia. Our priority is always to eliminate these problems
and lose excess weight. Excessive weight loss can cause
unsightly skin sagging, especially in the face, neck, arms,
chest, abdomen, hips and legs areas of the body. We can
correct these deformations with “Aesthetics After Obesity
Surgery”, that is, Post-Bariatric Surgery, which includes
many procedures.”
As the weight lost increases, the deformation
also increases
Recording that the increase in the amount of weight given
increases the body deformations, Medical Director Prof. Dr.
Alper Çelik said, “When the period of getting rid of excess
weight is over, we still need a waiting period. Our goal
here is primarily to ensure that the patient can stay at his
target weight for a certain period of time. After this stage,
Post - Bariatric Surgery comes into play. Such operations
may involve multiple stages depending on deformations.
Although the recovery process takes a little longer than
other surgical operations, Post- Bariatric Surgery is one of
the most important stages of obesity treatment. We see
this last stage of treatment as a necessity, not a choice, so
that patients can regain a healthy form and psychology.”
Post-Bariatrik Cerrahi operasyonlarla cilt
toparlanıyor
Obezite tedavisinin geniş bir yelpazede çeşitli uzmanların
kontrolünde obezite cerrahisi ile başlayıp vücudun estetik
operasyonlarla toparlanmasına kadar uzanan komplike
bir tedavi olduğunu kaydeden Prof. Dr. Alper Çelik, konuya
ilişkin şu değerlendirmede bulundu: “Tedavide tüm
aşamaların doğru zamanda yapılması büyük önem taşıyor.
Diyabet, yüksek tansiyon, kansızlık sorunu yaşayanlar için
bu süreç büyük bir risk barındırıyor. Önceliğimiz her zaman
bu sorunların giderilmesi ve aşırı kiloların verilmesi…
Aşırı kilo kaybı vücudun özellikle yüz, boyun, kol, göğüs,
karın, kalça, bacak bölgelerinde göze hoş görülmeyen cilt
sarkmaları oluşturabiliyor. Bu deformasyonları, çok sayıda
işlemi kapsayan “Obezite Cerrahisi Sonrası Estetik” yani
Post-Bariatrik Cerrahi operasyonlarla giderebiliyoruz.”
Buradaki
hedefimiz öncelikle hastanın hedef kilosunda belirli bir
süre kalabilmesini sağlamak. Bu aşamadan sonra Post-
Bariatrik Cerrahi devreye giriyor. Bu tür operasyonlar,
deformasyonlara bağlı olarak birden çok etabı
kapsayabiliyor.
6 Soruda obezite testi!
General Surgery Specialist Prof. Dr. Bilgi Baca
Genel Cerrahi Uzmanı Prof. Dr. Bilgi Baca
Stating that obesity can cause many diseases from heart
to diabetes, from stroke to cancer in both children and
adults, it must be treated, Prof. Dr. Bilgi Baca said that very
successful results can be obtained if bariatric surgery is
performed with a multidisciplinary approach and to suitable
candidates for bariatric surgery. Pointing out that some
wrong information that is thought to be true in the society
may cause delay in treatment or negative results, General
Surgery Specialist Prof. Dr. Bilgi Baca Prof. He explained
8 wrongs that are believed to be true about obesity and
the criteria suitable for bariatric surgery with a 6-question
test, and made important warnings and suggestions.
The epidemic disease of the century, Covid-19, which has
affected the whole world for two years, caused another
dangerous disease to become widespread in this process.
The name of this dangerous disease, declared by the World
Health Organization as ‘the most serious health problem of
the 21st century’ is obesity.
Prof. Dr. Bilgi Baca, Lecturer, Department of General
Surgery, Faculty of Medicine, Acıbadem University, General
Surgery Specialist at Acıbadem Altunizade Hospital, said,
“While compulsory staying at home and working from
home caused inactivity during the pandemic that has been
problems”: WRONG!
ACTUALLY: Obesity is not just
an aesthetic disorder, it is a
chronic disease that can lead
to serious consequences.
According to the World
Health Organization (WHO),
“Obesity is a chronic disease
characterized by an increase
in body fat mass compared
to lean mass, and an energy
metabolism disorder that can
cause physical, mental and
social problems caused by
excessive fat storage in the
body” and is accepted all over
the world. has seen. Therefore,
obesity is a health problem.
• After the obesity surgery,
I eat as I want, I do not gain
weight either: WRONG!
ACTUALLY: Obesity surgery
aims to make the person happy
and lose weight by eating less
food. If the patient does not
pay attention and does not
comply with what the doctor
and dietitian say, he can return
to his old weight.
• Obesity surgery can be applied to anyone who wants it:
WRONG!
ACTUALLY: It cannot be applied to everyone who wants
bariatric surgery. Whether the person is a suitable
candidate for bariatric surgery is decided by looking at the
following criteria:
1. Do you have any disease and only your body mass index
is 40 kg/m2 and above?
2. Your body mass index is 35 kg/m w and you have type-2
diabetes, hypertension, sleep apnea, asthma etc. Do you
have any co-morbidities and do these diseases negatively
affect your quality of life?
3. Your body mass index is 27 kg/m 2 and you have
concomitant diseases, have you not been able to benefit
from diet, exercise and pharmacological treatment with a
specialist for at least six months?
4. Advanced heart disease, cancer, liver cirrhosis, etc. do
you have any diseases? Obesity surgery cannot be applied
to those with this type of disease.
5. Do you have alcohol and substance addiction? Obesity
surgery cannot be applied to such addicted people.
6. Are you at reproductive age and are you planning to
become a mother soon? If you are planning to become a
mother in the near future, you should definitely consult
your doctor. Because pregnancy is not recommended for
24 months after bariatric surgery.
• It is enough for me to talk to the surgeon for the
DOĞRUSU: Obezite cerrahisi isteyen
herkese uygulanamaz. Kişinin
obezite cerrahisine uygun bir aday
olup olmadığına şu kriterlere
bakılarak karar verilir:
1. Herhangi bir hastalığınız olmayıp
sadece vücut kitle indeksiniz 40 kg/
4. İleri kalp hastalığı, kanser, karaciğer sirozu vb.
hastalıklarınız var mı? Bu tür hastalığı olanlara obezite
cerrahisi uygulanamıyor.
5. Alkol ve madde bağımlılığı var mı? Bu tür bağımlı kişilere
obezite cerrahisi uygulanamıyor.
6. Üreme çağında olup yakın zamanda anne olmayı
düşünüyor musunuz? Eğer yakın zamanda anne olmayı
planlıyorsanız mutlaka hekiminize danışmanız gerekir.
Çünkü obezite cerrahisi ameliyatı sonrası 24 ay hamilelik
önerilmiyor.
davranış değişimi ve hedef kiloya ulaşma, hedef kiloyu
koruma ve çevre değişimi programlarını bitiren, buna
rağmen hedef kiloya (mevcut kilonun yüzde 10-20’sini
sini kaybetme) ulaşamayan hasta, multidisipliner bir
ekiple değerlendirilir. Tüm değerlendirmeler sonunda
alınacak karara göre uygun cerrahi prosedüre karar verilir.
Cerrahi değerlendirme, anestezi değerlendirmesi, iç
hastalıkları değerlendirmesi (endokrin ve gastroentoroloji),
kardiyolojik değerlendirme, psikiyatrik değerlendirme,
fiziksel tıp ve rehabilitasyon değerlendirme ve diyetisyen
ACTUALLY: In bariatric surgery, being examined by the
surgeon is only one of the steps. The patient, who has
completed the programs of education and consciousness
change, behavior change and reaching the target
weight, maintaining the target weight and changing the
environment, but still cannot reach the target weight
(10-20% of the current weight), is evaluated with a
multidisciplinary team. According to the decision to be
taken at the end of all evaluations, the appropriate surgical
procedure is decided. Surgical evaluation, anesthesia
evaluation, internal medicine evaluation (endocrine and
gastroenterology), cardiological evaluation, psychiatric
evaluation, physical medicine and rehabilitation evaluation
and dietitian evaluations are made.
• I can have any surgery I want, the same surgery can be
applied to everyone: WRONG!
ACTUALLY: The patient cannot decide on his own what
surgery will be performed for obesity surgery. Not
everyone can be operated on. The surgical sleeve, which
is widely used in obesity surgery all over the world,
gastrectomy (tube stomach). It is preferred because it has
better physiological and functional results. However, the
patient’s age, body mass index, concomitant diseases,
eating habits play a major role in deciding the technique to
be used. After all consultations are completed, the surgeon
and the patient decide together which surgery will be
performed.
• If I have bariatric surgery, I will die or be left unfinished:
WRONG!
ACTUALLY: Correct patient selection, correct indication
and correct surgical method are very important in bariatric
surgery. Obesity surgery should be performed by a
professional team in an experienced and equipped center.
The risk associated with bariatric surgery is no higher than
the risk of gastric surgery. Removal or processing of a part
of the stomach does not constitute a significant deficiency
in the patient. Surgery complications that may develop in a
patient for whom surgery is recommended due to obesity
are less risky than the current obese state.
• After bariatric surgery, I’m done with the hospital and the
team: WRONG!
ACTUALLY: Prof. Dr. Bilgi Baca said, “A patient who has
undergone bariatric surgery has a journey of at least two
years with us. Follow-up continues in the long term. After
the first postoperative control, regular controls and blood
tests are performed in the first month, third month, sixth
month, 1st year and 2nd year. The results are evaluated
and vitamin and mineral supplements are provided if
necessary.”
• Within a month after bariatric surgery, I would be
completely free of excess weight: WRONG!
ACTUALLY: In obesity surgery, the goal of weight reduction
is completed in 1.5-2 years. The patient loses 75-80% of
their excess weight with bariatric surgery and diet. It is
recommended to lose excess weight gradually. After six
months, when exercise becomes a part of the life change,
patients regain their ideal weight in a healthy way.
• İstediğim ameliyatı olabilirim herkese aynı ameliyat
uygulanabilir: YANLIŞ!
DOĞRUSU: Obezite cerrahisi için ne ameliyatı yapılacağına
hasta kendi karar veremez. Herkese ameliyat uygulanamaz.
Obezite cerrahisinde tüm dünyada yaygın olarak kullanılan
ameliyat sleeve gastrektomi (tüp mide)’dir. Fizyolojik
Takipler uzun vadede devam eder. Ameliyat sonrası ilk
kontrolden sonra birinci ay, üçüncü ay, altıncı ay, 1.yıl
ve 2.yıl düzenli kontrol ve kan tetkikleri yapılır. Sonuçlar
değerlendirilerek gereklilik halinde vitamin ve mineral
destekleri sağlanır” diyor.
• Obezite cerrahisi sonrası bir ay içinde tamamen fazla
kilolarımdan kurtulmuş olurum: YANLIŞ!
DOĞRUSU: Obezite cerrahisinde kilo azalım hedefi 1,5-2
yılda tamamlanır. Hasta fazla kilolarının yüzde 75-80’ini
obezite cerrahisi ve uygulanan diyetle kaybeder. Fazla
Hand, foot, mouth disease can mimic the flu
El, ayak, ağız
hastalığı gribi taklit
edebilir
Noting that hand, foot and mouth disease, which
is common in infants and children, is a disease
that can be transmitted through respiratory tract,
saliva, close contact and feces and has no cure,
Pediatrician Dr. Tarık Ersöz, Medical Park Tarsus
Hospital, said, “Hand, foot, mouth disease can
mimic the flu infection.”
Stating that the hand, foot and mouth disease, which has
been seen in babies and children recently and which is not
known much, usually manifests itself as a throat infection,
Pediatrician Dr. Tarık Ersöz, Medical Park Tarsus Hospital,
gave information about the disease, which is mostly seen in
the hands, face, feet, knees and hips of the body.
Fever and throat infection are the most
prominent complaints
Pointing out that there is a history of admission to the
emergency room or hospital with fever, it is usually
diagnosed as throat infection in hand, foot, mouth disease,
Dr. Tarık Ersöz said, “As a symptom of the disease, high
fever, intense redness and aphthae in the tonsils, itchy
and painful roughness in the hands and feet, knees and
buttocks occur. At first, these lumps turn into small bubbles
filled with liquid in the future.”

Pay attention to the ways of transmission
Pointing out that this disease caused by the coxsacki virus
from the
enterovirus family can be easily transmitted through
epidemics in summer and autumn, Dr. Tarık Ersöz warned:
“Hand, foot, mouth disease can be transmitted through
the respiratory tract, saliva, close contact and feces. It can
mimic a flu infection. Since the disease is a viral disease,
antibiotic treatment is not effective and the disease goes
away on its own within a week. It is important that the sick
child is not in close contact with others during this period.
With the opening of the pool season in the summer, a
separate risk arises with the involuntary ingestion of pool
water. The most effective method of preventing hand-footmouth
disease is hygiene.”
Chickenpox-like blisters appear on the hands,
feet, and mouth
Emphasizing that at the end of the first 3 days in children
and babies infected with the disease, varicella-like, waterfilled
blisters around the hands, feet and mouth will
attract attention, Dr. Tarık Ersöz said, “Unlike chickenpox,
blisters do not form on the body, but around the hands,
feet and mouth. Liquid-filled red blisters on the insides of
the hands and feet, between the fingers, as well as the
soles of the feet, cause both pain, ache and itching. There
are sores called aphthae in the throat, and the child may
have swallowing problems and loss of appetite due to this.
Apart from this, redness in the tonsils, ear and throat pain,
weakness can be seen.”
Causes both pain and itching
Emphasizing that the liquid-filled red lumps that cover the
soles of the feet as well as the insides of the hands and
feet, as well as between the fingers, cause both pain, ache
and itching, Dr. Tarık Ersöz said, “Weakness and malaise are
accompanied by ear and throat pain. Aphthae in the mouth
and redness in the tonsils can make eating a torture.”
It can go away on its own within 10 days
Stating that antibiotics have no place in the treatment of
the disease because it is a viral infection, Dr. Tarık Ersöz
said, “There is no vaccine or medicine for which there is no
specific treatment for the disease. In the treatment, sprays
and various lotions can be used to reduce and alleviate
itching. Adding to his words that the disease went away
on its own within 10 days, Dr. Ersöz stated that liquid-filled
red blisters on the hands and feet may dry out and leave
the stain, but over time the stains will disappear and there
is no need to worry.
The most important precaution should be
hygiene
Pointing out that hygiene is the most important measure
to protect against hand and foot disease, Dr. Ersöz
made the following suggestions: “It is very important
to wash hands frequently and not to rub dirty hands
on the mouth and face.” Emphasizing that people with
this disease should be avoided because it is contagious,
Dr. Ersöz said, “It states that frequent ventilation of the
environment, disinfection of hands and surfaces in places
such as schools and nurseries, and compliance with the
necessary hygiene rules will prevent the risk of spreading
the disease.”
Loss of appetite and fatigue may occur
Stating that loss of appetite and fatigue can also be seen
during the disease, Dr. Ersöz pointed out that when such
effects are observed, children should be avoided as much
as possible from cool foods (yogurt, pudding, ice cream,
etc.) and foods that will not trigger mouth sores (spicy and
hot), and that plenty of water should be consumed.
The first 7 days are critical for transmission
Emphasizing that the first 7 days of the disease is the
period when the contagion is highest, Dr. Ersöz concluded,
“However, the virus continues to spread through oral
fluids and feces for days and weeks after the symptoms
have completely disappeared. The easiest way to prevent
the spread of the disease to others is to thoroughly wash
your child’s hands and your own hands. It is very important
to wash the hands, especially after wiping the child’s nose
and changing the diaper.”
ekleyen Uzm.
Anger management can be found out!
Öfke kontrolü öğrenilebiliyor!
Üsküdar Üniversitesi NPİSTANBUL Beyin
Hastanesi Uzman Klinik Psikolog Ömer Bayar,
Oscar ödül töreninde Oscar kazanan Will
Smith’in eşi üzerinden espri yapan Chris Rock’a
tokat atmasıyla gündeme gelen öfke kontrolü
konusuyla ilgili değerlendirmede bulundu.
Öfke kontrolü sağlamanın, duyguları tanımanın ya da
duyguları kontrol edebilmenin öğrenilebilen bir durum
olduğunu ifade eden uzmanlar, çocukluk döneminden
itibaren duyguların öğrenilmesi gerektiğini belirtiyor.
Çocuklukta öğrenilmemesinin, ileri yaşlarda öfke
kontrolünün öğrenilmeyeceği anlamına gelmediğini
vurgulayan uzmanlar, yetişkin bireylerin de gereken çabayı
göstermesi halinde öfke kontrolünü öğrenebileceğini
vurguluyor. Ayrıca, uzayan bir tartışmada kişinin daha çok
öfkelendiğini fark ettiği anda ara vermesini öneriyor.
Clinical Psychologist Ömer Bayar
Clinical Psychologist Ömer Bayar, NPİSTANBUL
Brain Hospital, Üsküdar University, made an
assessment about the anger control issue, which
came to the fore when Oscar winner Will Smith
slapped Chris Rock, who was joking about his
wife, at the Oscar award ceremony.
Stating that anger control, recognizing emotions or being
able to control emotions can be learned, experts state that
emotions should be learned from childhood. Emphasizing
that not being learned in childhood does not mean that
anger control cannot be learned in later ages, experts
emphasize that adults can learn anger control if they make
the necessary effort. He also recommends taking a break as
soon as a person realizes that they are getting more angry
in a prolonged discussion.
Learning to control internal stimuli
Noting that anger is one of the natural emotions such as
sadness, disappointment, happiness, jealousy and fear,
Specialist Clinical Psychologist Ömer Bayar said, “Each
of these emotions are internal stimuli that develop from
childhood and become aware of over time. Over time we
learn to make sense of and control these internal stimuli.”
Emotions need to be learned
Indicating that recognizing emotions and regulating them is
İçsel uyaranları kontrol etmek öğreniliyor
Öfkenin hüzün, hayal kırıklığı, mutluluk, kıskançlık, korku
gibi doğal duygulardan bir tanesi olduğunu kaydeden
Uzman Klinik Psikolog Ömer Bayar, “Bu duyguların her biri
çocukluktan itibaren gelişen, zamanla farkına vardığımız
içsel uyaranlardır. Zamanla biz bu içsel uyaranları
anlamlandırmayı ve kontrol etmeyi öğreniriz.” dedi.
++Duyguların öğrenilmesi gerekiyor
possible by gaining experience, as in everything else in life,
Specialist Clinical Psychologist Ömer Bayar waned, “Just
as a person is inexperienced in a subject that he has never
tried before, or that he has not studied enough, he may
have difficulties in anger control. If their emotions are not
translated by the family and their immediate surroundings
during their developmental stages, these children can turn
into individuals who cannot experience their emotions.”
Reasons for anger control may be different
Stating that there may be different reasons for the
difficulty of anger control Specialist Clinical Psychologist
Ömer Bayar, said, “Some mental disorders, problems that
people go through periodically, that is, situations that force
the mental integrity of the person, can make it difficult to
control not only anger but also other emotions.”
Watch out for later anger attacks!
Expressing that having difficulty in anger control can
sometimes be a harbinger of psychological problems,
Ömer Bayar said, “It is important to look at the flow of life
of the person in order to distinguish this. For example, if a
person who has never had an anger control problem before
suddenly starts to experience sudden, meaningless anger
attacks, there may be a psychological problem that goes
wrong. This may be a result of a person’s personality traits
from childhood, as well as a psychological problem.”
Anger control can be learned at an advanced age
Specialist Clinical Psychologist Ömer Bayar, who stated
that providing anger control, recognizing or controlling
emotions is a situation that can be learned, warned,
“Actually, we need to learn emotions from our childhood.
We learn emotion control through our experiences by
recognizing and understanding emotions, by having the
opportunity to experience and develop them. The fact that
it is not learned in childhood does not mean that anger
control cannot be learned in later ages. If the person makes
the necessary effort, he or she can learn to control anger.
For example, if some children see that the problems in the
house are solved by shouting and that the people in the
house do not experience their emotions in a healthy way,
they will learn not to control the anger, but to act out the
anger in an uncontrolled way.
“Bunun ayrımını yapmak için kişinin hayat akışına bakmak
önemlidir. Örneğin daha önce hayatında hiç öfke kontrol
problemi yaşamayan bir insan, bir anda ani, anlamsız
öfke atakları yaşamaya başladıysa yolunda gitmeyen bir
psikolojik sorun olabilir. Bu psikolojik bir sorunun yanı sıra
kişinin çocukluktan beri süre gelen kişilik özelliklerinin bir
sonucu olabilir.” Diye konuştu.
Öfke kontrolü ileri yaşlarda da öğrenilebilir
Öfke kontrolü sağlamanın, duyguları tanımanın ya da
kontrol edebilmenin öğrenilebilen bir durum olduğunu ifade
eden eden Uzman Klinik Psikolog Ömer Bayar, “Aslında
bizim çocukluğumuzdan itibaren duyguları öğrenmemiz
gerekir. Duyguları tanıyarak, anlayarak, onları yaşamaya ve
onları geliştirmeye fırsat bularak deneyimlerimizle beraber
duygu kontrolünü öğreniriz. Çocuklukta öğrenilmemesi
ileri yaşlarda öfke kontrolünün öğrenilmeyeceği anlamına
gelmiyor.
The reason for anger should be understood
Bayar, who noted that difficulties in anger control can be
treated, warned, “For this, it is necessary to understand
the factors that strengthen anger control. For example,
a child with dyslexia or understanding difficulties may be
experiencing anger management with that helplessness
because he cannot understand what he should do in
school life. A normally calm person may have difficulty in
anger control after alcohol use. In other words, first of
all, the reasons for the difficulty in anger control should
be understood and an appropriate treatment should be
found.”
Nisan - April 2022
32
Pay attention to these recommendations for
anger control!
Bayar also gave advice to people who have difficulty in
anger control and said, “First of all, it should be understood
that anger is not a problem to be feared. It is necessary to
be aware that anger is a natural emotion like happiness,
sadness and longing. For example, when we are aware
of situations that make us angry, we need to avoid those
situations.”
A protracted discussion must be interrupted!
“If we realize that we are getting more angry in a prolonged
discussion, we can take a break when a discussion starts,
and by taking breaks like I need to clear my head, we will
prevent this anger from escalating and becoming difficult
to control. In addition, when we get angry, we can distract
ourselves and do relaxation exercises.
Usually, when we get angry, the body goes into a tense
and stressful situation, and in this case, when appropriate
breathing exercises are done, it helps to reduce the blood
pressure and pulse in the body and relax.
If the difficulties in anger control are excessive and sudden
outbursts, at least until the person develops his own
anger control, he can take medication support, and it will
definitely be useful to get therapy support.”
Attention! This disease is insidious!
Dikkat! Bu hastalık sinsice ilerliyor!
Orthopedics and Traumatology Specialist Assoc.
Dr. Seyit Ali Gümüştaş, Acıbadem Fulya Hospital
In bone tumors that usually progress insidiously and can be
seen at any age, reluctance to go to the hospital with the
fear of Covid-19, especially during the pandemic process,
may cause early diagnosis to be missed. Orthopedics and
Traumatology Specialist Assoc. Dr. Seyit Ali Gümüştaş,
Acıbadem Fulya Hospital, said, “Bone tumors usually
progress slowly. Benign tumors are about 100 times
more common than malignant tumors. Early diagnosis in
malignant tumors facilitates the removal of the arm or
leg where the tumor is located and increases the patient’s
life expectancy. Collaboration of orthopedics, radiology
and pathology physicians is extremely important in the
diagnosis of bone tumors. However, delaying going to the
hospital and applying to the doctor for some complaints
during the pandemic process may cause the disease to
progress further. Emphasizing that listening to the patient’s
complaints and physical examination play a key role in the
diagnosis, Assoc. Dr. Seyit Ali Gümüştaş explained the 5
prominent symptoms of bone tumors and made important
warnings and suggestions.
Genellikle sinsice ilerleyen ve her yaşta görülebilen kemik
tümörlerinde, özellikle pandemi sürecinde Covid-19
endişesiyle hastaneye gitmeye çekinilmesi erken tanı
imkanının kaçırılmasına neden olabiliyor. Acıbadem Fulya
Hastanesi Ortopedi ve Travmatoloji Uzmanı Doç. Dr. Seyit
Here are 5 signals of Bone Tumor!
Pain that does not go away
It is the most common reason for admission in bone tumors.
In malignant tumors, pain that responds to painkillers
for a short time, does not go away, and even increases in
severity develops. While the pain, which is mild at first, gets
worse over time, this problem continues at rest. Although
pain awakening at night is an indicator of malignant bone
tumors, it can also rarely be seen in benign tumors (osteoid
osteoma).
Nisan - April 2022
35
Swelling
Like persistent pain, swelling should also suggest the
possibility of a bone tumor. Swelling or stiffness in the
painful area is a common symptom of a bone tumor, be
it benign or malignant. The patient develops irregularly
limited, hard, immobile and rapidly growing swelling,
especially around the knee, shoulder and hip. The fact
that the swelling is painful should be a warning in terms
of being malignant, and it should not be overlooked with
painkillers, and a physician should be consulted as soon as
possible.
Limping
Since malignant tumors originating from both the bone
itself and another organ and spreading to the bone often
involve the hip and knee circumference, patients may apply
to the physician with the complaint of limping. The reason
for this complaint is weakness and pain due to destruction
of the involved bone. For this reason, in cases of possible
limping, one should be cautious, and a physician should
be consulted by not neglecting lameness that does not
develop due to any external factor.
Broken
Although it is not as common as pain and swelling, both
benign and malignant bone tumors can spontaneously
cause bone fractures. Both can cause destruction in the
bone where they are placed. The feeling of pain before,
the presence of mild trauma, the patient’s known history
of cancer, and the presence of destruction in the fractured
bone should suggest the possibility that the fracture may
have developed due to the tumor.
Topallama
olabilir. Öncesinde ağrı hissedilmesi, hafif travma ile
oluşu, hastanın bilinen kanser hikayesinin olması ve kırılan
kemikte yıkım varlığı, kırığın tümöre bağlı gelişebildiği
ihtimalini akla getirmelidir.
Numbness and weakness
Especially in spinal tumors, depending on the region where
the tumor is located, the patient may apply to the physician
with numbness and weakness in the arms or legs, while
constipation may be the only complaint in the tumor of the
coccyx. However, complaints such as weakness, fatigue,
loss of appetite, and weight loss involuntarily should
not be neglected, and the underlying cause should be
investigated. In addition to all these complaints, benign
bone tumors are usually detected incidentally on an X-ray
taken for another reason, while metastatic bone tumors
can often be detected in the routine screening of cancer
patients.
Spring months are ideal for rhinoplasty!
Burun estetiği için bahar ayları ideal!
Ear Nose and Throat Specialist
Dr. Oğuzhan Oğuz
With the arrival of spring, the increase in
aesthetic concerns also increases the demands for
surgical operations. Stating that they observed
an increase in the demand for rhinoplasty in the
summer months with the desire to look beautiful,
Dr. Oğuzhan Oğuz stated that the spring months
are also the ideal time for healing processes.
Research shows that people’s desire to “renew” increases
with the arrival of spring. Seasonal changes closely affect
people’s spiritual and mental structures. People are making
decisions faster for self-confidence-boosting operations,
with the desire to achieve a new look. Stating that there
is an increase in demand especially in rhinoplasty, Dr.
Oğuzhan Oğuz said, “With the approach of summer,
physical concerns come to the fore. Rhinoplasty decisions
that are postponed during the winter months turn into a
serious demand in the spring months. Stating that spring
is the best time for rhinoplasty, Ear Nose and Throat
Specialist Dr. Oğuzhan Oğuz said, “After the operation,
the skin should not be exposed to serious sun rays. For
this reason, we can say that the spring months are the
most ideal times during the healing process.”
Non-personalized nose surgery causes
psychological problems!
Listing the points to be considered in rhinoplasty, Dr.
Oğuzhan Oğuz said, “Each person’s facial anatomy
differs. For this reason, aesthetic nose surgeries need
to be done individually. We encounter a lot of patients
who have undergone an operation, in which the person
is more unhappy afterwards. Patients coming for
revision surgery face many psychological problems
with their noses that are not custom made. For this
reason, it is necessary to establish a good patientdoctor
relationship, and to manage the communication
between what the patient wants and what can happen.
Here, the importance of choosing the right doctor comes
into play.”
Healthy breathing is possible with rhinoplasty!
Stating that in addition to aesthetic concerns, rhinoplasty
is performed for healthy breathing through the nose, Dr.
Oğuzhan Oğuz said, “Healthy breathing is very important
for physical health and quality of life. Although many
of our patients are interested in the aesthetics of
their noses, it is essential that the patient’s breathing
is correct in a well-performed rhinoplasty. After
rhinoplasty, people should leave the hospital happy both
aesthetically and in terms of quality of life.”
çıkıyor. Kış aylarında ertelenen burun estetiği kararları
bahar aylarında ciddi bir talebe dönüşüyor” diye konuştu.
Bahar aylarının burun estetiğinde en uygun zaman
olduğunu da belirten Kulak Burun Boğaz Uzmanı Dr.
Oğuzhan Oğuz, “Operasyon sonrasında cildin ciddi güneş
ışınlarına maruz kalmaması gerekiyor. Bu sebeple iyileşme
sürecinde bahar ayları en ideal zamanlar diyebiliriz” diye
konuştu.
Kişiye özel yapılmayan burun ameliyatı psikolojik
sorunlara yol açıyor!
Burun estetiğinde dikkat edilmesi gerekenleri sıralayan
Dr. Oğuzhan Oğuz, “Her kişinin yüz anatomisi farklılık
What to do when your child loses a tooth?
Çocuğunuz dişini kaybettiğinde ne yapmalı?
Losing teeth is a natural process in childhood. This
stage is a milestone in your child’s growth. The
falling out of the milk teeth is important to open
up enough space while the permanent teeth take
their place in the mouth. Dentist Pertev Kökdemir
gave 3 tips to help your child cope with the period
of tooth change.
1-Relax your child:
Children tend to get excited at the thought of losing their
first tooth, but they are not completely ready for the
pain. When the process begins and the first tooth falls
out, comfort your child, tell him everything is fine. During
this period, it would be useful to talk to him about the
importance of oral hygiene.
Diş kaybetmek, çocukluk çağında doğal bir
süreçtir. Bu aşama, çocuğunuzun büyümesini
gösteren bir dönüm noktasıdır. Süt dişlerinin
düşmesi, kalıcı dişler ağızda yerini alırken yeterli
alanın açılması için önemlidir. Diş Hekimi Pertev
Kökdemir, çocuğunuzun diş değişim dönemi ile
başa çıkmasına yardımcı olacak 3 ipucu verdi.
1-Çocuğunuzu rahatlatın:
Çocuklar ilk dişlerini kaybetme fikrinden heyecan duymaya
eğilimlidirler, ancak acıya tamamen hazır değillerdir. Süreç
başladığında ve ilk diş düştüğünde çocuğunuzu rahatlatın,
ona her şeyin yolunda olduğunu söyleyin. Bu dönemde ağız
hijyeninin önemi hakkında da onunla konuşmanız faydalı
olacaktır.
2-Take appropriate measures to alleviate their
suffering:
During this period, the gums may become sensitive, the
tissue holding the tooth may swell and begin to ache.
When this happens, you can try to relieve the pain using an
ice pack, or you can also consult your dentist and give mild
pain relievers.
3-Know the right time to get professional help:
In some children, the tooth replacement process may be
more painful or even delayed. If your child has not started
to change their teeth at the age of 8, if they complain of
pain and excessive bleeding, you should definitely consult
a dentist.
The process of changing milk teeth is one of the most
important growth stages in your child’s life as it paves the
way for a beautiful adult smile. Normally, permanent teeth
take their place in the mouth at the age of 6 and lasts until
the age of 12. However, this varies from child to child.
Dentist Pertev Kökdemir
2-Acılarını hafifletmek için uygun önlemleri alın:
Bu dönemde, diş etleri hassaslaşabilir, dişi tutan doku
şişebilir ve ağrımaya başlayabilir. Bu olduğunda, bir buz
torbası kullanarak ağrıyı hafifletmeyi deneyebilir veya diş
hekiminize danışarak hafif ağrı kesiciler de verebilirsiniz.
3-Profesyonel yardım almak için doğru zamanı
bilin:
Bazı çocuklarda diş değişim işlemi daha ağrılı hatta
geç olabilir. Çocuğunuz 8 yaşında dişlerini değiştirmeye
başlamadıysa, ağrı şikâyeti varsa ve aşırı kanamadan
şikâyet ediyorsa mutlaka bir diş hekimine başvurmalısınız.
Süt dişlerinin değişim süreci, güzel bir yetişkin
gülümsemesinin yolunu çizdiği için çocuğunuzun
hayatındaki en önemli büyüme aşamalarından biridir.
Normalde daimî dişlerin ağızda yerini alması 6 yaşında
başlar ve 12 yaşına kadar sürer. Ancak bu durum çocuktan
çocuğa değişir.
Pay attention to these signs in the eyes!
Gözlerdeki bu belirtilere dikkat!
Gözde aşırı sulanma, çapaklanma, kirpiklerde
yapışma gibi belirtiler ile kendini gösteren
gözyaşı kanal tıkanıklığı tedavi edilmediği
takdirde gözde iltihaplanmaya kadar giden
sağlık sorunlarına neden olabiliyor. Bebeklerde
ve yetişkinlerde de görülebilen gözyaşı kanal
tıkanıklığı hakkında bilgiler veren İEÜ Medical
Park Hastanesi Göz Sağlığı ve Hastalıkları
Bölümü’nden Prof. Dr. Hülya Gökmen, gözyaşı
kanal tıkanıklığının tedavi edilmediği takdirde
daha büyük sorunlara yol açabileceğini söyledi.
Prof. Dr. Hülya Gökmen, Department of Eye Health and
Diseases, IEU Medical Park Hospital
If the lacrimal duct obstruction, which manifests
itself with symptoms such as excessive watering
in the eye, burring and sticking of the eyelashes,
is not treated, it can cause health problems up to
inflammation in the eye. Informing on tear duct
obstruction, which can also be seen in infants
and adults, Prof. Dr. Hülya Gökmen, Department
of Eye Health and Diseases, IEU Medical Park
Hospital said that if the tear duct obstruction is
not treated, it can lead to bigger problems.
Bozukluk
Stating that the tear system must also be healthy in order
for the eyes to work healthily and smoothly, Gökmen said,
“Tears are secreted by the lacrimal gland and auxiliary tear
glands located in the upper outer part of the eye socket
and come to the surface of the eye. It constantly cleans the
eye surface, provides a smooth optical surface and lubricity.
It then goes to the nasal cavity through the tear duct and
then to the stomach. As a result of excessive secretion of
tears or the failure of the tear duct to work well, watering
occurs in the eye. In eye watering due to disorders in the
tear duct, it is first necessary to reveal the location and
cause of the problem. Is there a blockage anywhere in the
canal, or is the pump function not working even though
the canal is intact, is there a problem with the nose? Was
the disorder congenital or acquired? After the patient is
examined in detail, it is necessary to search for a correct
answer to these questions by performing some tear
tests, depending on the situation.”
Eye infections may develop
Informing on the symptoms of tear duct obstruction,
Gökmen said, “In these patients, there is excessive
watering and burrs in the eyes. Sometimes it can even
blur vision. There may be redness in the eye due to
sticking of the eyelashes and constantly wiping the tears.
Sometimes swelling and redness can be seen in the
lacrimal sac and nasal root. Because the canal does not
work properly, eye inflammations develop very often.
Therefore, it is important for eye health to be treated.”
It is divided into two groups
Indicating that patients should be separated as
congenital or acquired canal obstructions in terms of
treatment, Gökmen said, “These two groups require
completely separate approaches. Some of the congenital
ones can be corrected without surgery, with some
methods such as massage, without harming eye health.
However, this is determined by examination. Depending
on the age of the child and the characteristics of the
disease, the type of treatment and surgery is decided.
In adults, most patients require surgery. Although
there are various techniques, the main procedure is to
generate another path to the nasal cavity before the duct
is blocked. General anesthesia is essential in children. In
adults, it can be applied under both general and local
anesthesia, depending on the patient’s preference.”
The prescription for hypertension: Physical activity and exercise!
Hipertansiyonun reçetesi: Fiziksel aktivite ve egzersiz!
Orta ya da yüksek yoğunlukta egzersiz yapan
bireylerde kalp hastalıklarına bağlı ölüm
oranları %15 daha düşüktür. Dengeli beslenme
ve sağlıklı yaşam tarzında hipertansiyon
hastalarına en az 30 dk süreyle ve haftada 5-7
gün arasında olacak şekilde orta yoğunlukta
dinamik aerobik egzersizler (yürüme, bisiklete
binme, yüzme) önerilmektedir. Yeni Yüzyıl
Üniversitesi Gaziosmanpaşa Hastanesi Kardiyoloji
bölümünden, Dr. Ört. Üyesi Mert Sarılar
‘Hipertansiyon hastalarında egzersizin önemi’
hakkında bilgiler verdi.
Death rates due to heart diseases are 15% lower
in individuals who exercise at moderate or high
intensity. In a balanced diet and healthy lifestyle,
moderate-intensity dynamic aerobic exercises
(walking, cycling, swimming) are recommended
for at least 30 minutes and 5-7 days a week for
hypertension patients. Dr. Lecturer Mert Sarılar,
the Department of Cardiology, Gaziosmanpaşa
Hospital of Yeni Yüzyıl University, gave
information about ‘The importance of exercise in
hypertension patients’.
Cardiovascular diseases are the most important cause
of death worldwide. Therefore, early diagnosis and
appropriate treatment of cardiovascular diseases play
a major role in survival. Hypertension is also among
the important risk factors for the development of
cardiovascular disease. In a study conducted in 2015,
approximately 1.13 billion people in the world are thought
to have hypertension, and this number is expected to
increase to 1.5 billion in 2025. By definition, hypertension
is a systolic blood pressure of 140 mmHg or more or a
diastolic blood pressure of 90 mmHg or more.
Hypertension treatment includes two basic components:
lifestyle changes and drug therapy. Lifestyle change and
exercise undoubtedly reduce blood pressure in patients,
but the majority of patients require additional drug
therapy. systolic studies It has been shown that the risk
of death is reduced by 10-15% with a decrease in blood
pressure of more than 10 mmHg and a decrease in diastolic
blood pressure of more than 5 mmHg.
Lifestyle changes include limiting salt in the diet (maximum
5 g sodium intake per day), quitting smoking and alcohol,
eating a Mediterranean type diet rich in fresh vegetables
and fruits, brisk walking for 1 hour at least 5-7 days a week,
and weight control.
With exercise, a sudden rise
in systolic blood pressure
occurs first, and then, with a
rapid decrease, blood pressure
returns to normal levels. Various
observational studies have
shown that regular aerobic
exercise is beneficial in the
prevention and treatment of
hypertension. In this way, it even
reduces the cardiovascular risk.
Exercise types are grouped
in 3 categories as aerobic,
static-stretching and
resistance
Aerobic exercises are endurance exercises in which
large muscle groups participate. Exercises that increase
oxygen consumption such as walking, running, cycling
and swimming are aerobic exercises. Resistance exercises
(lifting weights, etc.) are exercises to increase muscle
strength and endurance. Static stretching (Isometric)
exercises are performed by bringing the body to a certain
position so that the muscle group is stretched.
Aerobic endurance exercises reduce resting systolic blood
pressure by 3.5 mmHg and diastolic blood pressure by 2.5
mmHg. In dynamic resistance exercises, a decrease of 1.8
mmHg in systolic blood pressure and 3.2 mmHg in diastolic
blood pressure is observed. In static stretching exercises,
it was shown that there was a 10.9 mmHg decrease in
systolic blood pressure and a 6.2 mmHg decrease in
diastolic blood pressure. However, studies in which these
benefits are observed may have scientific limitations as
the data were obtained by looking at individuals’ own
measurements. Endurance exercises reduce blood pressure
more significantly, especially in patients with hypertension,
compared to other exercise types (8.3 mmHg in systolic
blood pressure, 5.2 mmHg in diastolic blood pressure).
Unlike exercise type, exercise intensity also shows
differences on blood pressure and heart health. For
example, low-intensity and short-term exercises lower
blood pressure less than moderate or high-intensity
exercises. Individuals doing moderate or high-intensity
exercise had a 15% lower mortality rate due to heart
disease. Based on this study, moderate-intensity dynamic
aerobic exercises (walking, cycling, swimming) for at least
30 minutes and 5-7 days a week are recommended for
hypertension patients. In addition, resistance exercises
2-3 days a week are also recommended for hypertension
patients. The preventive effect of isometric exercise type
on heart disease and its effect on blood pressure has not
been clearly reported.
The positive effect of exercise on blood pressure and
heart health has been demonstrated by many scientific
studies, and regular exercise is recommended for all
patients, whether they have hypertension or not. However,
especially in patients whose blood pressure cannot be
controlled with exercise and lifestyle changes, long-term
drug therapy and a specialist physician examination are
essential.
egzersizlerdir. Statik germe (İzometrik)
egzersizleri kas grubunun gerilmesini
sağlayacak şekilde vücudu belli
pozisyona getirilerek yapılır.
Aerobik dayanıklılık egzersizleri
istirahat sistolik kan basıncında 3.5
mmHg, diyastolik kan basıncında 2.5
mmHg düşüş sağlar. Dinamik direnç
egzersizleri sistolik kan basıncında
1.8 mmHg, diyastolik kan basıncında
3.2 mmHg düşüş izlenir. Statik
The stars of exports in the chemical industry received their awards
Kimya sektöründe ihracatın yıldızları ödüllerine kavuştu!
Türkiye’nin en fazla ihracat gerçekleştiren
ikinci sektörü olan kimyada 2021 yılının ihracat
yıldızları belli oldu. İstanbul Kimyevi Maddeler
ve Mamulleri İhracatçıları Birliği’nin (İKMİB)
her yıl düzenlediği “İKMİB İhracatın Yıldızları
Ödül Töreni”, bu yıl 21 Mart 2022 tarihinde
gerçekleştirildi. 2021 İKMİB İhracatın Yıldızları
Ödül Tören’inde 34 kategoride 170 ödül
sahiplerini buldu.
The export stars of 2021 have been determined
in chemistry, which is Turkey’s second largest
exporting sector. The “IKMIB Stars of Export
Award Ceremony”, organized annually by the
Istanbul Chemicals and Products Exporters’
Association (IKMIB), was held on March 21, 2022
this year. At the 2021 IKMIB Stars of Export
Award Ceremony, 170 awards were found their
owners in 34 categories.
Turkey’s second largest exporting sector, chemistry, broke
a new record with its exports of 25.4 billion dollars in
2021. The sector’s exports grew by 39 percent in 2021.
Representing more than 8,500 active exporting companies,
İKMİB realized 58.75 percent of Turkey’s total chemical
exports.
The chemical industry, which plays a strategic role in the
economy by supplying raw materials or semi-finished
products to other sectors, along with 16 sub-sectors from
plastics, paints, cosmetics to pharmaceuticals, provides a
great added value to the country’s economy with its 11.2
percent share in total exports. According to the data for
the first three quarters of 2021, world chemical exports
amounted to $3.07 trillion, while the Turkish chemical
industry ranked 29th with a 0.76 percent share in global
industry exports.
The 2021 IKMIB Stars of Export Award Ceremony, which
was organized for the seventh time this year by IKMIB in
yılı ilk üç çeyrek verilerine göre, dünya kimya ihracatı 3,07
trilyon dolar olurken, Türk kimya sektörü küresel sektör
ihracatında yüzde 0,76 pay ile 29’uncu sırada yer aldı.
Kimya sektöründeki üye firmalarını başarılı ihracatlarından
dolayı onurlandırmak ve teşvik etmek amacıyla İKMİB
tarafından bu yıl yedincisi düzenlenen 2021 İKMİB
İhracatın Yıldızları Ödül Töreni, 21 Mart 2022 tarihinde
gerçekleştirildi. İKMİB’in Youtube kanalından da canlı
54
order to honor and encourage its member companies in
the chemical industry for their successful exports, was held
on March 21, 2022. The ceremony, which was broadcast
live on IKMIB’s Youtube channel, was attended by Rıza Tuna
Turagay, the Turkish Deputy Minister of Trade, Mehmet
Fatih Kacır, the Turkish Deputy Minister of Industry and
Technology, İsmail Gülle, Chairman of Turkish Exporters
Assembly (TIM), Adil Pelister, Chairman of the Board of
IKMIB and the representatives of the awarded companies.
At the Award Ceremony, 170 companies that were in the
top 5 in a total of 34 categories in sub-sectors and product
groups of chemistry were entitled to receive awards.
“The chemistry sector clearly and clearly
demonstrates that it is one of the leading sectors
of Turkey at PMI”
Speaking at the award ceremony, Rıza Tuna Turagay, the
Turkish Deputy Minister of Trade, reminded that the yearend
target of the country’s exports is 250 billion dollars
and said, “Our exports are increasing. We closed 2021 with
11 percent growth. We have become one of the countries
with the highest growth rate in the world. When we look at
the PMI figures, we come across the chemical industry and
the plastics industry. Our chemistry and plastics industry
clearly demonstrates that it is one of the leading industries
in Turkey at PMI. The chemical industry finished last year
with an export of 25.4 billion dollars. As of March 20, it
reached $6.4 billion in 3 months this year. We are talking
about an increase of 37.1 percent. While Turkey’s total
exports increased by 32.9 percent last year, our chemical
industry increased its exports by 38.8 percent. When we
look at this year, while our exports increased by 21.4 in
the first two months, our chemical industry increased its
exports by 37 percent. There are high increases in raw
material prices and high increases in commodities, but
we still achieve success thanks to you. The industry also
has a structure that is 75 percent dependent on imports.
For this, investments that will open the future of Turkey
such as industrial zones, super incentives, petrochemical
investments are important. Our share in world exports is 5
per thousand. We need to raise this much higher. We need
to work harder and invest more. We still have a long way
to go in the chemical industry. The Chemical Technology
Center is an important breakthrough, an important step.”
Mehmet Fatih Kacır, Türkiye İhracatçılar Meclisi (TİM)
Başkanı İsmail Gülle, İKMİB Yönetim Kurulu Başkanı
Adil Pelister ve ödül alan firma temsilcileri katıldı. Ödül
Töreni’nde kimyanın alt sektör ve ürün gruplarında toplam
34 kategoride ilk 5’e giren 170 firma ödül almaya hak
kazandı.
“Kimya sektörümüz PMI’da Türkiye’nin öncü
sektörlerinden biri olduğunu açık ve net olarak
ortaya koyuyor”
Ödül töreninde konuşan T.C. Ticaret Bakan Yardımcısı Rıza
Tuna Turagay, ülke ihracatının yıl sonu hedefinin 250 milyar
dolar olduğunu hatırlatarak şunları söyledi: “İhracatımız
artıyor. 2021 yılını yüzde 11 büyüme ile kapattık. Dünyada
en yüksek büyüme hızına ulaşan ülkelerden biri olduk. PMI
rakamlarına baktığımız zaman karşımıza kimya sektörü
ve plastik sektörü çıkıyor. Kimya ve plastik sektörümüz
PMI’da Türkiye’nin öncü sektörlerinden biri olduğunu açık
ve net olarak ortaya koyuyor. Kimya sektörü geçen yılı 25,4
milyar dolarlık ihracatla tamamladı. Bu sene 3 ayda 20 Mart
itibariyle 6,4 milyar dolara ulaştı. Yüzde 37,1’lik artıştan
bahsediyoruz.
“Kimya Teknoloji Merkezi projesini destekliyoruz”
“We support the Chemistry Technology Center
project”
Expressing that the chemical industry is a very important
and critical industry in his speech, Mehmet Fatih Kacır, the
Turkish Deputy Minister of Industry and Technology, said,
“The chemical industry is a very important industry all over
the world. There is no developed country that has been
successful without developing this sector. Because we are
talking about a sector that makes critical contributions
to the development of many sectors and that provides
77 of its 100 products as input to other sectors. We are
aware of this importance and we carry out our work with
this understanding. So far, we have worked very closely
Nisan - April 2022
56
with our Exporters’ Association and our stakeholders.
We designed the move program together. We identified
priority products and innovative technology areas together.
We will continue to pave the way for many R&D projects
and investments that will hopefully announce the results
of this program in the near future and will add value to
Turkey’s future. As the Ministry, we also prioritized the
chemical industry in our project-based support. Our support
will be at your side in the Chemistry Technology Center
project that we are working with. We will carry Turkey’s
National Technology Move targets first to 2023 and then to
much more advanced targets together.”
“As of today, our chemical industry has risen to
the first sector position”
Emphasizing that last year was an unforgettable year in
every sense, İsmail Gülle, Chairman of Turkish Exporters
Assembly (TIM) said, “We completed 2021 with a great
record. Now, we have an export target of 250 billion dollars
in front of us. Turkey has underlined its promise of being a
supply center due to the deterioration in the supply chain,
which we frequently mentioned last year, with an export
of 25 billion dollars. Turkey will be instrumental in many
good developments regarding this supply center in the
next period. We see that Turkey will be the shining star in
the post- pandemic world where the war and the regional
economy will be reshaped in the next period. The biggest
strength of this will be you, the exporters. Our chemical
industry was our second industry last year, when I look at
the figures this year, our chemical industry has risen to
the first sector position as of today. In the two and a half
months so far, the export performance of our chemical
industry really deserves a lot of applause. This achievement
İsmail Gülle, “2021’i büyük bir rekorla tamamladık.
Nisan - April 2022

is yours, I congratulate you wholeheartedly. From 3rd to
2nd, from 2nd to 1st… We are proud of this industry. Our
country had a share of over 1% in world trade for the first
time in 2021. So, our goal is to get a 1 percent share from
the world trade as the chemical industry. We believe that
we will achieve our export target of 50 billion dollars by
2030. The fact that our Ministry of Industry and Technology
has allocated such a building in the Information Valley to
our sector shows their confidence in the potential and
future of the sector. Thank you to our minister. Hopefully,
the research and developments to be made here will
lead to more value-added exports, which is our goal.
Therefore, it is a very important project, I congratulate it
wholeheartedly. We are proud of our chemical industry.
I congratulate all our exporters who will receive awards
today.”
“We are prepared for digital transformation”
Reminding that they published the 2020 award ceremony
online and on digital platforms due to the epidemic, Adil
Pelister, Chairman of the Board of IKMIB said, “In 2020, we
organized the first digital purchasing committee, the first
digital award ceremony, the first virtual fair organization
and the first digital platform with our Chemical Industry
Platform. We also organized our Chemistry Sector Council
and proved that we are prepared for digital transformation
in this way. Due to the ongoing epidemic, the last two
years have been very difficult for all of us. Despite all the
adverse conditions, we struggled hand in hand, shoulder
to shoulder and managed to adapt quickly to this difficult
process. Of course, we cannot ignore the support of our
state. I owe a debt of gratitude to our government and
public bureaucracy, especially to our President.”
era and a new order has emerged, Pelister said, “There
is no doubt that we, as a country, should be among the
playmakers of this new order with a total understanding. In
order for deep economic fluctuations to enter an upward
trend again and to gain a rapid upward momentum, we
need to be able to produce and export products with
high technology and high added value. As İKMİB, we hope
to implement our Chemical Technology Center project,
which we have been working on for two years, in order
to evaluate this fluctuation in favor of our industry and
our country. To our SME companies that want to export
and improve their exports with the SME support center,
which will reduce the sample costs of our exporters by
approximately 70 percent and save time, internationally
accredited reference laboratories, entrepreneurship and
start- up areas and crowd-funding organizations, which will
enable new inventions and stimulate industry and university
cooperation, I am honored to share with you the justified
pride of having signed a very important project that will
provide intelligence, index and trend support and open
its digital library and scientific publications to users with a
single click. I hope we will open it together again this year.”
“We aim to be the first on a sectoral basis with an
export of 50 billion dollars in 2030”
Stating that they have determined the “Vision 2030”
strategy for the new period in chemical exports, Pelister
said, “Our goal is to increase our exports in the chemical
industry to 50 billion dollars in 2030 and to become the
first on a sectoral basis. I have no doubt that we will achieve
this. As the Chemistry Sector Platform, we have rolled up
our sleeves to organize an international council within
the framework of the 2030 vision next year. By means of
this council, we will have created our 2023-2030 Chemical
Industry Strategy Document. Our whole aim and target is
2030. Because global institutions and organizations and our
state are also arranging their sustainability-based economic
development goals according to 2030 plans. United Nations
goals in this direction. With all of these projects, we will also
give İKMİB the prestigious Global Compact status that the
United Nations Development Program gives to reputable
institutions.”
bazda birincilik hedefliyoruz”
Kimya ihracatında yeni dönem için “Vizyon 2030” stratejisini
belirlediklerini ifade eden Pelister, “Hedefimiz 2030
