SPO/HMP IX Interface License Key Registration
Permanent Key Request Form
To obtain a permanent key, you must identify the UNIX server system on which Single Point Operations Console software
is installed. In the first table below, enter your server identification code in the Server ID column. You can find this code by
selecting About under the Help menu in the Single Point Operations Console software. If you are running Single Point
Operations Console on more than one server (for instance, on redundant servers), you must supply the server ID codes of
all Single Point Operations Console systems.
Enter the server host names and server IDs of your Single Point Operations Console server systems below.

Attention: Verify that your server ID values are transcribed correctly on this form!
Your permanent license key (or keys) will be bound to the server IDs identified on this form. You will receive one key for
each server. The license count authorized by the key will equal your total entitlement for SPO/HMP IX Interface software
as provided through your ClearPath IXS4xxx-xx contracted shipments.
