this directory do not guarantee that each physician in that group is under contract.

the FOCUS Preferred Provider Organization.

you may also contact FOCUS Customer & Provider Relations at 1-800-243-2336 for a
referral of a treating physician in your area.
How to Use This Directory

he directory consists of two sections beginning with Hospitals, followed by all other
Specialty Providers. Providers are sorted by specialty and then by city. In addition,
there is an alphabetic provider index by name at the end of the directory.
