Treatment Integrity Checklist—Demand Condition
Instructions: This checklist may be used for training purposes and for determining
treatment fidelity in implementation of functional analysis conditions. Place a √ beside the
relevant procedural step if correct; an X if incorrect; or an Ø (an omission or no response)
if it did not occur at all when it should have.
