Introduction<br />

<strong>The</strong> <strong>Dutch</strong> <strong>Cochrane</strong> <strong>Centre</strong> - Annual Report <strong>2010</strong><br />

<strong>The</strong> <strong>Dutch</strong> <strong>Cochrane</strong> <strong>Centre</strong><br />

Since October 1994 the <strong>Dutch</strong> <strong>Cochrane</strong> <strong>Centre</strong> is <strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong>’s<br />

reference centre for <strong>The</strong> Netherlands, Belgium (handled by the Belgian Branch in<br />

Leuven), Surinam and the Netherlands' Antilles. <strong>The</strong> basic ideas of <strong>The</strong> <strong>Cochrane</strong><br />

<strong>Collaboration</strong> of identifying all randomised trials, building specialised registers of<br />

these trials and producing and updating high-quality systematic reviews of the effects<br />

of all areas of health care, form the basis for its activities. Since 2008, <strong>The</strong> <strong>Cochrane</strong><br />

<strong>Collaboration</strong> also facilitates the preparation of systematic reviews about the accuracy<br />

of diagnostic tests. <strong>The</strong> <strong>Dutch</strong> <strong>Cochrane</strong> <strong>Centre</strong> collaborates with others to facilitate<br />

the preparation, maintenance and dissemination of systematic reviews about health<br />

care interventions and diagnostic tests.<br />

<strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong><br />

<strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong> is an international organisation that aims to help people<br />

make well-informed decisions about health care by preparing, maintaining and<br />

promoting the accessibility of systematic reviews of the effects of health care<br />

interventions and the validity of diagnostic tests. Systematic reviews are reviews<br />

regarding a well formulated clinical question that use systematic and explicit methods<br />

to identify, select and critically appraise relevant research, and to collect and analyse<br />

data from the studies that are included in the review. <strong>Cochrane</strong> reviews provide upto-date,<br />

comprehensive and unbiased summaries of the best available evidence of the<br />

benefits and harms of all types of health care interventions, including preventive,<br />

therapeutic, rehabilitative, educational and organisational interventions. <strong>The</strong>se<br />

reviews are published electronically in the <strong>Cochrane</strong> Library. <strong>The</strong> <strong>Cochrane</strong><br />

<strong>Collaboration</strong>’s basic idea of preparing and updating high-quality systematic reviews<br />

of the effects of health care has been readily accepted throughout the world since its<br />

start in 1993. More information about <strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong> can be found on<br />

www.cochrane.org.<br />

Main issues and activities in <strong>2010</strong><br />

In this report we present the activities of <strong>The</strong> <strong>Dutch</strong> <strong>Cochrane</strong> <strong>Centre</strong> in the year<br />

<strong>2010</strong>. <strong>The</strong> main issues were the following.<br />

- Thirty new protocols for a systematic review and 31 new systematic reviews with a<br />

<strong>Dutch</strong> speaking first author were published in the <strong>Cochrane</strong> Library. <strong>The</strong> number of<br />

active reviews and protocols with a <strong>Dutch</strong> speaking first author in <strong>The</strong> <strong>Cochrane</strong><br />

Library increased from 284 to 319.<br />

- We prepared four systematic reviews for the <strong>Dutch</strong> Health Care Insurance Board<br />

(College voor Zorgverzekeringen, CvZ) and one systematic review for the <strong>Dutch</strong><br />

Society of Paediatricians.<br />

- We contributed to a successful grant application for a series of systematic reviews<br />

for the European <strong>Centre</strong> for Disease Prevention and Control (ECDC), which was<br />

initiated by the KNCV Tuberculosis Foundation.<br />

- <strong>The</strong> Netherlands Trial Register (NTR), one of the first primary registers recognised<br />

by the WHO, now encompasses over 2700 trials.<br />

- We produced eleven summaries of new or updated <strong>Cochrane</strong> reviews for the<br />

Nederlands Tijdschrift voor Geneeskunde (<strong>Dutch</strong> Medical Journal).<br />

- In <strong>2010</strong>, four workshops “Developing a systematic review” were given. We<br />

organised a total of 23 courses and workshops about evidence-based medicine and<br />

systematic reviews. <strong>The</strong>re were over 850 participants.<br />

- <strong>The</strong> Continental Europe Support Unit ran one induction training session for <strong>The</strong><br />

South African <strong>Cochrane</strong> <strong>Centre</strong> in Cape Town, South Africa. In collaboration with<br />

the UK Support Unit we organised the third (two-day) workshop “<strong>Cochrane</strong><br />

Systematic Reviews of Diagnostic Test Accuracy” for authors of <strong>Cochrane</strong> reviews.<br />

- On December 14, we co-organised the tenth Annual <strong>Dutch</strong> <strong>Cochrane</strong> Contributors<br />

Meeting. This year’s topic was Publication Bias in Clinical and Laboratory Animal<br />

Research and prof. Kay Dickersin was the invited speaker. This masterclass was<br />

followed by a “Ruysch-lecture” of which dr. Gerben ter Riet was the host.<br />

- On January 1 st , Rob Scholten has been appointed as professor of Evidence-Based<br />

Medicine by the University of Amsterdam.<br />

- In October, the <strong>Cochrane</strong> <strong>Collaboration</strong> Steering Group appointed Dr. Menno van<br />

Leeuwen, member of the DCC’s Advisory Board, as Ombudsman.<br />

- In October, Rob Scholten retired from the <strong>Cochrane</strong> <strong>Collaboration</strong> Steering Group<br />

after having served for two terms (six years).<br />

- In October, Rob Scholten was appointed as Director of <strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong><br />

Trading Company (with Lorne Becker and Jon Deeks).<br />

- René Spijker joined the <strong>Cochrane</strong> Trial Search Coordinators’ Executive and Rob<br />

Scholten joined the <strong>Centre</strong> Directors’ Executive.<br />

- Rob Scholten joined the Monitoring and Evaluation (M&E) subgroup of the WHO<br />

Nutrition Guidance Expert Advisory Group (NUGAG), which the WHO Department of<br />

Nutrition for Health and Development (NHD) has formed for the development of<br />

WHO guidelines in the area of evidence-based guidelines for nutritional status<br />

assessment and monitoring and evaluation of nutrition programmes.<br />

To contribute to <strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong> to support evidence-based medicine.<br />

Activities<br />

2.1 To support and to stimulate the preparation of high quality <strong>Cochrane</strong><br />

systematic reviews about health care interventions and diagnostic test<br />

accuracy.<br />

2.2 To contribute to <strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong> and to maintain an adequate<br />

infrastructure for <strong>The</strong> <strong>Dutch</strong> <strong>Cochrane</strong> <strong>Centre</strong>.<br />

2.3 To identify scientific publications (evidence) about the efficacy of healthcare<br />

interventions.<br />

STAFF MEMBERS<br />

In <strong>2010</strong>, Annefloor van Enst (March 1 st ) and Esther van de Glind (June 14 th ) joined<br />

our team. Annefloor has an MSc in Health Sciences and works as a junior researcher<br />

for our Systematic Review Unit. In addition, she will work on a PhD thesis. Esther has<br />

interrupted her training as geriatrist and will conduct a PhD project regarding<br />

empirical methods for evidence-based geriatrics.<br />

Lotty Hooft was employed for 0.5 FTE with the Netherlands Trial Register, a project<br />

funded by ZonMw (the <strong>Dutch</strong> MRC).<br />

Lotty Hooft (0.1 FTE), Mariska Leeflang (0.1), Hans Reitsma (0.1) and Rob Scholten<br />

(0.1) partially worked for the Continental Europe Support Unit (CESU) funded by <strong>The</strong><br />

<strong>Cochrane</strong> <strong>Collaboration</strong>. <strong>The</strong> aim of CESU is to help <strong>Cochrane</strong> Review Groups based<br />

on the European continent with the implementation of <strong>Cochrane</strong> Diagnostic test<br />

accuracy reviews.<br />

<strong>The</strong> current staff consists of 0.7 FTE administrative staff and 6.1 FTE scientific staff<br />

(partially employed on external resources, mostly from commissioned reviews).<br />

Accomplishments in Relation to the Objectives for <strong>2010</strong><br />

OBJECTIVE 1<br />

To stimulate and support evidence-based medicine activities in the Netherlands to<br />

help professionals and consumers to make well-informed decisions about health care.<br />

Activity 1.1<br />

To support the use of evidence-based medicine in <strong>Dutch</strong> health care.<br />

Accomplishments<br />

- We prepared four systematic reviews for the <strong>Dutch</strong> Health Care Insurance Board (College<br />

voor Zorgverzekeringen, CvZ) and one systematic review for the <strong>Dutch</strong> Society of<br />

Paediatricians.<br />

- We produced a systematic summary of the value of diagnostic instruments for a guideline<br />

working group regarding autism spectrum disorders, chaired by the Trimbos Institute.<br />

- We contributed to a successful grant application for a series of systematic reviews for the<br />

European <strong>Centre</strong> for Disease Prevention and Control (ECDC), which was initiated by the<br />

KNCV Tuberculosis Foundation.<br />

- We contributed to a successful grant application for a project regarding the use of n of 1<br />

trials, led by the Vereniging Spierziekten Nederland.<br />

- We produced eleven summaries of new or updated <strong>Cochrane</strong> reviews for the Nederlands<br />

Tijdschrift voor Geneeskunde (<strong>Dutch</strong> Medical Journal).<br />

- For the Quality of Care Council (Regieraad Kwaliteit van Zorg) Rob Scholten participated<br />

in a working group that developed a guideline for guidelines.<br />

- Rob Scholten continued his membership of the Advisory Committee of Nederlands<br />

Kenniscentrum Farmacotherapie bij Kinderen (NKFK) and joined HARING (led by IQ<br />

Healthcare), a project that aims to produce tools for guideline developers.<br />

- Lotty Hooft, Mariska Leeflang and Rob Scholten joined a working group to develop and<br />

promote systematic reviews of laboratory animal studies.<br />

- Lotty Hooft and Esther van de Glind supported a guideline working group regarding<br />

resuscitation of elderly (chaired by VERENSO).<br />

- Miranda Langendam participated in the <strong>Dutch</strong> GRADE Working Group.<br />

- Rob Scholten attended two Invitational Conferences organised by WB-Zorg /<br />

Kwaliteitsinstituut voor de Gezondheidszorg CBO and IQ Healthcare) and two Working<br />

Conferences organised by Health Services Research in Europe and CvZ.<br />

- Staff members of the <strong>Dutch</strong> <strong>Cochrane</strong> <strong>Centre</strong> were advisors of and participated in the<br />

“EBRO-platform”, which consists of representatives of almost all <strong>Dutch</strong> entities that are<br />

involved with guideline development, and of various other scientific and advisory<br />

committees.<br />

To train medical professionals in evidence-based medicine.<br />

Accomplishments<br />

- In <strong>2010</strong>, we organised 23 courses and workshops about evidence-based medicine and<br />

systematic reviews. <strong>The</strong>re were over 850 participants. See page 15 for an overview.<br />

- For the second year we ran an EBM module for the Nederlands Kenniscentrum<br />

Verzekeringsgeneeskunde (KCVG).<br />

- We worked with various <strong>Dutch</strong> health care institutions (among which the<br />

Kwaliteitsinstituut voor de Gezondheidszorg CBO and various scientific societies) to run<br />

in-house courses on EBM and Evidence-based Guideline Development. See page 15 for<br />

an overview.<br />

To contribute to <strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong> to support evidence-based medicine.<br />

African <strong>Cochrane</strong> <strong>Centre</strong>.<br />

- <strong>The</strong> UK and Continental Europe Support Units organised the third (two-day) workshop<br />

“<strong>Cochrane</strong> Systematic Reviews of Diagnostic Test Accuracy” for authors of <strong>Cochrane</strong><br />

reviews. <strong>The</strong>re were 37 participants.<br />

- In collaboration with many other Cochranites, staff members of the DCC ran various<br />

workshops regarding Diagnostic test accuracy reviews at the Colloquium in Keystone (15-<br />

50 participants per workshop).<br />

- For the <strong>Cochrane</strong> Diagnostic Test Accuracy Editorial Team we peer-reviewed over 30<br />

protocols and/or reviews.<br />

- On December 14, we co-organised the tenth Annual <strong>Dutch</strong> <strong>Cochrane</strong> Contributors<br />

Meeting. This year’s topic was Publication Bias in Clinical and Laboratory Animal Research<br />

and prof. Kay Dickersin was the invited speaker. This masterclass was followed by a<br />

“Ruysch-lecture” of which dr. Gerben ter Riet was the host. <strong>The</strong>re were 93 participants.<br />

- We answered on average 5-10 queries per week by phone or e-mail. Approximately 40<br />

persons visited the <strong>Dutch</strong> <strong>Cochrane</strong> <strong>Centre</strong> for consultation.<br />

the <strong>Dutch</strong> <strong>Cochrane</strong> <strong>Centre</strong>.<br />

Accomplishments<br />

- To promote <strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong> and evidence-based medicine we published 44<br />

articles (page 31) and gave 25 presentations (page 13).<br />

- Staff members of the DCC participated in 66 <strong>Cochrane</strong> entities (see page 35).<br />

- <strong>The</strong> Netherlands Trial Register encompasses over 2700 trials.<br />

- Lotty Hooft was Chair of the WHO Working Group on Best Practice for Clinical Trials<br />

Registers (http://www.who.int/ictrp/network/bpg/en/index.html).<br />

- Lotty Hooft continued to participate in an EU project regarding the development of an<br />

European Health Technology Assessment network (EUnetHTA).<br />

- Rob Scholten joined the Monitoring and Evaluation (M&E) subgroup of the WHO Nutrition<br />

Guidance Expert Advisory Group (NUGAG). This Group’s remit is to develop evidencebased<br />

WHO guidelines for nutritional status assessment and monitoring and evaluation<br />

of nutrition programmes.<br />

- Mariska Leeflang continued her co-convenorship of the <strong>Cochrane</strong> Screening and<br />

Diagnostic Tests Methods Group.<br />

- In October, the <strong>Cochrane</strong> <strong>Collaboration</strong> Steering Group appointed Dr. Menno van<br />

Leeuwen, member of the DCC’s Advisory Board, as Ombudsman.<br />

- In October, Rob Scholten retired from the <strong>Cochrane</strong> <strong>Collaboration</strong> Steering Group after<br />

having served for two terms (six years).<br />

- In October, Rob Scholten was appointed as Director of <strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong><br />

Trading Company (with Lorne Becker and Jon Deeks).<br />

- René Spijker joined the <strong>Cochrane</strong> Trial Search Coordinators’ Executive and Rob Scholten<br />

the <strong>Centre</strong> Directors’ Executive.<br />

- Upon request of various foreign colleagues we assessed <strong>Dutch</strong> articles. In addition, we<br />

retrieved various <strong>Dutch</strong> articles and reports that were not readily available outside the<br />

Netherlands.<br />

- We maintain a database of over 1800 individuals who have shown interest in the<br />

activities of <strong>The</strong> <strong>Cochrane</strong> <strong>Collaboration</strong>. We have sent out regular mailings, including the<br />

<strong>Cochrane</strong> newsletter and information regarding the <strong>Cochrane</strong> Colloquium.<br />

- Our website was kept up-to-date and was visited over 26,000 times.<br />
