Make YourWishes AboutYou (My Way)By Elizabeth Anderson,LCSW, DSW and Dale Lupu, MPH, PhDWhen I was first told I hadchronic kidney disease (CKD),I was grateful that there werelifesaving treatments, but I was also scaredbecause I didn’t have a plan. I worried thatI might be in pain or become a burden tomy family. I felt better when I was able todiscuss my desires and healthcare decisionswith my family and healthcare team.What kind of medical care would you wantif you were too ill to express your wishes?Like most people, you may not have thoughtabout it, or you may have a difficult timediscussing it with your family and kidney careteam. However, planning for your healthcarein the event that you cannot speak foryourself may help decrease your worries. Thisprocess is called advance care planning.Some people know exactly what they wouldwant to happen if they became very ill. Forother people, these decisions take time andthought. It helps to talk to a friend or familymember and to ask your kidney care team tohelp you make a plan and put it in writing.The Coalition for Supportive Care forKidney Patients has outlined five steps tohelp you with your advance care planning:1. Choose someone to make healthcaredecisions for you if you get sick andcannot speak for yourself.2. Think about what kind of healthcareyou would want if you were unlikelyto get better. Discuss your wishes withyour family and friends.3. Write your wishes down in a legalform, known as an advance directive(sometimes called a “living will” or amedical power of attorney).4. Give a copy of your advance directive toyour healthcare agent and your kidneycare team. Continue talking to yourfamily about your wishes.5. Discuss with your kidney care team whena medical order form (usually calledPOLST or MOLST) might be right for you.The guide from the Coalition’s projectcalled Make Your Wishes About You (MyWay) is available for free. (http://go.gwu.edu/MyWayACP). The My Way guide iswritten especially for patients with kidneydisease. It guides patients through thesteps of advance care planning and giveshelpful worksheets and tips.You can begin to work on each of thefive steps for advance care planning byfollowing these detailed instructions fromthe My Way patient guide:STEP 1: Who is your healthcare agent?Choose someone—such as a trustedfamily member or friend—to act as yourhealthcare agent. This person will makehealthcare decisions for you if you get sickand cannot speak for yourself.• A healthcare agent has no authority inyour healthcare unless you are unable tomake decisions for yourself.• A healthcare agent has no power overany other part of your life (e.g., finances,will, etc.).• If you do not have a healthcare agent,then your kidney care team will usuallyask your next of kin.• A healthcare agent is only official if youcomplete an advance directive. Be sure toshare the advance directive with familyand your kidney care team so they knowwho to turn to if needed.STEP 2: What would you want peopleto know if you got sick and were notable to speak for yourself?Think about what kind of healthcare youwould want if you were unlikely to get better.If you got so sick that you could no longer domost of your daily activities, or if you werenear the end of your life, what would youprefer to be the focus of your care?• Staying alive at all costs, no matter thepain or discomfort involved.• Being as comfortable as possible, even ifit meant you might not live as long.• Not sure or it depends on the situation.Each person has a different view of whatthey would want at the end of life basedon his or her personal values and currenthealth status. Talk to your kidney careteam to learn more about the risks andbenefits of different treatments for you.STEP 3: How do I make my medicalwishes known?Write your wishes down in a form calledan advance directive (sometimes calleda “living will” or a medical power ofattorney). Your kidney care team can help.• An advance directive is a legal documentthat allows you to have control of yourhealthcare decisions if you are everunable to speak for yourself.• Use the form that is recognized in thestate where you live.
