� To examine existing health care delivery mechanisms to identify performance gaps
and develop strategies to bridge them
� To actively engage a wide range of stakeholders from the community, including the
Panchayat, in the planning process
� To identify priorities at the grassroots level and set out roles and responsibilities at the
Panchayat and block levels for designing need-based DHAPs
� To espouse inter-sectoral convergence approach at the village, block and district
levels to make the planning process and implementation process more holistic
Need Of District Health Action Plan:
� Effective implementation of the programme to achieve goal within the time line.
� Identify the gaps and take into action to fill the gaps within the Time Frame.
