Dosage considerationsThe presence of pain, inflammation, signs of delayed healing, undue tissuetension and problematic adherences are the primary indicators for treatment. Thetypical duration of 60 minutes is indicated. Frequency of one treatment per weekis a general guideline. Later stage care will typically be implemented for 6–12weeks.
Clinical ConsiderationsNon-threatening movement and loading during the healing process areinstrumental in decreasing the potential of functional loss. Movementensures that newly formed collagen fibers align in a manner that allows forunrestricted motion, and loading stimulates the tissue to form in ways thatwill support healthy function (e.g. the ability to meet tensional or loadbearingdemands). For example: with muscular injuries (following anappropriate rest period allowing the scar tissue to stabilize) the benefits ofcontrolled loading include improved alignment of regenerating myotubes,faster and more complete regeneration, and minimization of atrophy ofsurrounding myotubes (Jarvinen et al. 2007). In studies conducted ontendon (considered a form of fascia), controlled loading type exerciseresulted in a more normalized tendon structure, as gaged by thickness andby the reduction of hypoechoic areas (Ohberg et al. 2004, Boyer et al.2005). Movement also helps to reduce the potential of undue adhesionsthat can impact not only local tissues but also associated tissues and/orstructures. It is important to follow standard therapeutic movement andexercise protocols during the various stages of rehabilitation (e.g. passive,active assisted, active, non-weight bearing, weight bearing).
