Regularitiesof formation of the personal profiles of the systemic hemodynamicshave been determined in this category of patients. Afresh method of antihypertensive therapy is offered which isbased on establishment of relationship between the personalprofile of the systemic hemodynamics and the hemodynamicdrug effect. Thenew revised edition of the document is substantiallydifferent from the previous one; proposed is a fundamentallynew variant of the initial therapy of AH: insteadof a single drug therapy it is recommended nowto begin the treatment immediately with the use of atwo-drug combination. Selection of the drug combination,as it has been the case before, is based on thepresence of diseases associated with AH, as well as thestate of the hypertension-mediated organs, i.e., the organsinvolved in pathophysiological processes causedby AH. The Guidelines’ developers present a variety ofdifferent strategies to initiate and escalate BP-loweringmedication to improve BP control rates initiating treatmentwith different monotherapies and then sequentiallyadding other drugs until BP control is achievedalmost as a fundamental solution of the AH problem,but at the same time they complain that "Despite this,BP control rates have remained poor worldwide”. Thestrategy developers believe that "bad" doctors shouldbear responsibilities for the stated inadequacy: they arethose who are afraid of prescribing the proper dosageof the medical drugs and who are incapable of controllingBP medication; at the same time it is consideredthat patient adherence to treatment is low so thatnot all of them complete the course of their medicationin full [1].The effectiveness of the strategic approaches istreated in the Guidelines casually; in the opinion ofthe developers, there is a forceful argument in favor ofthe Guidelines’ correctness: “The evidence from RCTsdemonstrating that BP control can be achieved in mostrecruited patients, and that no more than 5 − 10% ofthese patients exhibit resistance to the selected treatmentregimen, suggests that ineffective drug therapy isnot the source of the problem.”[1] (i.e. we, the strategydevelopers, escape our responsibility). In other words,if you have competent physicians and an eligible patient’sadherence, you can rise the efficacy of the AHtreatment up to 90-95%!Among the other things, the mere proposal to doublethe amount of the used drugs is an evidence forIssue 16.
