Journal of the Arab Board of Health Specializations Vol. 20, No. 4, 2019قد تُنتقَد هذه الدراسة بسبب عدد المرضى المحدود، والواقع أن ذلكيعكس الصعوبة الحقيقية التي كنا نواجهها في استقطاب المرضىالمُرَشَّ حين لالستفادة من عمليات تجريف خثرات وبطانة الشرايين الرئوية،وهي العقبة التي تعاني منها غالبية المراكز المشابهة في الدول الناميةاألخرى، 14،13 فعلى صعيد المثال، أشار التقرير األول لخبرة المكسيكمؤخراً إلى إجراء تجريف خثرات وبطانة الشرايين الرئوية عند مريضٍ واحدفقط من أصل 50 مريضاً تم تشخيص إصابتهم بارتفاع التوتر الرئويالصمّي-الخثري المزمن. 15 لقد عُ زي ذلك بشكلٍ رئيسي إلى عدم معرفةأطباء الصدر والقلب بإمكانية تحقيق الشفاء من ارتفاع التوتر الرئويالصمّي-الخثري المزمن من خالل التداخل الجراحي، أو إلى مبالغتهم فيتقدير خطورة العمليات الجراحية، باإلضافة إلى عوامل أخرى محلية مثل5الممارسات الطبية المحلية وضعف توفّر الموارد الالزمة.يُشكّل نشر التوعية حول اإلصابة والخيارات العالجية المتاحة التحدّياألبرز أمام المتحمّسين للتطبيق األوسع لهذه العمليات، وقد يُفيد في ذلكاعتماد مراكز إقليمية متخصصة تقوم بتعزيز وتعميق الخبرة بتدبير ارتفاعالتوتر الرئوي الصمّي-الخثري المزمن، باإلضافة إلى رعاية األبحاثالمخبرية والسريرية الضرورية، ووضع التوصيات حول أفضل المقاربات5العالجية المُتاحة.االستنتاجاتتشير هذه المراجعة إلى النتائج المُرضية للخبرة المحليّة بعمليات تجريفخثرات وبطانة الشرايين الرئوية، والتي ترافقت مع معدّالت منخفضة منالوفيات ومع نتائج جيدة طويلة األمد من خالل خفض ضغوط الشرايينالرئوية وتحقيق تحسّ ن هام في األعراض واالنذار. نحن نعتقد بأن زيادةالوعي بانتشار هذه الحالة وبإمكانية تحقيق الشفاء الجراحي سوف تساهمفي إنقاذ أعدادٍ أكبر من المرضى المصابين بهذا المرض الموهن والقاتلفي نهاية المطاف.REFERENCES1. While a role for Th17 cells in promotinginflammation and autoimmune disorders has been extensively demonstrated, it is still controversial whether and howTh17 cells influence tumor immunity. In this study, we examined the proportion of Th17 cells and the concentrationof a related cytokine (IL-17) in peripheral blood from Syrian patients with breast cancer.Methods: The study included 69 patients with breast tumors. They were divided into 2 categories: patients affectedwith benign breast tumors (n=28) with age range (26-79) years, and breast cancer patients (n=41) with age range(22-74) years. Th17 cells were evaluated in peripheral blood by flow cytometry and reported as a percentage of totalCD4+ cells, and ELISA method has been used for estimation IL-17 levels in serum of studied groups. The presentstudy also assessed the association of cell percentage and IL-17 levels with clinico-pathological parameters.Results: Breast cancer samples had significantly higher concentrations of IL-17 compared with benign breasttumor samples, and this elevation was correlated with angiogenesis and metastasis. However, no significantdifference of Th17 cell percentages was found between the two studied groups. Similarly, no significant associationwas recognized between Th17 frequency and clinico-pathological factors.Conclusions: We observed significantly increased levels of IL-17 in serum of breast cancer patients, and asignificant association between these levels and both angiogenesis and metastasis.
