Solid Renal Mass<br />

Differential Diagnoses: Kidney<br />

(Left) Longitudinal ultrasound<br />

shows a large, homogeneous,<br />

hyperechoic renal mass <br />

similar in echogenicity to the<br />

renal sinus fat ſt. While<br />

suggestive of<br />

angiomyolipoma, confirmation<br />

with CT or MR is necessary.<br />

(Right) Axial T1 opposed-phase<br />

MR of the same patient shows<br />

multiple regions of low signal<br />

ſt within the mass indicating<br />

fat next to soft tissue. This<br />

confirms angiomyolipoma.<br />

Renal Angiomyolipoma<br />

(Left) Longitudinal ultrasound<br />

performed for hematuria<br />

shows a hypoechoic solid mass<br />

in the upper renal sinus ,<br />

which did not have detectable<br />

color flow. Absence of color<br />

flow does not exclude tumor,<br />

presence of color flow is<br />

suggestive of tumor. Unlike<br />

the column of Bertin, this mass<br />

did not arise from the cortex.<br />

(Right) Longitudinal<br />

ultrasound shows a solid renal<br />

pseudolesion extending<br />

from cortex to renal sinus. The<br />

echogenicity is identical to<br />

cortex.<br />

Upper Tract Urothelial Cancer<br />

(Left) Longitudinal color<br />

Doppler ultrasound shows a<br />

hyperechoic, wedge-shaped<br />

cortical lesion st with<br />

decreased internal color flow.<br />

(Right) Longitudinal<br />

ultrasound shows the left<br />

kidney (between calipers) with<br />

mild pelvic diltation st. A solid<br />

"mass" ſt projects from the<br />

lower pole. The "mass"<br />

resembles normal kidney. No<br />

kidney was seen in the right<br />

renal fossa.<br />
