Enlarged Kidney
Differential Diagnoses: Kidney
(Left) Longitudinal
transabdominal ultrasound
shows focal acute
pyelonephritis in a pregnant
woman. There is a wedgeshaped
area of increased
echogenicity ſt in the upper
pole. (Right) Longitudinal
ultrasound shows an enlarged
kidney ſt with a large solid
mass in the lower pole st.
Given the size of this mass,
renal cell carcinoma was
suspected but it was an
oncocytoma at surgery.
Acute Pyelonephritis
Primary Renal Tumors: Benign and
Malignant
(Left) Longitudinal ultrasound
shows an enlarged kidney
measuring 14 cm. The kidney
is increased in echogenicity
with increased prominence of
the pyramids ſt in this patient
with a contralateral, severely
hydronephrotic, poorly
functioning kidney. (Right)
Longitudinal ultrasound shows
a nonobstructed duplex kidney
with splitting of central sinus
echoes by a hypoechoic band
of tissue ſt. Duplex kidneys
are normal variants, which are
longer than normal kidneys.
Compensatory Renal Hypertrophy
Duplex Kidney
(Left) Longitudinal ultrasound
shows an enlarged (14 cm),
hyperechoic kidney in a
diabetic patient with fever,
flank pain, and positive blood
cultures. There is a large renal
abscess ſt with acoustic
enhancement. (Right)
Longitudinal ultrasound shows
the right kidney in a patient
with lupus nephritis, fever, and
bacteriuria. There is severe
hydronephrosis and cortical
thinning ſt. The calyces are
filled with low-level echoes
representing pus st.
Pyonephrosis
