Focal Splenic Lesion
Differential Diagnoses: Spleen
(Left) Longitudinal US shows
dense curvilinear calcification
of a longstanding splenic cyst
ſt in a patient with a remote
history of left upper quadrant
injury. CT (not shown)
revealed clear cystic content.
The features are suggestive of
a calcified pseudocyst. (Right)
Longitudinal US of the spleen
shows a thin-walled, anechoic
cyst ſt with thin internal
septa st and posterior
enhancement . This
appearance is nonspecific. At
histology, an epidermoid cyst
has an epithelial lining,
distinguishing it from other
splenic cysts.
Acquired Splenic Cyst
Congenital (Epidermoid) Cyst
(Left) Longitudinal
transabdominal US shows an
early splenic abscess ſt with
nonspecific, rounded, welldefined,
hypoechoic
appearance. (Right)
Longitudinal splenic US shows
a large, splenic cyst ſt with
internal debris , forming a
fluid level. Drainage revealed
purulent material.
Infected Cyst/Abscess
Pyogenic Abscess
(Left) Longitudinal
transabdominal US of the
spleen shows a rounded
splenic abscess with partial
liquefaction ſt. A thick
irregular rim of inflammatory
tissue remains in the
periphery. (Right) Color
Doppler US of the spleen
shows a large, irregularshaped,
avascular, and largely
anechoic collection in the mid
spleen, with internal debris
ſt. This was confirmed to
represent pyogenic abscess.
