Dilated Gallbladder
Differential Diagnoses: Biliary System
(Left) Longitudinal oblique
ultrasound of the gallbladder
shows an obstructing stone in
the neck ſt. The gallbladder is
distended with diffuse
hypoechoic wall thickening st
in this patient with acute
calculous cholecystitis. (Right)
Longitudinal oblique
ultrasound shows a distended
gallbladder with fundal stones
ſt and wall thickening st.
The patient had a positive
Murphy sign consistent with
acute calculous cholecystitis.
Acute Calculous Cholecystitis
Acute Calculous Cholecystitis
(Left) Sagittal CECT of the
same patient shows a
distended gallbladder ſt with
diffuse wall thickening st. The
stones were not visible on CT.
(Right) Transverse ultrasound
shows a gangrenous
gallbladder with no stones.
The wall is edematous st with
a focal defect anteriorly ſt. A
pericholecystic abscess had
developed (not shown).
Acute Calculous Cholecystitis
Acute Acalculous Cholecystitis
(Left) Oblique color Doppler
ultrasound of a distended
gallbladder filled with
avascular low level echoes st
with one small stone ſt. The
posterior wall is irregular .
The lumen was filled with
necrotic adenocarcinoma.
(Right) CECT shows a dilated
gallbladder with low density
contents. There is subtle wall
calcification ſt and internal
heterogeneity. This was
largely necrotic
adenocarcinoma with a small
invasive tumor in the
gallbladder neck.
