Failed First Trimester Pregnancy (Left) Longitudinal transvaginal ultrasound shows a large empty gestational sac with a MSD of 40 mm and poor decidual reaction. A yolk sac and embryo were not seen, compatible with anembryonic pregnancy. (Right) Longitudinal transvaginal ultrasound shows another example of an anembryonic pregnancy with an enlarged angular empty gestational sac showing an empty amnion st and cystic decidual change ſt. Diagnoses: Female Pelvis (Left) Transverse transvaginal ultrasound shows an irregular empty sac , poor decidual reaction, and cystic change st. Findings were suspicious for failed pregnancy; however, MSD was < 25 mm and followup was obtained. (Right) Follow-up transvaginal ultrasound in the same patient shows findings compatible with interval complete abortion. There is a thin endometrial strip with no residual endometrial contents ſt. Note small persistent decidual cysts st. (Left) Transvaginal ultrasound of a retroflexed uterus (fundus better seen in a different plane) shows a GS with a YS ſt abnormally positioned in the lower uterus just above the internal os, which is beginning to dilate st. Prognosis is guarded. (Right) Longitudinal transabdominal ultrasound performed for follow-up in the same patient shows interval pregnancy failure with enlargement of the GS ſt and development of marked cystic change st in the decidua. Though cardiac activity was not seen, the embryo is below < 7 mm, and follow-up was recommended.
