Approach to Sonography of the Female Pelvis 724 Diagnoses: Female Pelvis Imaging Anatomy Uterus The position of the uterus is variable and affected by the bladder filling, parity, fibroids, and bowel. Most commonly, the uterus lies in the midline with the fundus pointing towards the anterior abdominal wall and the external os pointing towards the rectum (anteversion and anteflexion). The opposite configuration (retroversion and retroflexion) is also commonly encountered and sometimes the uterus moves during the ultrasound exam. Both variants of uterine position are well imaged at endovaginal sonography because the transducer can be placed in the anterior or posterior vaginal fornices and close to the uterine serosa, allowing for high-frequency and high-resolution imaging. The uterus may also be in a neutral position, which is often the most difficult to image because the fundus points superiorly, away from the transducer. In this position, the fundus is at quite some depth from the vaginal fornix, making it difficult to penetrate with the higher frequencies of endovaginal transducers. In this situation, the patient or sonographer can apply pressure to the anterior abdominal wall to try and move the uterus to a favorable position. Ovaries The position of the ovaries is also variable and can be affected by the bladder filling, parity, uterine position, and pathology. The ovaries are generally found along the pelvic sidewall, adjacent to the external iliac vessels. The broad ligament, fallopian tube, and mesosalpinx are found between the ovary and uterus. These structures are often not seen as discrete anatomy when normal and are generally a heterogenous hyperechoic area containing variable-sized traversing vascularity. The ovaries are often found by scanning in the transverse plane and following the broad ligament laterally. During pregnancy, the ovaries move superiorly, out of the pelvis. In some women, they do not return to the ovarian fossa. Occasionally, an ovary will not be seen by transvaginal ultrasound, but will be well seen by transabdominal scanning because of a high position in the pelvis. In postmenopausal women, the ovaries may be difficult to identify because of a combination of atrophy, high position, &/or intervening bowel gas. In general, when the ovary is not identified, additional cross-sectional imaging can be avoided because it can be safely assumed that no significant mass exists since ovarian cancer does not present as a small ovarian mass. Imaging Protocols The pelvic ultrasound begins with a transabdominal exam typically using a curved low-frequency transducer. The patient is asked to fill her bladder prior to the exam such that the bladder displaces the small bowel and creates a sonographic window through which the uterus and adnexa are better seen. It should be noted that an overdistended bladder can push the uterus and ovaries far posteriorly and hinder evaluation. The patient is then asked to void in preparation for the endovaginal ultrasound. An endovaginal ultrasound routinely should be performed for all indications, though occasionally the anatomy is seen sufficiently by transabdominal imaging and an ultrasound can be avoided. In general, there are no absolute contraindications to endovaginal scanning, but it is generally not used in virginal women. Images are always acquired in the sagittal (longitudinal) and transverse planes. It is important to remember that depending on the angulation of the endovaginal transducer and lie of the uterus, the transverse view may be more in the coronal or coronal oblique plane with respect to the patient, rather than a true axial plane. The uterus should be measured and then evaluated for myometrial lesions such as fibroids (leiomyomas). The endometrium should be measured in the longitudinal plane perpendicular to the endometrial stripe, so as not to falsely elongate the true thickness. If fluid or blood is present in the endometrial cavity, it should be excluded from the measurement. The endometrium should also be evaluated for masses, cysts, or abnormal vascularity. The cervix should be imaged in two planes. Imaging of the vagina is not a routine part of pelvic ultrasonography, but can be performed transabdominally, endovaginally with minimal insertion of the transducer, or translabially. The ovaries should be scanned in two orthogonal planes and measured in three dimensions. Finally, the cul-de-sac should be imaged to evaluate for free fluid, hemorrhage, or masses. Anatomy-Based Imaging Issues Endometrium The endometrium varies significantly in thickness and echogenicity depending on the phase of the menstrual cycle. Immediately after menstruation, the endometrium is seen as a thin echogenic line (1-2 mm) representing the 2 coapted layers of endometrium. In the proliferative phase, under the influence of estrogen, the endometrium starts to thicken. In the late proliferative phase, just prior to ovulation (day 14), the endometrium has a distinctive trilaminar or striated appearance with alternating hyper- and hypoechoic lines. After ovulation, when progesterone is produced, the endometrium becomes thicker and hyperechoic, losing definition of the layers. During menstruation, the endometrium sloughs and fluid and mobile debris may be seen in the cavity. Ovaries The ovaries are also under the influence of the menstrual cycle and normal physiologic changes are commonly encountered at ultrasound and should not be mistaken for pathology. In the follicular phase (day 0-14), one or more dominant follicles develop. Ovulation occurs on day 14. The follicle then becomes a corpus luteum which either supports an early pregnancy or regresses. A developing follicle is typically a simple cyst less than three cm (although can occasionally be larger) and should not be mistaken for a pathologic cyst or cancer. Indications for Pelvic Ultrasound Clinical indications for pelvic ultrasound include the following: • Acute or chronic pelvic pain • Pelvic masses • Abnormal bleeding (dysmenorrhea, menorrhagia, amenorrhea) • Pregnancy (infertility, normal, complicated, postpartum) • Intrauterine device evaluation • Pelvic inflammatory disease • Difficult clinical exam of the pelvis • Congenital uterine anomalies • Endocrine abnormality (polycystic ovaries) • Screening for malignancy in high-risk patients
