Gonadal Stromal Tumors, Testis
(Left) Sagittal grayscale
ultrasound of the left testis in
this 18-year-old man
demonstrates a hypoechoic,
solid mass . (Right)
Corresponding color Doppler
ultrasound of the left testis in
the same patient shows
presence of marked internal
vascularity . Pathology
confirmed a Leydig cell tumor.
Diagnoses: Scrotum
(Left) Leydig cell tumor (LCT)
is often a well-circumscribed
mass with a homogeneous
yellow-tan cut surface. Focal
cystic change is present .
Hemorrhage or necrosis is
lacking. LCT often do not
replace the entire testis.
(Right) A well-circumscribed
Sertoli cell tumor with a tanwhite
firm cut surface ſt is
shown. The gross finding is
different from Leydig cell
tumor, which usually is tanbrown
to yellow due to high
lipid content.
(Left) Granulosa cell tumor
shows a well-circumscribed,
homogeneous, tan-white
nodule . The tumor is small
and, like many sex cord
stromal tumors, does not
extensively involve the testis.
Hemorrhage and necrosis are
lacking. (Right) Gross photo
shows a well-circumscribed,
unclassified sex cord stromal
tumor (SCST) . It has
heterogeneous, soft, tan
nodular areas with dense
white fibrotic septa. SCSTs
frequently do not replace the
entire testis.
