Extraovarian Adnexal Mass
Differential Diagnoses: Female Pelvis
(Left) Coronal ultrasound
shows a tender,
incompressible, thick-walled
tubular mass adjacent to
the normal right ovary ſt.
Low-level echoes are seen in
the lumen. (Right)
Longitudinal ultrasound of the
same patient confirms the
tubular structure to be blind
ending and traceable to
the cecum ſt, consistent with
an inflamed appendix. Mural
stratification st and
intraluminal debris are
noted.
Appendicitis
Appendicitis
(Left) This well-marginated,
pear-shaped mass has
echogenic contents with a
typical onion skin appearance
centrally, representing a
pelvic appendicular mucocele.
(Right) CT in the same patient
shows the mass to be
inseparable from the base of
the appendix/cecum ,
representing an appendicular
mucocele. Also note the
typical wall calcification ſt.
Appendicular Mucocele
Appendicular Mucocele
(Left) US demonstrates a solid
adnexal mass abutting the
ovary (physiological ovarian
cyst ſt). Note the absence of
typical refractory and acoustic
shadows of a fibroid/fibroma.
(Right) MR in the same patient
demonstrates an intimate
relationship between the
mass, adjacent small bowel
loops, and mesenteric vessels,
supporting the diagnosis of
gastrointestinal stromal
tumor. Note the loss of fat
plane with the small bowel
loops . The diagnosis was
confirmed after resection.
