Focal Lesion in Prostate
Müllerian Duct Cyst
Müllerian Duct Cyst
(Left) TRUS at the level of the
prostate base shows a midline
unilocular cystic structure ſt.
(Right) Sagittal image in the
same patient shows the
teardrop shape of the cyst ſt
with extension just beyond the
prostate base. Findings are
most consistent with a
müllerian duct cyst.
Differential Diagnoses: Prostate
Müllerian Duct Cyst
Müllerian Duct Cyst
(Left) TRUS image in a 65-
year-old man at the level of
the prostate base shows a
small midline cyst with
hypoechoic halo . (Right)
On sagittal image, this cyst
has a teardrop shape but
does not extend beyond the
prostate base and possibly
communicates with the
urethra. Patient's age and
absence of other GU
abnormalities favor a
müllerian duct cyst rather
than utricle cyst, but these are
often indistinguishable. Mild
wall thickening may be due to
inflammation or superimposed
infection.
Retention Cyst
Prostatic Abscess
(Left) TRUS shows several tiny
cysts within the enlarged
transition zone ſt, likely
reflecting retention cysts.
(Right) Axial noncontrast CT
shows a lobulated, cystic mass
within the left prostate gland
, causing rightward
deviation of a urethral
catheter ſt. In this patient
with fever and UTI, findings
are consistent with a prostatic
abscess. The patient
subsequently underwent
TRUS-guided drainage.
