EAPC Atlas of Palliative Care in Europe 2019

Conclusion: PC health policies developed in recent years have promoted vigorous development across Europe. Preliminary data on the integration of PC into different fields are encouraging though inequalities between countries and sub-regions persist. Further comparative analysis exploring factors leading to uneven progress may inform strategies to provide PC for all people in need. Per land is de stand van zaken van de (kinder)palliatieve zorg weergegeven.

Chapter 11. Integration <strong>of</strong> Oncology<br />

and <strong>Palliative</strong> <strong>Care</strong><br />

EUROPEAN CONTEXT<br />

B<br />

TECHNICAL DATA<br />

ON THE <strong>2019</strong> <strong>EAPC</strong><br />

SURVEY ON PALLIATIVE<br />

CARE INTO ONCOLOGY<br />

Population: 54 countries <strong>of</strong> the<br />

<strong>Europe</strong> WHO region and Lichtenste<strong>in</strong>.<br />

Complementary Survey<br />

Areas explored: 5a) existence<br />

<strong>of</strong> centres certified for the <strong>in</strong>tegration<br />

<strong>of</strong> <strong>in</strong>tegrated Oncology<br />

and PC, b) Cl<strong>in</strong>ical trials on<br />

early <strong>in</strong>tegration <strong>of</strong> PC <strong>in</strong> Oncological<br />

treatments and c) publications<br />

on <strong>in</strong>tegration <strong>of</strong> PC <strong>in</strong><br />

Oncology.<br />

Method: on-l<strong>in</strong>e search <strong>in</strong> public<br />

databases: ESMO, Cl<strong>in</strong>ical<br />

Trials.gov and Scopus.<br />

Data collection: April <strong>2019</strong>.<br />

Project Management: ATLAN-<br />

TES Research Group, Institute for<br />

Culture and Society, University<br />

<strong>of</strong> Navarra, Pamplona (Navarra),<br />

Spa<strong>in</strong> .<br />

Scientific Advice: Carlos Centeno<br />

(Spa<strong>in</strong>).<br />

Promotor: <strong>Europe</strong>an Association<br />

for <strong>Palliative</strong> <strong>Care</strong> (<strong>EAPC</strong>).<br />

“palliative care is applicable early <strong>in</strong> the course <strong>of</strong> illness, <strong>in</strong> conjunction<br />

with other therapies that are <strong>in</strong>tended to prolong life,<br />

such as chemotherapy or radiation therapy, and <strong>in</strong>cludes those<br />

INVESTIGATIONS needed to better understand and manage distress<strong>in</strong>g<br />

cl<strong>in</strong>ical complications” (1).<br />

The sentence from WHO’s current<br />

def<strong>in</strong>ition <strong>of</strong> palliative care emphasizes<br />

the importance <strong>of</strong> <strong>in</strong>tegrat<strong>in</strong>g<br />

oncology and palliative care. Same<br />

concept <strong>of</strong> the <strong>in</strong>tegration <strong>of</strong> PC throughout<br />

the course <strong>of</strong> an illness is also conta<strong>in</strong>ed <strong>in</strong> the<br />

modern def<strong>in</strong>ition <strong>of</strong> IAHPC result <strong>of</strong> a broad<br />

<strong>in</strong>ternational consensus process (6).<br />

Evidence from multiple randomized cl<strong>in</strong>ical<br />

trials support the early <strong>in</strong>tegration <strong>of</strong> PC with<strong>in</strong><br />

the oncological treatments: “true <strong>in</strong>tegration <strong>of</strong><br />

palliative and oncology care provides patients<br />

with optimal oncology care” (VR & Temel). From<br />

research and <strong>in</strong>ternational consensus the <strong>in</strong>tegrated<br />

care model has also become a topic <strong>in</strong><br />

cardiology, pneumology, and other specialties.<br />

In this chapter we aim to review the situation<br />

<strong>in</strong> <strong>Europe</strong> look<strong>in</strong>g at three ma<strong>in</strong> <strong>in</strong>dicators: a)<br />

The existence <strong>of</strong> certified centres <strong>of</strong> <strong>in</strong>tegrated<br />

Oncology and <strong>Palliative</strong> <strong>Care</strong>, b) the <strong>in</strong>creas<strong>in</strong>g<br />

number <strong>of</strong> cl<strong>in</strong>ical trials address<strong>in</strong>g questions<br />

regard<strong>in</strong>g the early <strong>in</strong>tegration <strong>of</strong> <strong>Palliative</strong> <strong>Care</strong><br />

with anti-cancer treatments and c) the grow<strong>in</strong>g<br />

number <strong>of</strong> scientific publications <strong>in</strong> the last few<br />

years concern<strong>in</strong>g our topic.<br />

Rafael Martínez, Amaia Urritzola.<br />

Our first search looked at the <strong>in</strong>itiative <strong>of</strong> the<br />

<strong>Europe</strong>an Society <strong>of</strong> Medical Oncology (ESMO)<br />

to improve the delivery <strong>of</strong> supportive and PC by<br />

oncologists, oncology departments and cancer<br />

centres. Through a rigorous process, s<strong>in</strong>ce<br />

2001, ESMO nom<strong>in</strong>ate centres that achieve<br />

high standards <strong>of</strong> <strong>in</strong>tegration <strong>of</strong> both discipl<strong>in</strong>es.<br />

In 2018 there are more than 150 centres<br />

from 25 countries.<br />

Another test for the <strong>in</strong>tegration <strong>of</strong> PC with<br />

Oncology could be the ris<strong>in</strong>g number <strong>of</strong> cl<strong>in</strong>ical<br />

trials focus<strong>in</strong>g on early PC <strong>in</strong> Oncology. Across<br />

<strong>Europe</strong> more than 25 Cl<strong>in</strong>ical Trials started <strong>in</strong><br />

recent years. Eight <strong>of</strong> them are already concluded<br />

and other are currently <strong>in</strong> recruitment<br />

phase. France, Italy and Switzerland are leaders<br />

<strong>in</strong> this research with over three trials runn<strong>in</strong>g.<br />

Regard<strong>in</strong>g the publications <strong>of</strong> any k<strong>in</strong>d, a quick<br />

search (“oncology AND palliative care AND<br />

<strong>in</strong>tegration”) performed by the Scopus scientific<br />

database found 359 documents from <strong>Europe</strong>an<br />

countries published <strong>in</strong> the past 10 years.<br />

To compare the situation, <strong>in</strong> the same period<br />

the USA published almost 200. The country<br />

ahead <strong>in</strong> this scientific <strong>in</strong>terest is Germany, with<br />

49 publications, followed by Italy, United K<strong>in</strong>gdom<br />

and Israel with around 21 publications.<br />
