SERVICES SERVICES CHARNLEY FOLD WITHY TREES Meals Activity Programme Day Trips Respite Family & Carer Support Assessments are made prior to places being offered at both facilities to ensure people receive the right level of care. Support Plans are utilised routinely and updated regularly. Age Concern does not provide direct personal care such as bathing and dressing. We can help facilitate transport needs if required. FAMILY AND CARER SUPPORT Age Concern recognises the vital role that family and carers provide in supporting people with dementia and memory concerns to navigate the ‘system’ and to get the help needed. At Age Concern family members and carers are fully supported from advice and information and carers Groups through to joint activities within the day centres. Families and carers can rest assured that Age Concern will support them every step of the way.
