ABOUT US<br />

Age Concern has almost 40 years of experience in<br />

helping adults to live well and age well and is renowned<br />

for delivering specialist dementia <strong>services</strong>.<br />

Age Concerns’ dementia <strong>services</strong> are tailored to the needs and<br />

preferences of those diagnosed with dementia and memory concerns<br />

whilst offering support, guidance and compassion to their family and<br />

carers.<br />

Within its bespoke facilities Age Concern emphasises focused activity,<br />

social interaction and help maintaining daily living skills through<br />

a wide-range of therapeutic activity programmes and innovative<br />

approaches.<br />

As a “Preferred Provider” by the local authority we maintain our service<br />

to the highest “Quality Standards”.


COMMUNITY SUPPORT<br />

Diagnosis is a scary time for everyone living with<br />

dementia including friends and family, that’s why our<br />

teams are where you need us to be.<br />

If you would like to know more about the next steps or just<br />

seeking some general advice or help we have a fantastic<br />

Information and peer support service within our day support<br />

centres.<br />

If you aren’t able to make it to one of our support centres<br />

the <strong>Dementia</strong> Community Links team work throughout local<br />

communities and within people’s homes to offer information<br />

and support in regards to the symptoms of dementia and how<br />

to seek help. We aim to build confidence through a variety of<br />

sessions and groups so that those who live with dementia feel<br />

confident to live independent and have an active and fulfilled life<br />

The centre has professional, skilled and experienced<br />

staff who are supported by trusted specialists such as<br />

an Occupational Therapist.

Assessments are made prior to places being offered at both facilities to ensure<br />

people receive the right level of care. Support Plans are utilised routinely and<br />

updated regularly.<br />

Age Concern does not provide direct personal care such as bathing and<br />

dressing. We can help facilitate transport needs if required.<br />

FAMILY AND<br />

CARER SUPPORT<br />

Age Concern recognises the vital role that family and carers provide in<br />

supporting people with dementia and memory concerns to navigate the<br />

‘system’ and to get the help needed.<br />

At Age Concern family members and carers are fully supported from advice and<br />

information and carers Groups through to joint activities within the day centres.<br />

Families and carers can rest assured that Age Concern will support them every<br />

step of the way.


RAISING THE STANDARD<br />

OF DEMENTIA CARE LOCALLY<br />

Age Concern plays an active role within local <strong>Dementia</strong><br />

Action Alliances and dementia operating groups and works<br />

with key partners to continually improve <strong>services</strong> and<br />

quality of life for people living with dementia and their<br />

family and carers.<br />
