application performa available from the <strong>University</strong> office on payment of Rs. 1000/-. The<br />
performa may be downloaded from the <strong>University</strong> website www.graupunjab.org and it<br />
may be sent to the <strong>University</strong> alongwith fee of US$ 500 plus Rs.1000 or equivalent in<br />
US$ being the cost of the application form in the shape of demand draft/cashier’s<br />
cheque in favour of the Registrar, <strong>Guru</strong> <strong>Ravidas</strong> <strong>Ayurved</strong> <strong>University</strong> <strong>Punjab</strong>, Hoshiarpur,<br />
Pb, INDIA drawn on any bank payable in India.<br />
The applicant has to attach following documents along with the application:-<br />
(i) 10+1/ equivalent examination detailed marks certificate(s)<br />
(The certificate(s) must show the marks of Physics, Chemistry, Biology & English)<br />
(ii) 10+2/ equivalent examination detailed marks certificate(s)/Official Transcript<br />
(The certificate(s) must show the marks of Physics, Chemistry, Biology & English)<br />
(iii) Explanation Sheet of grades.<br />
(iv) Copy of the certificate qualifying examination<br />
(v) Date of Birth Certificate.<br />
(vi) Character/Recommendation Certificate from the Head of the Institution last attended.<br />
(vii) Proof of NRI status of candidate/parents.<br />
Note:- These documents should be original along with Photocopies duly attested<br />
by a competent authority.<br />
A candidate should fill in the form preferably in his/her own handwriting & send it to the<br />
Deputy Registrar (Admission), <strong>Guru</strong> <strong>Ravidas</strong> <strong>Ayurved</strong> <strong>University</strong> <strong>Punjab</strong>, Jodhamal<br />
Road, Hoshiarpur, 146001 (Pb.), INDIA along with a Bank Draft for US$ 500 or<br />
equivalent amount in Indian rupees plus Rs.1000 or equivalent in US$ as a cost of<br />
form, in favour of the Registrar, <strong>Guru</strong> <strong>Ravidas</strong> <strong>Ayurved</strong> <strong>University</strong> <strong>Punjab</strong>, Jodhamal<br />
Road, Hoshiarpur, 146001 (Pb.), INDIA payable at Hoshiarpur.
