6.3 Reporting Requirements for Consultant Produced Reports<br />

The title page must include ODOT's project identification which contains the county, route, section,<br />

and PID (Project Identification Number) for the project. Information obtained for the ESA Screening<br />

such as regulatory database records, mapping, and visual observations should be summarized and<br />

the relevant information included and discussed in the report. Clear and concise conclusions must be<br />

provided which are supported by the data included. Recommendations for a petroleum contaminated<br />

soils plan note should be based on the conclusions <strong>of</strong> the No ROW ESA Screening and should be<br />

parcel-specific. The signature page should identify the report’s primary author and reviewers.<br />

Typically, the Office <strong>of</strong> <strong>Environmental</strong> Services, ESA Unit and the District Office's <strong>Environmental</strong><br />

<strong>Department</strong> each require one hard copy <strong>of</strong> the report. The following is the preferred format <strong>of</strong> the ESA<br />
