The <strong>Environmental</strong> Records section <strong>of</strong> the screening checklist form should be completed. At a<br />
minimum, environmental databases searched should include the NPL List, Proposed NPL List,<br />
CERCLIS, CERC-NFRAP, CORRACTS, RCRA-GEN, ERNS, SWF/LF, UST List, VCP File, No<br />
Contact List (Fish Advisory).<br />
5.3 Reporting Requirements For Consultant Produced Reports<br />
The title page must include ODOT's project identification which contains the county, route, section,<br />
and Project Identification Number (PID) for the project. Information obtained for the ESA Screening<br />
such as regulatory database records, mapping, and visual observations should be summarized and<br />
the relevant information included and discussed in the report. Clear and concise conclusions must be<br />
provided which are supported by the included data. Recommendations for further investigation should<br />
be based on the conclusions <strong>of</strong> the ESA Screening and should be parcel-specific. The signature page<br />
should identify the report’s primary author and reviewers. Typically, the Office <strong>of</strong> <strong>Environmental</strong><br />
Services, ESA Unit, and the District Office's <strong>Environmental</strong> <strong>Department</strong> each require one hard copy <strong>of</strong><br />
the report. The following is the preferred format <strong>of</strong> the ESA Screening report.<br />
