As the 2004-05 fiscal year concludes and
we celebrate the year�s accomplishments,
we�d like to thank our retiring Board
members who�ve opened their hearts,
minds and pockets to further the
Association�s mission. A special thank you
to retiring Board members: Tom Grimsley,
Greg Grupp, and of course our retiring
chair, Ken O�Dea: we are a stronger
organization because of each of you.

A few highlights from 2004-2005:
Launched an aggressive outreach
initiative designed to reach
individuals with Alzheimer�s and
their families throughout rural Iowa.
Implemented a new restorative
therapy component to Senior Day
Services� including the introduction
of the Senior Miles (S-MILES)
exercise program to help our clients
get moving!
This spring, we launched a new early
stage group respite program for
families in Le Mars: the Friday Fun
Club. Stop in and see us!
The most successful Wine & Roses
Festival weekend in our Chapter�s
history raised a record $216,000 in
support of our programs and services.
Thank you for your generous

Finally, we completed a new 3-year
strategic plan to guide the
Association in our efforts to reach
and serve more families dealing with
this devastating disease.
In order to capture firsthand the difference
we made this past year, we�d like to share a
letter from a caregiver and Senior Day
Services client family �
Eight years ago, my husband had
two strokes and couldn�t work
anymore�but the bills didn�t stop
coming just because he was sick. I
had to continue to work to pay the
bills, but I couldn�t leave him alone
at home.
I didn�t know what to do. For a
while, I even had to work two jobs
to pay the bills. And then I found
out about Senior Day Services
through the hospital.
They gave me a discount so he was
able to go to Senior Day Services
five days a week. If it weren�t for
them I wouldn�t be able to work�
�� d have to put my husband in the
nursing home. I know he�s getting
the best care possible, and having a
great time as well. With a little
help, he can take care of himself
and do the things he still enjoys
doing. And when we get home, we
both have a day to share with each
other.
Thank you for sharing your hearts, time,
energy and generous financial support to
help Siouxlanders fighting this disease, both
directly and indirectly. While our look
may have changed, we remain committed
to conquering this disease.

Around 4‰ million Americans over 65 are currently
fighting Alzheimer�s disease or a related disorder�
over 10,000 of them in greater Siouxland and
northwest Iowa.
Whether it�s helping older adults with dementia
remain at home for as long as safely possible, or
advocating for high quality, responsive long-term
care, our primary goals are to maximize their quality
of life and reduce stress on their primary
caregivers�often family members balancing their
caregiving with a career or family of their own.
Core services
Awareness: A core component of our strategic
plan is to alert the public to the growing millions
affected by Alzheimer’s and the importance of brain
health, and mobilize them to join our movement.
Safe Return: A nationwide registry to help
identify, locate and return individuals with
dementia who wander from their homes.
Care Consultation: The Family Assistance
Program assists persons with Alzheimer�s or related
dementias and/or their families in planning for and
dealing with all aspects of the illness experience.
Education Services: Workshops and conferences
about Alzheimer�s and related issues for family
caregivers, concerned friends and neighbors, and
the general public. Learn at home via our fullystocked
educational Resource Library. The
Dementia Care Learning Institute provides training
and in-services for professionals throughout the
long-term care continuum.
Helpline: 24 hours a day, 7 days a week�support,
information and assistance is only a phone call (or
an e-mail) away at the Contact Center, wherever
you are. Call (800) 272-3900 or visit us anytime at
www.alz-sioux.org.
Memory Loss Awareness Groups: Regularly
scheduled in-person or virtual gatherings of persons
with Alzheimer’s disease, family, friends or
caregivers who interact around issues relating to
Alzheimer’s. Groups can have social, educational
and/or support components and are facilitated by
trained individuals.
Market-driven services
Senior Day Services: An activity-based program
designed to maximize the well-being of cognitively
or functionally impaired, or ill and frail, older adults
by providing safe, supervised care in a social and
homelike environment. The Friday Fun Club
provides group respite services for Le Mars and
Plymouth County.
In-home Respite: Need a break? Trained
volunteers are matched with families who share
their interests, and provide brief periods of �timeout�
for 24-hour caregivers in need.
First Contact: First Contact enables caregivers to
contact trained volunteers in their own
communities�friends and neighbors they know
and trust. Local volunteers respond to requests for
assistance in their area and perform initial
assessments; follow-up will be provided with help
from Association staff if necessary.
First Contact programs in Buena Vista, Carroll,
Crawford, Emmet, Palo Alto, and Sac Counties are
made possible by a grant from the federal
Corporation for National and Community Service.

Senior Day Services: A decade of quality care
This summer, the Alzheimer�s Association,
Big Sioux Chapter marks the tenth
anniversary of what�s become one of our
most prominent programs: Senior Day
Services (SDS).
While not one of the Association�s core
services, SDS is a remarkable example of
rising to the challenge of meeting the needs
of Siouxland residents with dementia. Adult
day services were first offered in 1994, as a
joint venture between St. Luke�s Regional
Medical Center and Siouxland Easter Seals.
Most clients referred to the new service
were older adults with cognitive and
functional impairments, particularly a littleknown
disease called Alzheimer�s. Most
Siouxlanders, and indeed most Americans,
would only hear about Alzheimer�s for the
first time in the fall of that year, when
former President Reagan announced to the
nation that he was living with the disease.
In 1995, St. Luke�s began working with the
Alzheimer�s Association (then with a staff of
2) to establish an independent adult day
service�one with a working referral
relationship with both of Siouxland�s major
medical centers, as well as other
components of the long-term care
community.
Where we are: The Big Sioux Chapter serves the following counties.

Since then, SDS has helped nearly three
hundred older adults remain at home with
their families for an average of six to twelve
months�in some cases, for several years.
In addition to reducing stress on their
caregivers and keeping families together,
this represents a potential savings of
millions of dollars in long-term care costs.
By conservative estimates, nursing home
care costs average $100 per client per day�
costs that would otherwise be borne by
family caregivers, or by public long-term care
funding sources such as Medicaid.
We�ve also removed barriers to service
access by offering transportation to SDS
clients, which nearly doubled our client
census within a few months of its
introduction in late 1996; participated in the
Robert Wood Johnson Foundation�s
Partners in Caregiving program; and used
our expertise in best-practice models of care
to help design the first ever certification
system for Iowa adult day programs.
A decade later, we provide nearly 30,000
hours of care every year�helping our clients
get the best and most responsive care they
can, and helping their families take care of
themselves.

The First Contact concept was originally a
response to the results of the Big Sioux
Chapter�s groundbreaking work on responding
to the needs of older adults, both urban and
rural, living alone (without a primary caregiver)
with Alzheimer�s disease or a related dementia.
While we learned a great deal about how our
community�s providers identify and respond to
the needs of live alones, that wasn�t all we
learned. We also found a window into the
unique needs of older adults living in rural and
underserved communities, without easy
access to the network of providers available to
residents in Sioux City.
Census data indicate that the proportion of rural
Iowans of advanced age (and thus at greatest
risk for Alzheimer�s) is significantly higher than
in the state�s urban counties�in some cases,
nearly double. With assistance, namely home
and community-based services, these older
adults can remain safe and independent at
home for longer periods, but there are often
challenges unique to rural service delivery:
The isolated nature of the smaller
communities or family farms where
most of these individuals live.
A general lack of services outside
county seats (if they are available at all).
Limited transportation options for rural
seniors, if they exist at all.

families touched by Alzheimer�s.
First Contact: Reaching out to rural communities
Most importantly, a reluctance on the
part of many rural older adults either to
ask for help or to rely on
�outsiders� (those not from their
immediate vicinity) for assistance, no
matter how badly they needed it.
First Contact was designed to address some of
these needs. Local volunteer networks,
community by community, serve as �points of
entry� for rural residents in need of support or
assistance. Through these local volunteers,
residents in need would have access to the full
resources of the Alzheimer’s Association and
its aging network partners: education, caregiver
support, advocacy, and referrals to nearby
home and community-based services.
Memory Loss Libraries, centrally located in
county public libraries and other areas where
information can be easily and discreetly
accessed, serve as a further resource for local
residents in need or at risk. These miniature
libraries are intended to educate potential
clients�those who are as yet unsure whether
or not they need assistance, or who are
reluctant to ask for help.
Last year, the Association received a federal
grant to expand First Contact as a pilot program
in six rural Iowa counties: Buena Vista, Carroll,
Crawford, Emmet, Sac, and Palo Alto. Three
local volunteer coordinators are responsible for
recruitment and training of local volunteers.
Support for the First Contact program is provided by a grant from the
federal Corporation for National and Community Service.
