This contract is entered this ____day of _____________ in the<br />
year ______ This contract is between “ Your <strong>Direct</strong> <strong>Health</strong> <strong>Care</strong>”<br />
and the care provider/giver _____________________________.<br />
The purpose of this contract is to provide non medical in home<br />
senior care services to client(s) of Your <strong>Direct</strong> <strong>Health</strong> <strong>Care</strong>.<br />
Some of the many services you may provide as a caregiver are:<br />
meal preparation, medication reminders, light housekeeping and<br />
acitivites of daily living such as assistance w bathing, dressing,<br />
toileting. <strong><strong>Care</strong>giver</strong> will be asked to only do jobs within their<br />
scope of practice. <strong><strong>Care</strong>giver</strong> understands that they are never to<br />
drink on the job. <strong><strong>Care</strong>giver</strong> is given a 3 strike warning in which<br />
after 3 strikes caregiver shall be put on probation.
Strikes can occur with issues such as excessive tardy behavior<br />
or not filling out time sheets properly after multiple warnings,<br />
and other improper behavior, such as no-show, friends/family<br />
with client.<br />
<strong><strong>Care</strong>giver</strong> realizes that client is under the agency “Your <strong>Direct</strong><br />
<strong>Health</strong> <strong>Care</strong>” there<strong>for</strong>e all payment will occur from the agency<br />
!(not the client) as the caregiver is as an employee of the agency.<br />
Pay periods are twice monthy: from the 28th to the13th and the<br />
14th thru the 27th. Time sheets must be turned in by the13th<br />
and 27th so that caregiver can be paid on time-the 14th &<br />
28th respectively. You may take a picture and email your time<br />
sheets here to info@yourdirecthealthcare.com. Payment can be<br />
set up through your bank <strong>for</strong> direct deposit or caregiver can have<br />
checks mailed to them or picked up if need be. In addition the<br />
caregiver has agreed to the pay rate of _____ dollars per hour<br />
or_____per day. If you would like your pay deposited into your<br />
bank, (fastest method), please attach a voided check so we can<br />
deposit money into your account.<br />
<strong><strong>Care</strong>giver</strong> also agrees that by signing this contract that they will<br />
not solicit or offer private services to client. In event caregiver<br />
breaks this contract and upon investigation is found to have taken<br />
client <strong>for</strong> private pay, then caregiver is liable to pay agency<br />

5,000 dollars <strong>for</strong> any and all damages. Furthermore, caregivers<br />
in<strong>for</strong>mation will be passed to other caregiver agencies and home<br />
health agencies as a RED ALERT case warning other agencies<br />
of caregiver's work ethic. We are an equal opportunity employer. !<br />
Furthermore ! caregiver can not leave client alone even in the<br />
event of a family emergency. <strong><strong>Care</strong>giver</strong> must wait until another<br />
caregiver comes. If caregiver leaves client without anyone, then<br />
legal charges <strong>for</strong> abandonment can be placed on caregiver. !<br />
Your family and friends are not to be at client's home or place of<br />
residen. Your main focus is the care of client, there<strong>for</strong>e time<br />
spent on smart phones, lap tops, tv's or other devices is to be<br />
limited. Reports of excessive use will be reason <strong>for</strong> strikes and/or<br />
dismissal.
!When you commit to working with us, we and the clients expect<br />
you to show up on time. If you ever get sick, your child gets sick,<br />
or some emergency occurs then we need to have evidence as a<br />
doctors letter or other verifiable proof that you cant work. Calling<br />
us ! and telling us your child is sick with no proof from a Doctor<br />
will not permit us to give you the time off.<br />
If you can not make it to work we ask <strong>for</strong> two weeks notice. If you<br />
have an emergency you can never leave client alone. If you get<br />
sick or have another emergency which prevents you from working<br />
you must have a doctors note or valid proof. Calling in sick the<br />
day be<strong>for</strong>e work with no medical proof does not count. You also<br />
