Many happy returns…<br />
More babies are born in late September and early October than at any<br />
other time of the year. According to the Office for National Statistics,<br />
the most common birthday in England & Wales is 26 September.<br />
So there should be plenty of birthday parties over the next few weeks. These celebrations should be joyous<br />
occasions, but not all children enjoy parties and integrate with their peers as readily as others. For some, birthday<br />
parties and other social situations can be daunting, distressing and difficult. We are now working with Dr. Deborah<br />
Elijah, founder of the Elijah Social Cognitive Skills Centre, who has developed a pioneering early intervention<br />
programme for children with social and communication challenges. If you are concerned about<br />

And talking of birthdays, if you’re a woman approaching 45, if you haven’t already, you’ll<br />

Finally, I’d just like to congratulate all the Olympians who performed so well in Rio. It really<br />

If your teenager got pleasing grades then<br />
there is little psychological work to be<br />
done, other than to emphasise how<br />
pleased and happy you are that their hard<br />
work paid off.<br />
If however your youngster is disappointed with their<br />
results or didn't pass key subjects, then school advice is<br />
the best place to start.<br />
How do we prevent our youngsters<br />
from psychological damage and the<br />
well-flagged worry about lowering<br />
their self esteem?<br />
grades. If parents remain positive and supportive, stress<br />
that this happens to us all and share your own<br />
anecdotes, youngsters are less likely to ‘internalise’ their<br />
disappointment and feel like a failure.<br />
We know that confident people attribute external<br />
factors to their failure and internal factors to their<br />
successes, and vice versa. This appears to be<br />
psychologically protective for us, and makes us more<br />
resilient. However, it is healthy to learn to accept some<br />
disappointment and ride it, emerging stronger and<br />
more determined as a result.<br />
Feeding back this wisdom to your teen will better<br />
equip them to face life's challenges.<br />
It is my belief that a good internal sense of ‘self’ can<br />
ride the knocks that life throws at us all. The first real<br />
sense of failure a youngster may face is failing their<br />

Dr Abelman Dr Walter Abelman is the founder and<br />
Medical Director of JDoc. He qualified as a Doctor in 1992<br />
from Witwatersrand University, Johannesburg in South<br />
Africa and later moved to London. Once here, Dr.<br />

MENOPAUSE<br />

Menopause is the last menstruation. It occurs for most after the age of 45<br />
but can be earlier, after removal of the ovaries or ovarian insufficiency.<br />
80% of menopausal women experience symptoms,<br />
sometimes severe, such as hot flushes and night sweats,<br />
urinary infections and continence problems, poor sleep,<br />
migraines, vaginal dryness, low mood and low libido.<br />
Many women in their fifties work and/or care for<br />
relatives, and may find it difficult combining these<br />
responsibilities with symptoms of the menopause.<br />
HRT can improve sleep and mood,<br />
maintain and enhance muscle<br />
mass and strength, improve postmenopausal<br />
migraines and delay<br />
signs of aging<br />
Arguably, the most dangerous potential consequence<br />
of the menopause is osteoporosis. Accelerated then<br />
sustained bone loss can result in fractures and vertebral<br />
collapse. This can be life changing and even life<br />
threatening, but we can assess the risk of postmenopausal<br />
osteoporosis. HRT is the most effective<br />
way of increasing bone mass and preventing fractures. It<br />
is recommended for women under 60.<br />
Low dose vaginal oestrogens are effective in improving<br />
vaginal dryness, sexual function and relieving urinary<br />
frequency, and can be used long term. Because of their<br />
low concentration in the blood, they may help women<br />
affected by cancer, although expert consultation is<br />
required. Continuous HRT with oestrogen and<br />
progesterone reduces the risk of endometrial cancer<br />
and colorectal cancer.<br />
HRT can be taken in tablet form or as gel, patches or<br />
pessaries. Non-oral preparations don’t effect the<br />
metabolism or interact with other medicines.<br />
Oestrogens can be given to women who do not have a<br />
uterus anymore. They are combined with progesterone<br />
to avoid overgrowth of the lining of the womb.<br />
Data regarding the risk of coronary artery disease<br />
associated with HRT is inconclusive. At standard dose,<br />
non-oral oestrogens do not carry an increased risk of<br />
venous thrombo-embolism (VTE). Oral oestrogens<br />
double the risk (which increases with age), so non-oral<br />
oestrogens are therefore more suitable for overweight<br />
or diabetic women.<br />
HRT increases the risk of breast cancer by two per<br />
thousand in women in their fifties. This is similar in scale<br />
to two units of alcohol a day, and is lower than being<br />
obese. There is no proven increase in the number of<br />
women dying of breast cancer because of HRT.<br />
Women who start HRT early and continue until the<br />
menopause (because of loss of ovarian function), are<br />
not at increased risk of breast cancer. This also applies<br />
to women with BRCA genes who have had their<br />
ovaries removed.<br />
There is some evidence suggesting an increase in<br />
ovarian cancer under HRT of one extra case per<br />
thousand women over a five year period. Many BRCA<br />
mutation carriers choose to have their ovaries<br />
removed to reduce the risk.<br />
Advice, Treatment and Support<br />
Previous trials suggest that ‘hormones are dangerous’,<br />
but emerging evidence does not support this. Women<br />
can rest reassured that the use of appropriate HRT is<br />
easy and rewarding.<br />

A pioneering early intervention<br />
programme for children with social and<br />
communication challenges has been<br />
developed at the Elijah Social Cognitive<br />
Skills Centre.<br />
It aims to develop the interaction and cooperation skills<br />
that children need in order for them to functionally<br />
integrate into mainstream life.<br />
Addressing challenges early in life prevents them from<br />
becoming entrenched and inhibiting children from<br />
forming wholesome relationships. The earlier we<br />
address social and communication issues, the less likely<br />
children are to have compromised social, cognitive and<br />
physical abilities.<br />
The ESCSC was founded by Dr Debora Elijah, a Health<br />
& Care Professions Council registered Cognitive<br />
Neuropsychologist and Educational Consultant. Her<br />
areas of specialism are Autism Spectrum Disorders,<br />
Asperger’s, ADHD/ADD and Social Communication<br />
Skills for a wide range of age groups; from 18 months<br />
to young adults. Dr Elijah is a leading authority on<br />
autism and early intervention in the UK, and has spoken<br />
at numerous conferences and seminars across the<br />
world, including The Portland Hospital.<br />
The Elijah Centre Early Intervention Programme is a<br />
combination of best practices in education and clinical<br />
neuro-psychology. Through an intense, highly<br />
structured schedule of games and activities including<br />
pre-linguistic and neuro-motor skills, children get the<br />
necessary input to relate better to the environment<br />
around them. Intervention is carried out both in oneto-one<br />
and group settings.<br />
The Elijah Centre believes that all individuals are<br />
unique, and this is reflected in the highly individualised<br />
social cognitive program that is developed for each<br />
child. Activities and daily routines are tailored to each<br />
child’s individual developmental needs. This is the<br />
underlying principal of the Social Cognitive Intervention<br />
Program in Groups of Pairs in Social Networks<br />

relate better to the world around them, and skills<br />

Following my article ‘The Food Fighters’ in<br />
The <strong>Apple</strong> (April), in this edition I discuss<br />
a diet which is commonly prescribed for<br />
wind, bloating and diarrhoea predominant<br />
IBS when all else fails.<br />
Stress, busy lifestyles, medication and poor dietary<br />
choices can alter the number and variety of gut<br />
bacteria. Food supplements, including probiotics, will<br />
influence bacteria and bowel symptoms, and should<br />
be trialled first. If someone has diarrhoea predominant<br />
IBS and/or wind and bloating, a low fermentable<br />
oligo-di-mono-saccharide and polyol diet (low<br />
FODMAP diet) is the next step. This must be<br />
under the guidance of a Dietitian as it is a very<br />
difficult diet to follow.<br />
Bacteria’s four main actions are to eat, metabolise their<br />
food, excrete their waste and reproduce. Imagine these<br />
bacteria are lining the gut, competing for food and<br />
space every minute of the day. Of course everything<br />
we consume has to get past them. Some strains of<br />
bacteria will take a bite at some things and others<br />
won’t. If you provide a particular food source that<br />
bacterial strains like, they will keep eating it and<br />
possibly be a more successful strain.<br />
This is the concept of the low FODMAP diet; it’s about<br />
reducing the food sources which bacteria eat, causing<br />
less diarrhoea and gas through fermentation, and from<br />
drawing less water into the large bowel. This in turn<br />
makes us feel better.<br />
After 6-8 weeks,<br />
high FODMAP foods should<br />
be re-trialled one by one.<br />
Following the low FODMAP diet changes the variety of<br />
bacteria in the gut, but in the long-term that may not<br />
be so healthy so it is essential that as many foods as are<br />
tolerated are reintroduced.<br />

She has a an exotic heritage though. Her father was of<br />
Sudanese and Egyptian decent, whilst her mothers<br />
parents were Italian and Greek - and her husband is<br />
Scottish! Their wedding ‘car’ was a Peterbilt 379 truck.<br />
Christina is multilingual, studied law and spent time<br />
as a voluntary probation officer. Her<br />
experience also includes retail<br />
and veterinary medicine before<br />
establishing her career in healthcare<br />
working with GPs, NHS hospitals<br />

A lot of people worry about going to see a<br />
Psychiatrist. They worry that they are going to be<br />
judged and can find the situation extremely<br />
intimidating. However, this is something that you<br />
really do not need to worry about, as a Psychiatrist’s<br />
job is to make you feel as comfortable as possible. I<br />
will empathise with you and understand what you are<br />
going through; I certainly won’t judge you.<br />

relating to mental health,<br />
such as stress, post-natal<br />
depression, personality<br />
problems, obsessive<br />
compulsive disorder, eating<br />
disorders, depression,<br />
chronic pain, anxiety,<br />

During our first meeting I will get to know you, and<br />

your job, your relationships and any other significant<br />

Once I have got to know you a bit better, I assess the<br />
effect your mental health problems have on you. The<br />
goal is to determine whether the best course of<br />
treatment is therapy, medication or a combination of<br />
both. This all depends on your situation, personality<br />
and personal preference. People worry that<br />
psychiatrist's will use sedation or medications that<br />
will make them feel differently. I tend to only use<br />
medication in small doses, and only when necessary.<br />
A lot of people worry that going to a ‘shrink’ means<br />
that they are crazy. That’s nonsense - there are many<br />
people all over the world that have issues with their<br />
mental health. This doesn’t reflect badly on those that<br />
are effected, yet it’s important that you deal with<br />
your issues so you can have a more enjoyable and<br />
stress-free life.<br />

of disease. Individuals from certain populations have a<br />
particularly high risk to have children with genetic<br />
conditions, such as the Askhenazi Jewish population<br />
where 20% of individuals are carriers of a genetic<br />
condition. Some couples may decide to have genetic<br />
testing to try to clarify their risk of having a child with<br />
an inherited condition. PreconceptGene analyses more<br />
than 200 common genes which can cause inherited<br />
diseases. Some of these diseases, such as Tay Sachs,<br />
may be lethal in childhood while others may have a<br />
lesser effect on a person’s health. This test includes 191<br />
