Many happy returns…
More babies are born in late September and early October than at any
other time of the year. According to the Office for National Statistics,
the most common birthday in England & Wales is 26 September.
So there should be plenty of birthday parties over the next few weeks. These celebrations should be joyous
occasions, but not all children enjoy parties and integrate with their peers as readily as others. For some, birthday
parties and other social situations can be daunting, distressing and difficult. We are now working with Dr. Deborah
Elijah, founder of the Elijah Social Cognitive Skills Centre, who has developed a pioneering early intervention
programme for children with social and communication challenges. If you are concerned about
the social development of your child, JDoc365 can put you in touch with Dr. Elijah, who
has written about her work on page 8. Acting early can make all the difference.
And talking of birthdays, if you’re a woman approaching 45, if you haven’t already, you’ll
soon be facing the symptoms of the menopause. This can be an unsettling time, but my
colleague Dr. Sylvia Abramov, has some good advice. Her article can be found on page 7.
With all these birthdays, the holidays in full swing and so much to celebrate, it’s not always
easy to keep your figure under control. If you’re struggling to control your weight - and most
of us are - we have just launched a new weight loss programme which you may find helpful. See
page 3 for more details.
Finally, I’d just like to congratulate all the Olympians who performed so well in Rio. It really
was an outstanding spectacle of human achievement. Now our attention switches to the
Paralympics. In London 2012, it was the Paralympians who really stole our hearts,
and I wish them all the best of luck again this year.
EMMA CITRON
By Emma Citron, Consultant Clinical Psychologist
If your teenager got pleasing grades then
there is little psychological work to be
done, other than to emphasise how
pleased and happy you are that their hard
work paid off.
If however your youngster is disappointed with their
results or didn't pass key subjects, then school advice is
the best place to start.
How do we prevent our youngsters
from psychological damage and the
well-flagged worry about lowering
their self esteem?
grades. If parents remain positive and supportive, stress
that this happens to us all and share your own
anecdotes, youngsters are less likely to ‘internalise’ their
disappointment and feel like a failure.
We know that confident people attribute external
factors to their failure and internal factors to their
successes, and vice versa. This appears to be
psychologically protective for us, and makes us more
resilient. However, it is healthy to learn to accept some
disappointment and ride it, emerging stronger and
more determined as a result.
Feeding back this wisdom to your teen will better
equip them to face life's challenges.
It is my belief that a good internal sense of ‘self’ can
ride the knocks that life throws at us all. The first real
sense of failure a youngster may face is failing their
driving test or getting lower-than-hoped-for exam
IN HOSPITAL OR AT HOME?
Where would
you rather be?
From time to time your antibiotic medication may need to be administered
intravenously (through a drip). This invariably means an inconvenient and
often distressing stay in hospital.
JDoc365 offer a smart solution to eliminate overnight hospital
admissions for intravenous antibiotic medication; OPAT
(Outpatient Parenteral Antibiotic Therapy)
A fully qualified medical professional will visit you and administer your
antibiotics in your own home. For most patients, the comfortable and familiar
setting of their own home is infinitely preferable to a stay in hospital.
If you are currently facing the prospect of hospital admission for antibiotic
medication, or if you need to be admitted for it in the future, OPAT may just
be the ‘get out of hospital’ card you need.
MENOPAUSE
By Dr Sylvia
Abramov
Menopause is the last menstruation. It occurs for most after the age of 45
but can be earlier, after removal of the ovaries or ovarian insufficiency.
80% of menopausal women experience symptoms,
sometimes severe, such as hot flushes and night sweats,
urinary infections and continence problems, poor sleep,
migraines, vaginal dryness, low mood and low libido.
Many women in their fifties work and/or care for
relatives, and may find it difficult combining these
responsibilities with symptoms of the menopause.
HRT can improve sleep and mood,
maintain and enhance muscle
mass and strength, improve postmenopausal
migraines and delay
signs of aging
Arguably, the most dangerous potential consequence
of the menopause is osteoporosis. Accelerated then
sustained bone loss can result in fractures and vertebral
collapse. This can be life changing and even life
threatening, but we can assess the risk of postmenopausal
osteoporosis. HRT is the most effective
way of increasing bone mass and preventing fractures. It
is recommended for women under 60.
Low dose vaginal oestrogens are effective in improving
vaginal dryness, sexual function and relieving urinary
frequency, and can be used long term. Because of their
low concentration in the blood, they may help women
affected by cancer, although expert consultation is
required. Continuous HRT with oestrogen and
progesterone reduces the risk of endometrial cancer
and colorectal cancer.
HRT can be taken in tablet form or as gel, patches or
pessaries. Non-oral preparations don’t effect the
metabolism or interact with other medicines.
Oestrogens can be given to women who do not have a
uterus anymore. They are combined with progesterone
to avoid overgrowth of the lining of the womb.
Data regarding the risk of coronary artery disease
associated with HRT is inconclusive. At standard dose,
non-oral oestrogens do not carry an increased risk of
venous thrombo-embolism (VTE). Oral oestrogens
double the risk (which increases with age), so non-oral
oestrogens are therefore more suitable for overweight
or diabetic women.
HRT increases the risk of breast cancer by two per
thousand in women in their fifties. This is similar in scale
to two units of alcohol a day, and is lower than being
obese. There is no proven increase in the number of
women dying of breast cancer because of HRT.
Women who start HRT early and continue until the
menopause (because of loss of ovarian function), are
not at increased risk of breast cancer. This also applies
to women with BRCA genes who have had their
ovaries removed.
There is some evidence suggesting an increase in
ovarian cancer under HRT of one extra case per
thousand women over a five year period. Many BRCA
mutation carriers choose to have their ovaries
removed to reduce the risk.
Advice, Treatment and Support
Previous trials suggest that ‘hormones are dangerous’,
but emerging evidence does not support this. Women
can rest reassured that the use of appropriate HRT is
easy and rewarding.
JDoc365 offers menopause advice, treatment, a support
network, and access to specialists and physiotherapists.
Call us toady on 0800 043 1492 if you have any
concerns about the menopause.
A pioneering early intervention
programme for children with social and
communication challenges has been
developed at the Elijah Social Cognitive
Skills Centre.
It aims to develop the interaction and cooperation skills
that children need in order for them to functionally
integrate into mainstream life.
Addressing challenges early in life prevents them from
becoming entrenched and inhibiting children from
forming wholesome relationships. The earlier we
address social and communication issues, the less likely
children are to have compromised social, cognitive and
physical abilities.
The ESCSC was founded by Dr Debora Elijah, a Health
& Care Professions Council registered Cognitive
Neuropsychologist and Educational Consultant. Her
areas of specialism are Autism Spectrum Disorders,
Asperger’s, ADHD/ADD and Social Communication
Skills for a wide range of age groups; from 18 months
to young adults. Dr Elijah is a leading authority on
autism and early intervention in the UK, and has spoken
at numerous conferences and seminars across the
world, including The Portland Hospital.
The Elijah Centre Early Intervention Programme is a
combination of best practices in education and clinical
neuro-psychology. Through an intense, highly
structured schedule of games and activities including
pre-linguistic and neuro-motor skills, children get the
necessary input to relate better to the environment
around them. Intervention is carried out both in oneto-one
and group settings.
The Elijah Centre believes that all individuals are
unique, and this is reflected in the highly individualised
social cognitive program that is developed for each
child. Activities and daily routines are tailored to each
child’s individual developmental needs. This is the
underlying principal of the Social Cognitive Intervention
Program in Groups of Pairs in Social Networks
(PROSCIG) programme developed by Dr. Elijah.
8 JDOC365 APPLE MAGAZINE

As part of the PROSCIG programme, every child
receives an individual neuro-cognitive programme that
addresses his or her needs both at nursery or school
and at home.
Targets are set for each child and measured at regular
intervals to ensure progress is steady and goals are
being reached. Children receive the necessary input to
relate better to the world around them, and skills
gained are easily transferrable to a typical mainstream
classroom scenario. The end goal is to mould each
child into a confident and capable individual, capable of
tackling everyday challenges like everyone else.
PROSCIG focuses on the
following areas
Developing interaction and
cooperation in groups (age 2 to 10
years old)
Promoting independent thinking
and behavior
Development of cognitive skills,
social communication skills, selfconfidence
and self-worth
Functional spontaneous
communication
Encouraging children to play and
socialise with peers
How to decide whether your
child might need PROSCIG
intervention:
Language delays
Poor concentration
Inability to follow instructions
Inability to focus and engage
Aloofness
Poor social skills
Difficulties with sequencing, task completion
and comprehension
Low energy levels
Delays in gross or fine motor skills
Challenges with visual perception
Sensory processing issues
Dr. Elijah believes that every child can contribute and
be an active member of the family, at school and in the
community. Where young people experience a
disconnection or social and communication difficulties,
it is our job to make a connection with them and
make them whole; to empower every child to achieve
their true potential and lead a happier and more
fulfilled life.
Minimising delays in development
Supporting families to help them
achieve their own goals
Creating a framework for
observing social rules, both at
home and at school
Social competence
Generalisation of skills
Proactive approaches to
problem behavior
Reinforcing daily school
routine
The Elijah Social Cognitive Therapy
Skills Centre and JDoc365 enjoy a
PROFBIOTICS
IBS Part 3
The Low
FODMAP
Diet
By Tara Whyand, Specialist Dietitian
Following my article ‘The Food Fighters’ in
The Apple (April), in this edition I discuss
a diet which is commonly prescribed for
wind, bloating and diarrhoea predominant
IBS when all else fails.
Stress, busy lifestyles, medication and poor dietary
choices can alter the number and variety of gut
bacteria. Food supplements, including probiotics, will
influence bacteria and bowel symptoms, and should
be trialled first. If someone has diarrhoea predominant
IBS and/or wind and bloating, a low fermentable
oligo-di-mono-saccharide and polyol diet (low
FODMAP diet) is the next step. This must be
under the guidance of a Dietitian as it is a very
difficult diet to follow.
Bacteria’s four main actions are to eat, metabolise their
food, excrete their waste and reproduce. Imagine these
bacteria are lining the gut, competing for food and
space every minute of the day. Of course everything
we consume has to get past them. Some strains of
bacteria will take a bite at some things and others
won’t. If you provide a particular food source that
bacterial strains like, they will keep eating it and
possibly be a more successful strain.
This is the concept of the low FODMAP diet; it’s about
reducing the food sources which bacteria eat, causing
less diarrhoea and gas through fermentation, and from
drawing less water into the large bowel. This in turn
makes us feel better.
After 6-8 weeks,
high FODMAP foods should
be re-trialled one by one.
Following the low FODMAP diet changes the variety of
bacteria in the gut, but in the long-term that may not
be so healthy so it is essential that as many foods as are
tolerated are reintroduced.
POST-NATAL DEPRESSION │ PERSONALITY PROBLEMS │ OCD │ EATING DISORDERS │ DEPRESSION │ ANXIETY │ ADDICTION
Even the longest
begins with a single step
By Dr Mark Silvert MBBS MSc MRCPsych
DECIDING TO SEE A CONSULTANT PSYCHIATRIST IS A BIG
STEP, AND ONE YOU SHOULD BE VERY PROUD OF MAKING
A lot of people worry about going to see a
Psychiatrist. They worry that they are going to be
judged and can find the situation extremely
intimidating. However, this is something that you
really do not need to worry about, as a Psychiatrist’s
job is to make you feel as comfortable as possible. I
will empathise with you and understand what you are
going through; I certainly won’t judge you.
A psychiatrist can help with
virtually any problem
relating to mental health,
such as stress, post-natal
depression, personality
problems, obsessive
compulsive disorder, eating
disorders, depression,
chronic pain, anxiety,
addiction and much more.
During our first meeting I will get to know you, and
you can expect general questions about your life,
your job, your relationships and any other significant
events. I will ask you about the issues you are facing,
this includes finding out about your symptoms and
the impact they are having on your life.
Once I have got to know you a bit better, I assess the
effect your mental health problems have on you. The
goal is to determine whether the best course of
treatment is therapy, medication or a combination of
both. This all depends on your situation, personality
and personal preference. People worry that
psychiatrist's will use sedation or medications that
will make them feel differently. I tend to only use
medication in small doses, and only when necessary.
A lot of people worry that going to a ‘shrink’ means
that they are crazy. That’s nonsense - there are many
people all over the world that have issues with their
mental health. This doesn’t reflect badly on those that
are effected, yet it’s important that you deal with
your issues so you can have a more enjoyable and
stress-free life.
You may also want written reports, and your
psychiatrist should be able to provide you with this.
They may also liaise with your GP or other medical
doctors to ensure your road to recovery is as
effective as possible.
Whatever concerns or issues you may have, you
should seek the advice of a qualified medical
professional as soon as possible. We will support
you, and will find an appropriate solution to help you.
If you would like more
information about mental
health issues or would like to
GENETIC
TESTING
GeneHealth UK is a leading UK private
provider of state-of-the-art genetic
testing and genetic counselling services.
We are dedicated to improving patients’ quality of life
by offering molecular genetic tests that help healthcare
professionals and their patients make more informed
treatment and family planning decisions.
GeneHealth UK offers genetic testing for cancer
susceptibility such as BRCA1/2. This includes testing for
common gene alterations (mutations) in BRCA1/2
found in people of Ashkenazi Jewish descent. We also
offer panel testing, which looks at several genes known
to be involved in the development of breast, ovarian,
prostate and bowel cancer.
A discussion with a genetic counsellor will help people
decide what test would be most appropriate for them
based on their personal and family history of cancer.
in the Jewish population. We know
1-in-5 individuals of Ashkenazi Jewish
ancestry are carriers of an
inherited condition.
PreconceptGene can
significantly reduce the
risk of having a child with
one of these conditions.
This is because we are testing
for the common mutations
found in the genes currently known
to be associated with each condition. As with all
genetic tests, PreconceptGene is not able to look for all
possible genetic conditions and does not test for
chromosome abnormalities such as Down’s Syndrome.
PreconceptGene
The majority of all babies are born healthy; however,
everyone is at risk of having a child with a genetic
disease, even if they have no personal or family history
of disease. Individuals from certain populations have a
particularly high risk to have children with genetic
conditions, such as the Askhenazi Jewish population
where 20% of individuals are carriers of a genetic
condition. Some couples may decide to have genetic
testing to try to clarify their risk of having a child with
an inherited condition. PreconceptGene analyses more
than 200 common genes which can cause inherited
diseases. Some of these diseases, such as Tay Sachs,
may be lethal in childhood while others may have a
lesser effect on a person’s health. This test includes 191
autosomal recessive and 22 X-linked conditions. This
includes 57 genetic conditions that are more common
