Advice for travellers to Rio and South America
The tension is palpable…
There’s plenty going on in the next few weeks. Exam season is now upon
us, the European Championships promise football fans plenty to cheer
about (I hope), the Olympics and Paralympics will grip us all after that
and we’re celebrating our own 10 th birthday next month!
I look back fondly and with immense pride over our first decade, from our humble beginnings at Highgate Hospital
and Saturday night clinics in Warman Freed to the Sabel Clinic and converting my own garage into a surgery at home.
Then onto the Garden Hospital and finally to the fantastic facility we now have at the Wellington Diagnostic &
Outpatient Centre, and our superb surgery at the Platinum Medical Centre in St John’s Wood. Our next exciting
chapter will open later this year when we get the keys to our purpose built medical centre in Elstree.
Never mind ten years though, it’s been 50 years since England won a major tournament, but hopefully they’ll shine
during the Euros. Close games and penalty shoot-outs are very entertaining, but don’t get over excited and, without
wishing to tempt fate, don’t let any setbacks send you into a depression. ‘At the end of the day’, it’s ‘only a game’!
In August, Rio hosts the Olympics. After London 2012, it was great to see how Team GBs success in the cycling
encouraged so many people to get on their bikes and take more exercise. If you don’t already enjoy the benefits of
sport or exercise, see how you can factor some physical activity into your life - although it’s probably too late for Rio!
When you do need us, you may see some new faces at JDoc365 this summer. I’m delighted to
welcome new doctors, clinical assistant's and support staff to the team, and am sure they
will help us to provide an even better service for our patients.
Have a great summer, and enjoy the sunshine…
The Enemy Within...
By Dr. Heschil Lewin
Shingles is an infection of a nerve and the skin around
it. It is caused by the varicella-zoster virus, which also
causes Chickenpox. It lies dormant within your body
after the rash from Chickenpox has disappeared, often
undetected for many years. It then cruelly re-emerges
as Shingles when you are run down and vulnerable.
Shingles can appear at any age, but the elderly are
more affected.
You may contract Shingles if you
have never been infected with
Chickenpox
Early symptoms of Shingles
►
►
►
Feeling generally unwell and suffering
from headaches
Backache or a burning, tingling numbness
or itchiness in the affected area
You may have a raised temperature
These prodromal symptoms can
last for several days. The Shingles
rash then appears - usually on
one side of your body - and
develops on the area of skin
related to the affected
nerve. Red blotches
appear, and then
blisters which
are typically
arranged in
crops.
New blisters may appear
for up to a week or so
before they start to dry
up and disappear. Scabs
then form where the
blisters were, which can
leave slight scarring. It usually takes two to four weeks
for the rash to heal completely.
Although the rash will disappear, some sufferers may
be left with ongoing pain (which we call post herpetic
neuralgia). This pain will be centred on the affected
area of skin.
Shingles is not dangerous for most people, but it can
affect the general health in such a way that quality of
life and day-to-day comfort is compromised. One feels
generally unwell for some time, and a sensation of pain
may continue for several years.
You should see your GP
If you haven’t previously had Chickenpox or Shingles
and you think you may have been exposed to
someone with it, you should see your GP. If you
are pregnant or have a weakened immune
system, you should make this a priority.
Contact the doctor as soon as you see the rash,
as with medication we can reduce the severity
and duration of Shingles, and may be able to
prevent post herpetic neuralgia.
Contact JDoc365 if you detect the early
symptoms of Shingles. Call us on
0800 043 1492 as soon
as you can.
EMMA CITRON
By Emma Citron, Consultant Clinical Psychologist
At this time of year many of us have
teenagers sitting end of year, GCSE or GCE
exams. How do we best support them
psychologically, without letting our own
stress inadvertently impact upon them ?
I suggest asking your son or daughter whether they
would welcome you testing them on factual course
material, and make yourself available to them to do
this. Keep the home routine predictable. Make
mealtimes early so they don't stress by being hungry or
feeling unable to concentrate on their revision.
Encourage your teen to fit in some exercise, even if its
kicking a football in the garden, going
for a bike ride or walking (the
dog?) around the block.
Ask them how they
are managing, but
avoid being too
intrusive. They may
well be very sensitive and
potentially irritable right now!
Give your teenager extra space,
and don’t expect them to do
their usual chores at home
- if indeed they ever do any!
During exam season, if they tell you they’re very
stressed or feel they have blanked in a paper, don’t
underestimate the impact this can have. Refer them to
a registered clinical psychologist as it’s still not too late
to help them for their subsequent
papers. They can learn effective
anxiety management techniques
and receive support which will
see them cope better when
under pressure in
the future.
Mandy’s legacy
Last September, JDoc365 supported a fundraising event organised by
Mandy Fisher whilst she was battling cancer. Sadly, Mandy ultimately
lost her battle but her mother, Morella Kayman MBE, sent us a touching
letter and plea for help. Below, we publish extracts of that letter.
Whilst fighting ovarian cancer, my daughter Mandy took
on the challenge of playing four rounds of golf in one
day with four friends, Mandy's Fore Birdies, to raise
awareness of the disease and to help fund research.
Sadly, she passed away on 3 rd February this year at the
North London Hospice.
Mandy’s father had been diagnosed with Alzheimer's
when he was 49, and Mandy was diagnosed with ovarian
cancer in March 2014 aged just 55. Mandy made it clear
that she didn’t want anyone to feel sorry for her. She
believed she had been lucky to have lived a full and
meaningful life. She even felt lucky that her final two
weeks in the North London Hospice, surrounded by
first-class support, knowledge and kindness, was amazing.
She absolutely believed that things happen for a reason
and, regardless of your own situation, we should all be
positive and help others. During those final days, Mandy
asked me to carry on the charity challenge she had
started, and asked for the hospice to be added to the list
of beneficiaries.
Ovarian Cancer Research
Sarah Blagden’s Research Fund
Mandy's Fore Birdies - Diana Rubin, Ruth Langdorf,
Lindsay Blair, Julia Livingstone and Valerie
Lemer of Dyrham Park Country Club - are taking on
the challenge again this year in memory of their dear,
smiling, positive and inspirational friend Mandy. They will
be supported by a team from Hartsbourne Golf Club -
Philippa Newman, Jenny Reichman, Sue Moss
and Joy Shapiro. All will be raising money for the three
charities that Mandy wanted to support.
Mandy's Fore Birdies will play four rounds of golf at four different golf
courses in Barnet on Monday, 8 August 2016. With your support, they
will be able to raise much-needed funds for their charities.
I thank you in advance for your
generous support.
Balanced, or
just juggling?
HOW IMPORTANT IS IT TO IMPROVE THE BALANCE IN YOUR LIFE?
Remember the last time you were over-worked or
under stress? How easy was it to be a loving partner
or an engaged, tolerant parent? Not so easy I imagine.
When you are unwell, it can be difficult to give your
work or family the enthusiasm and energy it needs.
It’s all about balance; understanding that different areas
of our lives are intertwined and that we inhabit more
than one area at a time. It’s about paying attention to
the areas that we feel need adjustment or change.
It’s about learning that sometimes we need to give
ourselves time and space to get to a stronger, more
effective position.
THE HOLIDAY
When she told me of her plans, she
was so excited, particularly as she
rarely got a break from child-care and
was feeling drained. But, as her break
drew nearer she contemplated
cancelling. Surprised, I asked her why.
She told me of her fears. Would her
husband manage with the children?
She was full of ‘what ifs’ and
predictions of disaster.
We talked it through and she recalled
occasions that had gone well without
her input, and times when her husband
had coped wonderfully when alone.
She eventually decided to go.
When she got back, I asked how it
had gone. She laughed and told me
that her husband had thoroughly
enjoyed his time alone with the
children. The children had done
exciting things that she would not
normally have organised for them,
and she was relaxed and rested.
By taking some time for herself she
had created a great experience
for her whole family.
Retold with permission
How can you create a better balance?
2
Give yourself permission
Taking some time to make
changes is not selfish. You cannot
run on empty. Sometimes the
most loving thing we can do
for others is to become
happier and healthier
ourselves.
1
Decide what to change
Work out what you need to
improve your overall wellbeing.
Do you want more exercise?
Maybe a career change.
Perhaps more ‘down time’
to decompress
and relax.
3
Let go
Think about what you
could remove from your life
as well as what you might need
to add. You may need to
declutter before you
go shopping!
4
If you need help, ask
Decide who, if anyone, can help
you to make change. Maybe
the family could help more
to free up your time? Would
you like to talk to your
boss? Maybe a life
coach could help?
5
Be compassionate
Be kind to yourself. Start with
small, positive steps; keep it
simple and safe. Recognise
success and learn from
mistakes, pay attention to the
good things. Enjoy the
journey!
SPOR
Exercise improves general fitness and muscle tone,
and reduces the risk of chronic diseases such as
heart disease, type 2 diabetes and even cancer.
Exercise is also good for your mental health, can improve
mood, aid sleep and energy levels and reduce your risk of
depression, anxiety and dementia. However, exercise does
not come without its risks. Most people will experience
minor niggles such as muscular strains, but some will have
major injuries such as bone fractures or ligament tears.
HOW CAN YOU MINIMISE
YOUR RISK OF INJURY?
WARM UP BEFORE EXERCISE
Prepare physically and mentally for exercise. Five minutes of
general cardiovascular exercise warms up muscles in
preparation for exercise. It should make you slightly
breathless and increase your heart rate. You may also try
similar movements to those you will use in your exercise
(eg. practice swings for golfers).
WARM DOWN AFTER EXERCISE
After exercise, repeat the stretches or exercises from your
warm up to help prevent stiffness and cramp.
Build up the intensity of exercise slowly. This allows your
muscles to become used to the activity and condition your
body to exercise. Slowly increase the frequency, duration and
intensity of your activities.
Take recovery days between bouts of intensive exercise,
resting or doing low-impact exercise like swimming or cycling.
HAVE THE RIGHT EQUIPMENT
This may be in the form of well-maintained running shoes, or
shin pads if playing football.
DEVELOP GOOD TECHNIQUE
Poor technique can put extra stress on the wrong parts of
the body (eg. the lower back when using a rowing machine).
Get good advice from a coach or personal trainer if you’re
not sure about the correct technique.
FUEL FOR YOUR BODY
Eat well if you exercising regularly to maintain energy levels
and reduce fatigue during exercise. You can obtain this by
eating carbohydrates.
Protein helps growth and repair, and is particularly important
if you are trying to build muscle strength.
Trying to lose weight? Reduce the content of fat in your diet.
Guideline daily allowances or ‘Reference Intakes’
As part of a balanced diet, an adult's daily RI are:
Energy 8,400 kJ/2,000kcal Total sugars 90g
Total fat 70g
Protein 50g
Saturates 20g
Salt 6g
Carbohydrate 260g
Reference Intakes (RI) are based on an average-sized woman doing an average
amount of physical activity. This is to reduce the risk of people with lower energy
requirements eating too much, and to provide consistent information on labels.
Maintain good hydration by drinking fluids before, during and
after your activity. Be careful not to drink too much as this
can cause serious problems too.
TREATING INJURIES
Muscular aches and pains can be expected after activity and
are of no concern unless they persist after rest. Pain or
stiffness that occurs early into the activity or lasts for a long
time afterwards, may indicate an injury. You may need
medical advice if this is the case.
Acute injuries can occur suddenly and cause severe pain,
swelling, reduced movement or difficulty in weight bearing. If
your injury is acute, you should get advice from a healthcare
professional as soon as possible.
TS
By Dr. Evan Kabir
Overuse injuries can occur if you stress your body repeatedly
without sufficient recovery. As well as pain, you may have
swelling, numbness, tingling or stiffness. Do not return to full
activity until the injury has healed and you are pain free. You
can then slowly build up your activity, as the pain allows.
Most overuse injuries will gradually improve with rest or
reduction of activity, but it is important not to immobilise the
joint completely after the first 2-3 days and gentle
mobilisation, as pain allows, is encouraged.
Applying the principles of PRICE and avoid HARM in the
first 48-72 hours may help to treat minor injuries:
‘PRICE’ & ‘HARM’
PROTECTION Stop the activity and try to prevent further
injury by protecting the affected joint by using tape, padding
or crutches, for example.
REST Stop aggravating movements initially to aid recovery.
ICE This can help reduce pain and swelling. Do not apply
ice to skin directly as this can cause damage. You may wrap
ice in a damp towel. Apply to the affected region in 15-
minute intervals every 2 hours.
COMPRESSION Apply pressure to the injured region to
help reduce the onset of soft tissue swelling or bleeding
(between applications of ice).
Elastic compression bandages may help - but ensure these
are not too tight as it may cut off the circulation or cause
numbness in the region beyond the bandage.
ELEVATION Lift the affected joint above the level of the
heart to reduce blood flow and swelling (eg. by lying down
and resting an ankle injury on a pillow).
IN THE FIRST 48-72 HOURS - ‘HARM’
HEAT Do not apply heat (eg. hot shower or sauna) as it
will increase blood flow to the injury and cause swelling.
ALCOHOL Do not drink alcohol as this can also lead to
increased blood flow and further swelling. It can also mask
pain and it’s severity and may cause further injury.
RE-INJURY OR RUNNING Protect the injury until it has
healed to prevent re-injury.
MASSAGE Do not massage the affected body part as this
can promote blood flow and swelling.
In addition, painkillers such as Paracetamol or non-steroidal
anti-inflammatory drugs such as Ibuprofen, in the form of
tablets or creams may help to ease pain and swelling and
should be considered.
These principles should help with most injuries. However, if
you are still in a lot of pain and think there may be a serious
injury such as a fracture or dislocation, or if the injury is not
responding in the first 24 hours, you should seek medical
advice as soon as possible. Call JDoc365 on 0800 043 1492.
By Dr. Nathan Hasson
Consultant Paediatrician in Rheumatology
I often see children with arm and leg pain or
backache, and am inevitably asked if they are
‘growing pains’.
Let us be clear. ‘Growing pains’ don’t exist. They are a
diagnosis that some people refer to when everything
else seems normal. The main reason for musculoskeletal
aches and pains in children is that children who
are flexible and supple - otherwise called ‘hypermobile’
- require normal strength to control their joints. When
one examines children's’ muscle strength today, of the
22 muscles that are routinely examined, most children
today only have five normal functioning muscles left in
their body. This is due to a modern lifestyle.
by a Paediatric Rheumatologist. A diagnosis can then
be made of the weakness related to flexibility, and then
it is very important to build up strength and stamina.
We do this through physiotherapy, building up specific
muscle groups. Children are often quite flat footed if
they are flexible and podiatry can help with this.
It’s a fact that children very rarely cycle to school
nowadays, they play less sport and do less exercise
than used to be the case.
Concerns about safety are to blame, such as cycling on
the roads, and of small children being able to go to the
park and come home on their own when it’s dark,
which used to happen.
If you are also flexible and weak, then you will suffer
aches and pains. This can present with aches and pains
related to sports, pains the day after sports and even
waking up in the night with muscle cramps.
The best way to manage all of this is firstly to be seen
The inactivity of our children has a number of other
knock on effects, including issues related to weight gain
from not burning calories, general fatigue and tiredness.
They may also have poor concentration and very low
Vitamin D levels, as our children don’t play outside
anymore to get enough exposure to sunlight. It is
important for children to try and get as much sport and
activity into their lives as they can, but also taking a
good multivitamin with Vitamin D in it is very
important.
COMMUNITY AWARENESS EVENT
Hosted by Dr. Walter Abelman
We held the first in a series of Community Awareness
Events at the Wellington Diagnostics & Outpatients
Centre in Golders Green last month.
I was joined by two highly respected Consultant
Paediatricians. Dr Nathan Hasson spoke
about Hypermobility Syndrome.
He highlighted the fact that many
children do not exercise enough and are
weaker than they should be. This can
lead to pain and problems in joints, and
excessive tiredness. Dr. Hasson
encourages exercise and a
sensible diet for healthy living,
and promotes healthier
Dr. Abelman
habits for children.
Dr Helen Cauldield is a paediatric ENT. She spoke
about a range of topics including Glue Ear, which was
of great interest and very well received.
We plan to host further Community Awareness Events
in the future, and encourage everyone to attend as
they are informative and interactive, and excellent
refreshments are provided!
We will circulate details of the next event via email, so
make sure we have your email address on record.
