The tension is palpable…<br />
There’s plenty going on in the next few weeks. Exam season is now upon<br />
us, the European Championships promise football fans plenty to cheer<br />
about (I hope), the Olympics and Paralympics will grip us all after that<br />
and we’re celebrating our own 10 th birthday next month!<br />
I look back fondly and with immense pride over our first decade, from our humble beginnings at Highgate Hospital<br />
and Saturday night clinics in Warman Freed to the Sabel Clinic and converting my own garage into a surgery at home.<br />
Then onto the Garden Hospital and finally to the fantastic facility we now have at the Wellington Diagnostic &<br />
Outpatient Centre, and our superb surgery at the Platinum Medical Centre in St John’s Wood. Our next exciting<br />
chapter will open later this year when we get the keys to our purpose built medical centre in Elstree.<br />
Never mind ten years though, it’s been 50 years since England won a major tournament, but hopefully they’ll shine<br />
during the Euros. Close games and penalty shoot-outs are very entertaining, but don’t get over excited and, without<br />
wishing to tempt fate, don’t let any setbacks send you into a depression. ‘At the end of the day’, it’s ‘only a game’!<br />
In August, Rio hosts the Olympics. After London 2012, it was great to see how Team GBs success in the cycling<br />
encouraged so many people to get on their bikes and take more exercise. If you don’t already enjoy the benefits of<br />
sport or exercise, see how you can factor some physical activity into your life - although it’s probably too late for Rio!<br />
When you do need us, you may see some new faces at JDoc365 this summer. I’m delighted to<br />
welcome new doctors, clinical assistant's and support staff to the team, and am sure they<br />
will help us to provide an even better service for our patients.<br />

The Enemy Within...<br />

Shingles is an infection of a nerve and the skin around<br />
it. It is caused by the varicella-zoster virus, which also<br />
causes Chickenpox. It lies dormant within your body<br />
after the rash from Chickenpox has disappeared, often<br />
undetected for many years. It then cruelly re-emerges<br />
as Shingles when you are run down and vulnerable.<br />
Shingles can appear at any age, but the elderly are<br />
more affected.<br />

from headaches<br />
Backache or a burning, tingling numbness<br />
or itchiness in the affected area<br />
You may have a raised temperature<br />
These prodromal symptoms can<br />
last for several days. The Shingles<br />
rash then appears - usually on<br />
one side of your body - and<br />
develops on the area of skin<br />
related to the affected<br />
nerve. Red blotches<br />
appear, and then<br />
blisters which<br />
are typically<br />
arranged in<br />
crops.<br />
New blisters may appear<br />
for up to a week or so<br />
before they start to dry<br />
up and disappear. Scabs<br />
then form where the<br />
blisters were, which can<br />
leave slight scarring. It usually takes two to four weeks<br />
for the rash to heal completely.<br />
Although the rash will disappear, some sufferers may<br />
be left with ongoing pain (which we call post herpetic<br />
neuralgia). This pain will be centred on the affected<br />
area of skin.<br />
Shingles is not dangerous for most people, but it can<br />
affect the general health in such a way that quality of<br />
life and day-to-day comfort is compromised. One feels<br />
generally unwell for some time, and a sensation of pain<br />
may continue for several years.<br />
You should see your GP<br />
If you haven’t previously had Chickenpox or Shingles<br />
and you think you may have been exposed to<br />
someone with it, you should see your GP. If you<br />
are pregnant or have a weakened immune<br />
system, you should make this a priority.<br />
Contact the doctor as soon as you see the rash,<br />
as with medication we can reduce the severity<br />
and duration of Shingles, and may be able to<br />
prevent post herpetic neuralgia.<br />

At this time of year many of us have<br />
teenagers sitting end of year, GCSE or GCE<br />
exams. How do we best support them<br />
psychologically, without letting our own<br />
stress inadvertently impact upon them ?<br />
I suggest asking your son or daughter whether they<br />
would welcome you testing them on factual course<br />
material, and make yourself available to them to do<br />
this. Keep the home routine predictable. Make<br />
mealtimes early so they don't stress by being hungry or<br />
feeling unable to concentrate on their revision.<br />
Encourage your teen to fit in some exercise, even if its<br />
kicking a football in the garden, going<br />
for a bike ride or walking (the<br />
dog?) around the block.<br />
Ask them how they<br />
are managing, but<br />
avoid being too<br />
intrusive. They may<br />
well be very sensitive and<br />
potentially irritable right now!<br />
Give your teenager extra space,<br />
and don’t expect them to do<br />
their usual chores at home<br />
- if indeed they ever do any!<br />
During exam season, if they tell you they’re very<br />
stressed or feel they have blanked in a paper, don’t<br />
underestimate the impact this can have. Refer them to<br />
a registered clinical psychologist as it’s still not too late<br />
to help them for their subsequent<br />
papers. They can learn effective<br />
anxiety management techniques<br />
and receive support which will<br />
see them cope better when<br />
under pressure in<br />
the future.<br />

Dr. Adam Townley<br />
Dr Townley holds both British and American passports,<br />
although he was born and bred in North London. He is<br />
married with three children, is a practising GP, a GP trainer,<br />
helping to advance the careers of post graduate doctors,<br />
and has been part of the JDoc365 team for many years.<br />
Adam has lived and worked in far flung and exotic<br />
destinations such as New Zealand and Australia, and<br />
although he spent many happy years in Manchester too, he<br />
stops short of describing it as exotic.<br />
When not in the surgery, Adam continues his professional<br />
development by meeting regularly with other medical<br />
professionals to discuss challenging cases, listen to guest<br />
speakers and generally keep abreast of the latest advances<br />
in the world of medicine.<br />
Adam is also a keen cyclist, swimmer and winter sports<br />
enthusiast. He leads by example in keeping fit and active<br />
as part of his wellbeing regime,<br />
which he credits directly for<br />
his good health and love<br />

Mandy’s legacy<br />
Last September, JDoc365 supported a fundraising event organised by<br />
Mandy Fisher whilst she was battling cancer. Sadly, Mandy ultimately<br />
lost her battle but her mother, Morella Kayman MBE, sent us a touching<br />
letter and plea for help. Below, we publish extracts of that letter.<br />
Whilst fighting ovarian cancer, my daughter Mandy took<br />
on the challenge of playing four rounds of golf in one<br />
day with four friends, Mandy's Fore Birdies, to raise<br />
awareness of the disease and to help fund research.<br />
Sadly, she passed away on 3 rd February this year at the<br />
North London Hospice.<br />
Mandy’s father had been diagnosed with Alzheimer's<br />
when he was 49, and Mandy was diagnosed with ovarian<br />
cancer in March 2014 aged just 55. Mandy made it clear<br />
that she didn’t want anyone to feel sorry for her. She<br />
believed she had been lucky to have lived a full and<br />
meaningful life. She even felt lucky that her final two<br />
weeks in the North London Hospice, surrounded by<br />
first-class support, knowledge and kindness, was amazing.<br />
She absolutely believed that things happen for a reason<br />
and, regardless of your own situation, we should all be<br />
positive and help others. During those final days, Mandy<br />
asked me to carry on the charity challenge she had<br />
started, and asked for the hospice to be added to the list<br />
of beneficiaries.<br />

smiling, positive and inspirational friend Mandy. They will<br />
be supported by a team from Hartsbourne Golf Club -<br />
Philippa Newman, Jenny Reichman, Sue Moss<br />
and Joy Shapiro. All will be raising money for the three<br />
charities that Mandy wanted to support.<br />
Mandy's Fore Birdies will play four rounds of golf at four different golf<br />
courses in Barnet on Monday, 8 August <strong>2016</strong>. With your support, they<br />
will be able to raise much-needed funds for their charities.<br />

just juggling?<br />
HOW IMPORTANT IS IT TO IMPROVE THE BALANCE IN YOUR LIFE?<br />
Remember the last time you were over-worked or<br />
under stress? How easy was it to be a loving partner<br />
or an engaged, tolerant parent? Not so easy I imagine.<br />
When you are unwell, it can be difficult to give your<br />
work or family the enthusiasm and energy it needs.<br />
It’s all about balance; understanding that different areas<br />
of our lives are intertwined and that we inhabit more<br />
than one area at a time. It’s about paying attention to<br />
the areas that we feel need adjustment or change.<br />
It’s about learning that sometimes we need to give<br />
ourselves time and space to get to a stronger, more<br />
effective position.<br />
THE HOLIDAY<br />
When she told me of her plans, she<br />
was so excited, particularly as she<br />
rarely got a break from child-care and<br />
was feeling drained. But, as her break<br />
drew nearer she contemplated<br />
cancelling. Surprised, I asked her why.<br />
She told me of her fears. Would her<br />
husband manage with the children?<br />
She was full of ‘what ifs’ and<br />
predictions of disaster.<br />
We talked it through and she recalled<br />
occasions that had gone well without<br />
her input, and times when her husband<br />
had coped wonderfully when alone.<br />
She eventually decided to go.<br />
When she got back, I asked how it<br />
had gone. She laughed and told me<br />
that her husband had thoroughly<br />
enjoyed his time alone with the<br />
children. The children had done<br />
exciting things that she would not<br />
normally have organised for them,<br />
and she was relaxed and rested.<br />

had created a great experience<br />
for her whole family.<br />

SPOR<br />
Exercise improves general fitness and muscle tone,<br />
and reduces the risk of chronic diseases such as<br />
heart disease, type 2 diabetes and even cancer.<br />
Exercise is also good for your mental health, can improve<br />
mood, aid sleep and energy levels and reduce your risk of<br />
depression, anxiety and dementia. However, exercise does<br />
not come without its risks. Most people will experience<br />
minor niggles such as muscular strains, but some will have<br />
major injuries such as bone fractures or ligament tears.<br />
HOW CAN YOU MINIMISE<br />
YOUR RISK OF INJURY?<br />
WARM UP BEFORE EXERCISE<br />
Prepare physically and mentally for exercise. Five minutes of<br />
general cardiovascular exercise warms up muscles in<br />
preparation for exercise. It should make you slightly<br />
breathless and increase your heart rate. You may also try<br />
similar movements to those you will use in your exercise<br />
(eg. practice swings for golfers).<br />
WARM DOWN AFTER EXERCISE<br />
After exercise, repeat the stretches or exercises from your<br />
warm up to help prevent stiffness and cramp.<br />
Build up the intensity of exercise slowly. This allows your<br />
muscles to become used to the activity and condition your<br />
body to exercise. Slowly increase the frequency, duration and<br />
intensity of your activities.<br />
Take recovery days between bouts of intensive exercise,<br />
resting or doing low-impact exercise like swimming or cycling.<br />
HAVE THE RIGHT EQUIPMENT<br />
This may be in the form of well-maintained running shoes, or<br />
shin pads if playing football.<br />
DEVELOP GOOD TECHNIQUE<br />
Poor technique can put extra stress on the wrong parts of<br />
the body (eg. the lower back when using a rowing machine).<br />
Get good advice from a coach or personal trainer if you’re<br />
not sure about the correct technique.<br />
FUEL FOR YOUR BODY<br />
Eat well if you exercising regularly to maintain energy levels<br />
and reduce fatigue during exercise. You can obtain this by<br />
eating carbohydrates.<br />
Protein helps growth and repair, and is particularly important<br />
if you are trying to build muscle strength.<br />
Trying to lose weight? Reduce the content of fat in your diet.<br />
Guideline daily allowances or ‘Reference Intakes’<br />
As part of a balanced diet, an adult's daily RI are:<br />
Energy 8,400 kJ/2,000kcal Total sugars 90g<br />
Total fat 70g<br />
Protein 50g<br />
Saturates 20g<br />
Salt 6g<br />
Carbohydrate 260g<br />
Reference Intakes (RI) are based on an average-sized woman doing an average<br />
amount of physical activity. This is to reduce the risk of people with lower energy<br />
requirements eating too much, and to provide consistent information on labels.<br />
Maintain good hydration by drinking fluids before, during and<br />
after your activity. Be careful not to drink too much as this<br />
can cause serious problems too.<br />
TREATING INJURIES<br />
Muscular aches and pains can be expected after activity and<br />
are of no concern unless they persist after rest. Pain or<br />
stiffness that occurs early into the activity or lasts for a long<br />
time afterwards, may indicate an injury. You may need<br />
medical advice if this is the case.<br />
Acute injuries can occur suddenly and cause severe pain,<br />
swelling, reduced movement or difficulty in weight bearing. If<br />
your injury is acute, you should get advice from a healthcare<br />
professional as soon as possible.<br />

Overuse injuries can occur if you stress your body repeatedly<br />
without sufficient recovery. As well as pain, you may have<br />
swelling, numbness, tingling or stiffness. Do not return to full<br />
activity until the injury has healed and you are pain free. You<br />
can then slowly build up your activity, as the pain allows.<br />
Most overuse injuries will gradually improve with rest or<br />
reduction of activity, but it is important not to immobilise the<br />
joint completely after the first 2-3 days and gentle<br />
mobilisation, as pain allows, is encouraged.<br />
Applying the principles of PRICE and avoid HARM in the<br />
first 48-72 hours may help to treat minor injuries:<br />
‘PRICE’ & ‘HARM’<br />
PROTECTION Stop the activity and try to prevent further<br />
injury by protecting the affected joint by using tape, padding<br />
or crutches, for example.<br />
REST Stop aggravating movements initially to aid recovery.<br />
ICE This can help reduce pain and swelling. Do not apply<br />
ice to skin directly as this can cause damage. You may wrap<br />
ice in a damp towel. Apply to the affected region in 15-<br />
minute intervals every 2 hours.<br />
COMPRESSION Apply pressure to the injured region to<br />
help reduce the onset of soft tissue swelling or bleeding<br />
(between applications of ice).<br />
Elastic compression bandages may help - but ensure these<br />
are not too tight as it may cut off the circulation or cause<br />
numbness in the region beyond the bandage.<br />
ELEVATION Lift the affected joint above the level of the<br />
heart to reduce blood flow and swelling (eg. by lying down<br />
and resting an ankle injury on a pillow).<br />
IN THE FIRST 48-72 HOURS - ‘HARM’<br />
HEAT Do not apply heat (eg. hot shower or sauna) as it<br />
will increase blood flow to the injury and cause swelling.<br />
ALCOHOL Do not drink alcohol as this can also lead to<br />
increased blood flow and further swelling. It can also mask<br />
pain and it’s severity and may cause further injury.<br />
RE-INJURY OR RUNNING Protect the injury until it has<br />
healed to prevent re-injury.<br />
MASSAGE Do not massage the affected body part as this<br />
can promote blood flow and swelling.<br />
In addition, painkillers such as Paracetamol or non-steroidal<br />
anti-inflammatory drugs such as Ibuprofen, in the form of<br />
tablets or creams may help to ease pain and swelling and<br />
should be considered.<br />
These principles should help with most injuries. However, if<br />
you are still in a lot of pain and think there may be a serious<br />
injury such as a fracture or dislocation, or if the injury is not<br />
responding in the first 24 hours, you should seek medical<br />

MEET THE TEAM<br />
Christain Glen<br />
Clinical Assistant<br />
In 2004, Chris moved to the UK from South<br />
Africa. Now mother to two year old son Teo,<br />
Chris joined JDoc365 just last month.<br />
As a Clinical Assistant, Chris is a busy medical professional,<br />
and helps to ensure that the surgery runs smoothly. You<br />
may see her on reception, in the treatment room or<br />
working with doctors and their patients.<br />
A self-confessed ‘adrenalin junkie’,<br />
her biggest buzz so far has been<br />
skydiving in South Africa.<br />
Prior to joining JDoc365, Chris worked in Harley Street<br />
and at The Wellington Hospital. She has enjoyed looking<br />
after patients since joining the healthcare sector some 12<br />
years ago and, when she worked for South African<br />
Airways, her passengers too.<br />
TRAVEL UPDATE<br />
Dengue fever cases in Brazil increased considerably last year. Zika virus and Chikunyunga have also<br />
been confirmed there too with increasing regularity. You should be vigilant and take all sensible<br />
precautions to minimise the risk of being bitten by mosquitoes.<br />
Travellers should be up to date with routine vaccination courses and boosters. These include Measles-Mumps-Rubella<br />
(MMR) and Diphtheria-Tetanus-Polio vaccines, and Hepatitis A and Tetanus should also be vaccinated against.<br />
There is also a risk of Yellow Fever in parts of Brazil. JDoc365 is a fully<br />
accredited NATHNAC Yellow Fever Vaccine Centre.<br />

Consultant Paediatrician in Rheumatology<br />
I often see children with arm and leg pain or<br />
backache, and am inevitably asked if they are<br />
‘growing pains’.<br />
Let us be clear. ‘Growing pains’ don’t exist. They are a<br />
diagnosis that some people refer to when everything<br />
else seems normal. The main reason for musculoskeletal<br />
aches and pains in children is that children who<br />
are flexible and supple - otherwise called ‘hypermobile’<br />
- require normal strength to control their joints. When<br />
one examines children's’ muscle strength today, of the<br />
22 muscles that are routinely examined, most children<br />
today only have five normal functioning muscles left in<br />
their body. This is due to a modern lifestyle.<br />
by a Paediatric Rheumatologist. A diagnosis can then<br />
be made of the weakness related to flexibility, and then<br />
it is very important to build up strength and stamina.<br />
We do this through physiotherapy, building up specific<br />
muscle groups. Children are often quite flat footed if<br />
they are flexible and podiatry can help with this.<br />
It’s a fact that children very rarely cycle to school<br />
nowadays, they play less sport and do less exercise<br />
than used to be the case.<br />
Concerns about safety are to blame, such as cycling on<br />
the roads, and of small children being able to go to the<br />
park and come home on their own when it’s dark,<br />
which used to happen.<br />
If you are also flexible and weak, then you will suffer<br />
aches and pains. This can present with aches and pains<br />
related to sports, pains the day after sports and even<br />
waking up in the night with muscle cramps.<br />
The best way to manage all of this is firstly to be seen<br />

from not burning calories, general fatigue and tiredness.<br />
They may also have poor concentration and very low<br />
Vitamin D levels, as our children don’t play outside<br />
anymore to get enough exposure to sunlight. It is<br />
important for children to try and get as much sport and<br />
activity into their lives as they can, but also taking a<br />
good multivitamin with Vitamin D in it is very<br />

We held the first in a series of Community Awareness<br />
Events at the Wellington Diagnostics & Outpatients<br />
Centre in Golders Green last month.<br />
I was joined by two highly respected Consultant<br />
Paediatricians. Dr Nathan Hasson spoke<br />
about Hypermobility Syndrome.<br />
He highlighted the fact that many<br />
children do not exercise enough and are<br />
weaker than they should be. This can<br />
lead to pain and problems in joints, and<br />
excessive tiredness. Dr. Hasson<br />
encourages exercise and a<br />
sensible diet for healthy living,<br />
and promotes healthier<br />

Dr Helen Cauldield is a paediatric ENT. She spoke<br />
about a range of topics including Glue Ear, which was<br />
