The Pareto Epidemic…<br />
The Pareto principle (also known as the 80:20 rule) states that roughly<br />
80% of the output comes from 20% of the input. In the past few weeks<br />
we’ve seen something very similar in the surgery.<br />
Following extensive national media coverage regarding Meningitis B, many concerned parents are focussed on getting<br />
their children vaccinated. Meningitis B is thankfully very rare and is statistically unlikely to affect your family, although<br />
vaccination can take much of the credit for that fact. There are however, many ‘everyday’ factors that are much more<br />
likely to impact on our health and the wellbeing of our children, yet we do very little to address their causes.<br />
As one of many doctors who see the damage caused by poor diet, lack of exercise and the occasional accident or<br />
mishap almost every day, I urge you to invest a proportionate amount of your time and energy in your own<br />
wellbeing, and that of your family.<br />
Eat well, cook with fresh ingredients and<br />
avoid convenience foods as much as you can.<br />
Build regular exercise into your family life, walk to school or work, go for<br />
family bike rides at the weekend (wearing a safety helmet of course) and<br />
take the dog to the park more often.<br />

Accidental injuries are one of the leading causes of<br />
serious harm and death in young children in the UK.<br />
Each year two million children under the age of 15 go<br />
to A&E having been injured at home, and about 25% of<br />
them are under 5 years old. Many more are treated<br />
with first aid by their parents.<br />
Accidents can be caused by cuts, falls, burns, choking,<br />
suffocating, drowning, poisoning and more. Many<br />
should initially be treated at home, so as a parent, it is<br />
important to have a basic knowledge of first aid – there<br />
are many classes available if you need some training.<br />

supervision, making your home safe and recognising<br />

attend an emergency department for medical<br />
assessment. Timely care provided by a health<br />

Intrusive habit or<br />
serious disorder?<br />

We have all heard of Obsessive Compulsive Disorder (OCD)<br />
and may have worried at some point about whether we have<br />
OCD tendencies ourselves.<br />
OCD-like behaviours are especially common around<br />
Passover, Spring cleaning time or during the recent<br />
SARS, Ebola and Zika outbreaks.<br />
We may find ourselves washing our hands more than<br />
usual or recommended, or becoming more rigid in our<br />
routines. Perhaps we derive some comfort in this by,<br />
for example, straightening the books on the shelf, lining<br />
up all the shoes in the rack or checking our email ‘sent<br />
documents’ folder again and again for errors.<br />
OCD affects about 2% of the<br />
population, or may affect any one of<br />
us with a 2% likelihood.<br />
What is going on here? Is this OK, and when do our<br />
behaviours become a cause for concern? Obsessivecompulsive<br />
disorder is best described as when people's<br />
preoccupations get out of hand. It may be that they<br />
worry about contracting a disease and then ‘neutralise’<br />
this worry by engaging in one or several compulsions<br />
which act to neutralise it.<br />
Many of us become more obsessive and compulsive in<br />
our behaviours at times of stress, and the behaviours<br />
serve as a ‘quick fix’. However, compulsive behaviours<br />
where they escalate, actually make anxiety worse and<br />
professional help should be sought by a registered<br />
Clinical Psychologist.<br />

Dr. Stacey Hoffman<br />
Dr Hoffman studied and completed her medical degree<br />
qualifying from the University of Cape Town in South<br />
Africa. Having been in the UK for nine years and working<br />
as a family GP now, she is particularly focused on women’s<br />
and children’s health.<br />
A busy working mum with a lively, growing family of five,<br />
Dr. Hoffman has returned to work after having had<br />
another baby. She is now back at JDoc365 in Golders<br />
Green on Monday mornings.<br />
Dr Hoffman's first son - Gabriel Shai - arrived on 8<br />
September last year, weighing 3.55kg. Despite being an<br />

Dr. Bhundia trained at Kings College Hospital and at the Plastic Surgery Department<br />
at The Royal Free Hospital, and holds a Diploma in Dermatological Surgery<br />
For over six years, Dr. Bhundia has been operating on<br />
benign skin lesions, providing cryotherapy for warts and<br />
skin tags, and treating painful joints including shoulders,<br />
knees, fingers and wrists with special injections.<br />
Although often described as ‘minor’ surgery, these<br />
procedures provide major benefits and relief from longstanding<br />
conditions. There are no significant side effects,<br />
and apart from cosmetic and mobility benefits, they can<br />
also make a positive impact on patients emotional<br />
wellbeing too.<br />

Removal of Mole<br />
The patient was getting married in three months. She<br />
had a conspicuous mole on her neck, which she<br />
wanted to be removed. Dr. Bhundia duly excised it<br />
using a suturing technique that left no significant scar.<br />
The bride-to-be was very happy with the outcome and<br />
looked beautiful in her wedding pictures.<br />

Removal of Cysts<br />
A young man of 23 years suffered with cysts in his<br />
scalp. There was hair loss over the cysts and he was<br />
worried about further loss. Dr. Bhundia removed the<br />
cysts with minimal scarring, and the patient reported<br />
full hair regrowth shortly afterwards.<br />

Removal of Lipoma<br />
A 27 year old professional volleyball player had a large<br />
lipoma (fatty tissue growth) on her right shoulder<br />
which impeded her game. Dr Bhundia excised the<br />
lesion and the patient went on to represent her club<br />
and country with no further hindrance.<br />

Following my article on irritable bowel<br />
syndrome in the last edition (page 7), this<br />
time I look at food supplements which may<br />
be of use to IBS sufferers.<br />
As previously mentioned, the probable main cause of<br />
altered movements is down to changes in our gut<br />
bacteria. This can include stress, busy lifestyles, drugs<br />
and poor dietary choices. Unless you address the<br />
bacteria imbalances directly and work out what is<br />
affecting them, your symptoms will never go away. If<br />
you think about it, how you act and what you consume<br />
creates the environment for particular strains of<br />
bacteria to either flourish, or die off. What we want is<br />
to have a huge variety of strains living in the bowel.<br />
Leaving imbalances alone can lead to inflammation of<br />
the bowel lining.<br />
Some food supplements have been shown<br />
to help IBS, so if reducing stress and<br />
managing a busy lifestyle has not made a<br />
difference, they may be worth a try.<br />
The first method to consider is adding live bacteria into<br />
your diet. These are called probiotic bacteria and they<br />
help colonise the bowel to correct any imbalances that<br />
can exist. You should try a strong multistrain dose daily<br />
for four weeks, and if it does not help, there are some<br />
other methods to consider raising with your doctor.<br />
We know that vitamin D deficiency is common, but it<br />
may be even more common in people with IBS. We<br />
do not know why this is, but it may be a good idea to<br />
get tested as deficiency puts you at risk of more serious<br />
problems in the bowel and throughout the body.<br />
well as sunlight, dietary sources are just as important,<br />
for example oily fish such as salmon, or beef liver,<br />
mushrooms and egg yolks make good choices. To treat<br />
deficiency however, supplements are recommended.<br />
Although green tea has not been studied in regards to<br />
inflammation during IBS, the active ingredients called<br />
polyphenols inside have been helpful in treating<br />
inflammation during inflammatory bowel disease. Some<br />
people may wish to drink green tea, and others try<br />
capsules. Those sensitive to caffeine will need to limit<br />
their intake but those on medication interacting with<br />
green tea are best not to take any at all.<br />
Curcumin is an active compound within turmeric,<br />
packed full of polyphenols which have been shown to<br />
lead to a 60% reduction in IBS prevalence, including<br />
reduced abdominal pain and discomfort. Getting this<br />
through the diet is difficult as you would need to have<br />
several teaspoons of turmeric everyday, therefore a<br />
supplement is more appropriate for most IBS sufferers.<br />

YOUR ROAD TO RECOVERY AFTER A TRAFFIC ACCIDENT<br />

Whiplash refers to a neck injury that is<br />
caused by sudden movement of the head.<br />
This can occur forwards, backwards or sideways and<br />
often occurs following a road traffic accident.<br />
Common symptoms include:<br />
Pain and stiffness causing difficulty in<br />
moving the neck<br />
Headaches<br />
Tenderness over the muscles of the neck<br />
Pain in the arms or shoulders<br />
Commonly, symptoms do not occur immediately but<br />
can take at least 6-12 hours to develop after the injury<br />
and may continue for several days. Severe symptoms<br />
can cause concern but investigation often fails to find<br />
any abnormality. A description of events and the<br />
symptoms and examination will often reveal the<br />
diagnosis. Further tests rarely reveal any further<br />
problems but may be requested if a fracture is<br />
suspected.<br />
PAIN RELIEF<br />
Pain associated with whiplash can be initially treated<br />
with simple painkillers. Paracetamol can be helpful in<br />
treating mild neck pain. Non-steroidal antiinflammatory<br />
drugs (NSAIDs), such as Ibuprofen, can<br />
also help to reduce pain and inflammation. These<br />
options are usually sufficient for most people.<br />
Paracetamol or Ibuprofen can be taken separately ‘as<br />
required’ or regularly if needed. If pain persists,<br />
Paracetamol and Ibuprofen can be taken regularly<br />
together but this depends on the severity of the pain<br />
and/or patient preference.<br />
If further pain relief is required, your GP will be able to<br />
prescribe stronger painkillers. Always refer to the<br />
manufacturer’s recommended instructions when taking<br />
any painkillers. Ibuprofen should be avoided if you have<br />
a stomach ulcer or had this in the past. You should also<br />
avoid Ibuprofen if you have a history of severe heart<br />
failure or liver disease. Take medical advice if in doubt.<br />
EXERCISE<br />
Try to keep your neck mobile with<br />
There is evidence immobilising the<br />
collars can actually delay recovery.<br />
Therefore, early mobilisation and return to normal<br />
activities is encouraged. This may initially be painful but<br />
this is not harmful or a sign of further damage.<br />
If symptoms persist despite self-managing symptoms, a<br />
referral to physiotherapy may be required.<br />
Physiotherapy can help improve pain and stiffness<br />
associated with a neck injury. Your physiotherapist can<br />
give you exercises to continue at home as well.<br />
It is also important to maintain good posture when<br />
sitting, standing or walking, by keep your back straight<br />
and avoid slouching. You should also ensure you have a<br />
supportive pillow when sleeping.<br />
Most people with whiplash will find their symptoms<br />
improve with these simple measures, but occasionally<br />
symptoms persist for several months after the initial<br />
injury. Consult your GP for further advice if your neck<br />
pain is interfering with work and your day-to-day<br />
activities.<br />

With the good weather fast approaching,<br />
it’s a great time to take your exercise<br />
outdoors.<br />
This is a great opportunity to take in the fresh air and<br />
some vitamin D from the sunshine in your garden or a<br />
local park. The great thing about exercising outdoors is<br />
that it’s free and there’s enough space to do it with<br />
friends to make the experience more fun, and even<br />
competitive. Exercise doesn’t have to be a chore and it<br />
doesn’t have to take up your whole day.<br />
Exercising outdoors can be daunting and we<br />
understand that, so here is our guide to a great<br />
workout, using nothing but your surroundings, your<br />
bodyweight and maybe a mat.<br />
Start off by warming up. By elevating your heartrate<br />
you increase your body’s potential to burn fat, and help<br />
to avoid injury by loosening your muscles. Start with a<br />
jog for a minute, take up the speed for another minute<br />
then flick those heels up towards your bottom. Finish<br />
off with a minute of high knees (knees to chest) and<br />
then a final minute of jogging…make sure you’re really<br />
pumping your arms while you do it.<br />
We recommend interval training. There’s nothing<br />
better at burning calories and keeping them burning for<br />
the next day or two. This type of high intensity training<br />
will trigger the EPOC effect. This stands for ‘Excess<br />
Post-exercise Oxygen Consumption’. During your<br />
recovery period your metabolism is elevated due to<br />
the increased intake of oxygen into the body following<br />
strenuous exercise. This can continue for up to 38<br />
hours following high intensity exercise, meaning your<br />
daily energy expenditure is increased and more calories<br />
are burnt for longer.<br />
Time yourself for one minute and complete the<br />
following: Burpees, tricep dips (using a bench or wall),<br />
press ups, squat jumps, a plank and hill sprints. Repeat<br />
this routine three times. Follow up with a good stretch.<br />
So simple, yet ever so affective!<br />
This routine will take no more than 30 minutes, so put<br />
on your trainers and give it a go.<br />
I guarantee you’ll have a clearer<br />
head, less stress and feel fitter<br />
in no time!<br />

PATIENT UPDATE<br />
Following national publicity surrounding Meningitis B, our vaccination service has seen<br />
unprecedented demand. Whilst we have done all we can to satisfy the need for Meningitis B<br />
vaccinations, we are also mindful of the impact this has had on our service for other patients.<br />
When the sad story broke about a child fatality from Meningitis B, the Bexsero ® vaccine was in short supply as the<br />
manufacturer temporarily withdrew stock from the open market. JDoc365 nevertheless managed to maintain a<br />
regular supply and, as word spread, our telephone lines quickly became overwhelmed with calls from concerned<br />
parents trying to arrange for their children to be vaccinated.<br />
Despite setting up a dedicated line and hiring additional staff to cope with the volume of calls,<br />
some JDoc365 customers were unable to get through before being re- re-directed to our<br />
message service. We are sorry if you were inconvenienced by the temporary disruption this<br />
situation caused, and that you will find our normally excellent reception service is back to its<br />
best the next time you call.<br />

Caroline Katz knows how precious time<br />
can be. When someone in the family needs<br />
to see a doctor, time can be of the essence.<br />
Not everything demands urgent attention though.<br />
Sometimes Mrs Katz can wait to see her NHS doctor -<br />
after all, they do a fantastic job.<br />
Sometimes you need to see a<br />
doctor sooner rather than later.<br />
That’s when Mrs Katz and her<br />
family turn to JDoc365.<br />
Mrs Katz is a fitness instructor and her husband is CEO<br />
of a telecoms business. The family have been members<br />
of JDoc365 since the practice was first established in<br />
2006. Mr and Mrs Katz and their children are relatively<br />
healthy despite a busy lifestyle, pressure of work and<br />
school, family commitments and the demands of a<br />
close-knit and active community.<br />
They really appreciate the fact that Dr. Abelman<br />
and his team always find the time to see them, often at<br />
short notice. It’s not always a medical crisis needing<br />
emergency attention, it’s more that time is such a<br />
precious commodity, and scheduling everything into<br />
busy days can be quite a challenge.<br />
Mina Mikram, the JDoc365 Golders Green Clinic<br />
Manager, always recognises Mrs Katz when she visits<br />
the surgery, and his warm greeting is much appreciated.<br />
It’s the time, care and attention given by Dr. Abelman<br />
that Mrs Katz is particularly grateful for though. Mrs<br />
Katz explains, “It’s all in the detail. Dr. Abelman makes<br />
it his business to understand his patients – whether<br />
it’s me, my husband, one of my sons or my mother. He<br />
is so calm and knowledgeable.<br />
On one occasion I brought my eleven year old<br />
in three times in ten days. Whilst we were<br />
there, chest x-rays were organised and we<br />
just went straight downstairs for them.<br />
Because of that, my son was diagnosed<br />
accurately, quickly and conveniently.<br />
A precise dose of antibiotics were<br />
prescribed and I collected them<br />
from the pharmacy whilst I was<br />
there. My son made a speedy<br />
recovery, and the impact on<br />
my business through my own<br />
absence was minimised.<br />
Even when Dr. Abelman himself isn’t available,<br />
he has such a good team around him, it<br />
makes no difference who you see.<br />
Dr. Lewin saw us on one occasion and<br />
he was great. Dr. Abramov is also a<br />
fantastic doctor. She is so understanding<br />
and has such a caring bedside manner.<br />

Mr. Ezra is consultant ophthalmic surgeon to<br />
Moorfields Eye Hospital, and practices privately.<br />
Without doubt, almost all of you will have<br />
experienced an episode of red eyes at some<br />
point. There are many conditions that cause<br />
this, and some can be quite alarming.<br />
Eye problems contribute up to<br />
7% of GP visits, and the majority<br />
of these relate to red eyes<br />
No discussion of red eyes in primary care can be made<br />
without mention of blepharitis - an inflammatory<br />
disorder of the eyelid margins. It is extremely common,<br />
affecting up to 20% of people, and tends to present<br />
with non-specific symptoms of redness, grittiness and<br />
dryness. There are approximately 30 oil producing<br />
glands in each eyelid that supply oil for the tear film.<br />
This oily layer covers the watery layer of the tear film<br />
preventing evaporation and providing stability. Changes<br />
in the constitution of the oil secretions make them<br />
solid rather than liquid at body temperature, clogging<br />
the glands and allowing bacteria to colonise the eyelid<br />
margins. These bacteria in turn produce toxins that<br />
cause irritation (see figure 1). The best treatment is hot<br />
compresses and massage which act to heat the eyelid<br />
margin and liquefy the secretions. For more persistent<br />
or serious complaints, steroids and antibiotics may be<br />
required but this is unusual. Another option may be<br />
deep cleaning using specialised equipment which usually<br />
gives relief for 6-9 months.<br />
Another very common condition is infective<br />
conjunctivitis. This usually arises after an upper<br />
respiratory tract infection such as a cold or the flu.<br />
There are two types of infective conjunctivitis: bacterial<br />
and viral. Bacterial conjunctivitis is easy to recognise; it<br />
is characterised by significant mucous discharge which<br />
typically causes the eyelids to stick together in the<br />
morning, requiring considerable effort to reopen them<br />
(see figure 2). Bacterial conjunctivitis is treated with<br />
antibiotic eye drops. The viral form of conjunctivitis<br />
feels very different. It causes a more watery discharge<br />
with less stickiness, but paradoxically causes more<br />
irritation and grittiness. Viral conjunctivitis is not cured<br />
by antibiotic drops, but is treated with supportive<br />
measures such as lubricants until the infection resolves,<br />
usually after just a few days. Infective conjunctivitis,<br />
especially the viral type is highly contagious. It is<br />
important to prevent the spread of infection by<br />
ensuring that hands are always washed and not to<br />
share towels or bedsheets.<br />
The red eye condition with probably the most alarming<br />
appearance is also probably the most benign;<br />
subconjunctival haemorrhage. This is a condition where<br />
blood collects under the conjunctiva after a small<br />
capillary ruptures. This is usually entirely benign and<br />
tends to resolve after a few days (see figure 3).<br />

Whilst these conditions are not sight threatening,<br />
please remember that if you have a red eye with<br />
symptoms of significant pain, aversion to light or<br />
decrease in vision, this may indicate a more serious<br />
problem and you should see an Ophthalmologist (eye<br />
specialist) or a GP at the earliest opportunity.<br />
