I feel very privileged to be able to help my patients as a<br />

result of the study and training I have received, and the<br />

experience I have gained throughout my 30 year<br />

medical career.<br />

Whilst constant advances in medical science provide<br />

new insight, and the ability to prevent and treat many<br />

illnesses now, many doctors also posses an invaluable<br />

‘sixth sense’ when examining patients.<br />

Sometimes, an internal alarm bell<br />

will ring, and I will be compelled<br />

to check for something purely on<br />

an inexplicable ‘hunch’<br />

This happened to me not so long ago. A mother<br />

brought her nine month old baby boy to me for a<br />

check-up as he had a slightly raised temperature.<br />

Having conducted all the obvious checks with no cause<br />

for alarm, an inner voice told me that something was<br />

still not right. I referred the baby to hospital to be sure,<br />

and his mother took him straight there.<br />

When the baby was being examined in the hospital, he<br />

suddenly developed a rash which hadn’t been evident<br />

before. It turned out to be bacterial meningitis, so the<br />

appropriate medication was administered without<br />

delay. Thankfully he made a full recovery, thanks in part<br />

to being in the right place at the right time.<br />

Similarly, on another occasion a concerned mother<br />

brought her child to see me as he was struggling with a<br />

nasty cold. Maybe parents also develop a sixth sense<br />

when caring for their children, as suddenly his<br />

demeanour changed and I realised that something<br />

more serious was going on. I put the patient on the<br />

couch, and a moment later he stopped breathing and<br />

his eyes rolled up. Although terrifying for the patient<br />

and mother, we were quickly able to resuscitate him<br />

and return his vital signs to a normal level.<br />

Fortunately cases like this are very rare, but they do<br />

happen. Most of the time, the conditions are selflimiting<br />

and everything settles back to normal with little<br />

additional intervention. Even when more serious<br />

conditions are found, modern medicine can now treat<br />

them effectively. During my career, advances in medical<br />

science have given us access to new drugs and<br />

medication, and a deep understanding of how to<br />

administer them for the greatest effect. Guidelines have<br />

been developed to help us treat and monitor<br />

conditions, and there is a huge bank of research and<br />

information which doctors can access to help provide<br />

the best possible service to our patients.<br />

Having successfully tackled so many conditions, patients<br />

now live longer than ever before. As a direct result,<br />

new diseases like dementia have replaced them, and<br />

our aging population has new and different healthcare<br />

needs compared to previous generations.<br />

The challenge of meeting the needs of the patients of<br />

the future goes on. Further amazing advances in<br />

medical science continue to help, but the doctors sixth<br />

sense will always have an important role to play in<br />

looking after patients.<br />

Anxieties & Phobias in Children<br />

Children, like adults, can experience anxieties and phobias, and in equal<br />

measure. No individual is immune to anxiety across the age ranges.<br />

We see anxiety manifesting itself in children as young<br />

as a few months old in extreme circumstances. They<br />

may go off their food, have sleep disturbance, become<br />

withdrawn or hard to manage when stressed. This is<br />

well recognised now by parents, carers and educators.<br />

Some children are more anxious than others, and we<br />

recognise the child who worries about contracting<br />

SARS or any other ailments they learn about on<br />

posters, at school or via the media. Some may worry<br />

about the safety of their parents if they cycle to work<br />

or travel abroad, or terrorists or plane crashes. We<br />

should remember that children’s anxieties are no more<br />

‘irrational’ than our own.<br />

changes in behaviour, avoidance behaviours or<br />

apparent distress, then assessment by a registered<br />

Clinical Psychologist will be useful. They may consider<br />

cognitive behavioural therapy or other techniques.<br />

Don’t try this yourself, as you may inadvertently make<br />

the anxieties worse, especially if the child perceives that<br />

family members find their worries fanciful or absurd.<br />

Anxiety is useful and can help a child<br />

to perform at their best in a school<br />

show or test<br />

Anxiety prepares a person for ‘fight or flight’ and results<br />

in a surge of adrenalin. However, when worry causes<br />

distress and avoidance of usually enjoyed activities, this<br />

is an issue which is likely to need addressing.<br />

An 8 year old child came to me as he was convinced<br />

that there were snipers in his garden who were<br />

threatening the wellbeing of his loved ones. This may<br />

sound fanciful, but he was aware of well publicised<br />

random acts of violence. He knew about the London<br />

bombings and did not want to use the Underground.<br />

He also knew about Maddie and worried that he, or a<br />

sibling, may be kidnapped.<br />

These occurrences are real, so dismissing the worries as<br />

ridiculous will not help. If your child is significantly<br />

anxious over a period of two or more weeks with any<br />

Remember, many of our anxieties or phobias seem<br />

trivial to others. Why would you be afraid of dogs,<br />

spiders, snipers, bombers or the dark? They don’t harm<br />

you, or are so rare so as to be of negligible concern.<br />

Listen attentively to your child, and take their concerns<br />

seriously. Respond to their subjective distress and seek<br />

the help of a qualified professional where necessary.<br />

MEET THE DOCTOR<br />

Dr. Heschil Lewin<br />

Dr Heschil Lewin is multi-lingual, being fluent in<br />

Hebrew and German having grown up and trained in<br />

Munich, and also lived in Israel. He has been resident in<br />

London since 1987, where he met and married his wife<br />

Lorraine. They have three children and two grandchildren.<br />

Heschil has a Doctorate in Medicine (based on his<br />

research into urology and bladder cancer), a Diploma in<br />

Clinical Dermatology and a Diploma in Gynaecology &<br />

Obstetrics (DRCOG).<br />

Before becoming a GP in 1989, Heschil worked in A&E,<br />

gynaecology and obstetrics, later gaining further invaluable<br />

experience as lead GP at the Primary Urgent Care Centre<br />

at Homerton Hospital.<br />

He has just celebrated the first anniversary of joining the<br />

JDoc365 team, where he practices from Golders Green.<br />

How do you feel?<br />

20% of breast cancers can be found by self examination<br />

One in eight women will contract breast cancer. It is the commonest<br />

cancer in the UK with 80% of cases recorded in women aged over 50.<br />

85% of patients survive breast cancer for 5 years or<br />

more so early detection is essential. Some risk factors<br />

are inherited, a history of breast cancer or other breast<br />

conditions, an early start of periods or late menopause<br />

also increases the likelihood of breast cancer.<br />

Other factors include the use of hormones in the<br />

contraceptive pill, a late first pregnancy (over 30 years<br />

old), obesity, smoking, excessive drinking and postmenopausal<br />

HRT. No scientific evidence supports the<br />

theory that antiperspirant, tight bras, termination of<br />

pregnancy or breast implants impact on risk. However,<br />

prolonged breastfeeding seems to be protective, as<br />

well as exercise.<br />

Brisk walking for just two hours a<br />

week reduces the risk of breast<br />

cancer by 18%<br />

About 20% of breast cancers are found by physical<br />

examination rather than mammography. Look in the<br />

mirror for uneven breast size or shape, nipple changes<br />

or discharge, or a rash. Check with arms raised too.<br />

Feel your breasts while lying down. Use the pulp of<br />

your fingers to check the opposite breast in small<br />

circular movements. Start from the nipple and move in<br />

larger circles, covering the whole breast to the armpit.<br />

Breasts can feel lumpy, but you are looking for a lump<br />

that feels different from the rest of the glandular tissue,<br />

or a palpable lymph node in the armpit. Regular selfexamination<br />

helps to identify changes early. Not all<br />

lumps are cancer but if in doubt, consult your Doctor.<br />

If a lump or nipple discharge is found, or as a regular<br />

screening tool for women between 50 and 70, a<br />

mammogram may be carried out. MRI scans can be<br />

used for screening high risk women too, or to examine<br />

a suspicious area. If an abnormality is detected, a biopsy<br />

is the only way to tell if cancer really is present.<br />

How much does food<br />

influence your mood?<br />

If you have been diagnosed with irritable<br />

bowel syndrome (IBS), you may feel like<br />

it’s a never ending battle. You may have<br />

trialled a range of prescribed drugs or<br />

peppermint products but found many of<br />

their effects just do not last. Why is this?<br />

IBS is a functional bowel disorder, meaning bowel<br />

movements are affected. Apparently the cause of<br />

altered movements is down to changes in our gut<br />

bacteria, so unless you address the bacteria directly and<br />

work out what is affecting them, your symptoms will<br />

never go away. These bacteria can produce substances<br />

that cause inflammation, which is another good reason<br />

to nip this problem in the bud. One of the things often<br />

ignored is how our brain affects the bowel.<br />

Our state of mind impacts on the make-up<br />

of bacteria in the bowel, and effects the<br />

function and symptoms experienced.<br />

Stress and lower mood for example can also impact on<br />

the choice of foods and drinks consumed, which can<br />

then go on to make symptoms worse.<br />

Before seeing a Dietitian for any dietary manipulation it<br />

is really important for you to become aware of how<br />

your lifestyle affects your bowel symptoms. For this I<br />

recommend using a food and mood diary for at least 3<br />

days, including 1 weekend day. It will help you see a link<br />

between your mood, and why you choose to eat or<br />

drink something.<br />

People often start the day with overstimulated ‘loose’<br />

bowels which are linked to excess caffeine and stressful<br />

starts. Others miss meals or do not drink enough<br />

because they are rushed, leading to burping,<br />

constipation and bloating.<br />

Of course solving this is easier said than done. Stress<br />

management e.g. relaxation therapy, mindfulness,<br />

meditation, massage and all forms of exercise can help.<br />

Consider regular planned meals, frequent water sips<br />

and limiting to four caffeinated drinks a day too, before<br />

changing what food products you consume.<br />

One simple check could save your life. Even if you feel<br />

healthy now, you may have inherited health risks or<br />

problems that are not evident, but may be detected<br />

through examination and investigation.<br />

Despite increased awareness of lifestyle and inherited<br />

factors that influence our health, none of us really know<br />

how ‘healthy’ we are. For many going about their busy<br />

lives, subtle signs or bearable symptoms can be<br />

ignored, only for conditions to re-emerge with serious<br />

implications later on. Even when screening or checking<br />

for specific symptoms, it’s easy to miss other vital clues<br />

that something may be wrong.<br />

That’s why a thorough ‘Wellbeing Check’ is<br />

recommended. They are like your own annual MOT,<br />

the perfect opportunity to take stock of your health,<br />

and are a form of health screening which is<br />

preventative, helping to avert the onset of disease early.<br />

Many conditions such as high blood pressure, prediabetic<br />

states, high cholesterol, prostate cancer, heart<br />

problems and stress can be prevented or effectively<br />

treated before your health is compromised.<br />

illustrated by the case of the 52 year old woman who<br />

lobular carcinoma. Although this form of cancer can be<br />

felt, it is not always apparent on mammograms.<br />

Swift and thorough action was taken. All ended well, as<br />

after a further lumpectomy, breast conserving surgery<br />

and radiotherapy, the patient was given the all clear.<br />

If the lump had not been detected, it may have grown<br />

and spread with devastating consequences. Now, five<br />

years on, the patient remains fit, well and very grateful<br />

for the vital discovery that saved her life.<br />

Fight or Flight?<br />

regretted it shortly afterwards or felt<br />

yourself simmering with resentment for<br />

Anger can wreak havoc on relationships and have a destructive<br />

effect on yourlife.<br />

As a psychotherapist that specialises in anger<br />

management, I have seen three key approaches play a<br />

vital role in helping to control anger.<br />

MANAGING ANGER 1 GAINING CLARITY<br />

In the heat of the moment we often become irrational,<br />

blowing things out of proportion. Once we feel calmer,<br />

it’s important to look rationally and objectively at the<br />

anger-provoking situation. By changing the way we<br />

think about the situation, and seeing it from the other<br />

person’s perspective, we can change our emotional<br />

reaction to it.<br />

MANAGING ANGER 2 COMMUNICATION<br />

When a person feels angry two responses are<br />

common: fight (explode outwardly) or flight (retreat<br />

inwardly). Learning to communicate one’s feelings in an<br />

open, non-confrontational way is very important. It<br />

leads to creating a safe space where both sides can be<br />

understood and work to resolve their differences.<br />

MANAGING ANGER 3 PERSONAL CHANGE<br />

Simple lifestyle changes can often help us to become<br />

less prone to anger; for example getting more sleep,<br />

eating better or being more organised. On a deeper<br />

level, that may mean overcoming insecurities and<br />

confronting unresolved issues from the past.<br />

Zika<br />

What you should know about the latest<br />

tropical virus that’s all over the news…<br />

Zika was discovered in 1947, although<br />

outbreaks were rare prior to 2015.<br />

Until recently, Zika occurred mainly in Africa, Southeast<br />

Asia and the Pacific Islands. Now however, we see an<br />

increasing number of cases in Brazil, and across Latin<br />

America and the Caribbean.<br />

Zika virus is transmitted primarily by the bite of an<br />

infected Aedes mosquito. Whilst there have also been<br />

cases which were transmitted sexually or from mother<br />

to baby via the placenta, these are rare. Generally, Zika<br />

is NOT passed from person to person.<br />

What are the symptoms?<br />

Zika has an incubation period of 3-12 days. Symptoms<br />

include low grade fever, joint and muscle pains, rash,<br />

conjunctivitis, a headache and eye pain.<br />

Why is Zika such a serious virus?<br />

When contracted during pregnancy (especially in the early<br />

stages), the Zika virus is associated with microcephaly.<br />

This is a neurological condition in which a baby's head<br />

is significantly smaller than those of other children of<br />

the same age and sex, and this will often lead to<br />

developmental issues. During the Zika outbreak, there<br />

have also been an increased number of cases of<br />

Guillain-Barre Syndrome. This affects the peripheral<br />

nervous system. It occurs when the body's immune<br />

system attacks part of the nervous system.<br />

How can you minimise the risk?<br />

There is currently no vaccine against Zika virus, so<br />

pregnant women should avoid traveling to high risk<br />

areas. If travel is unavoidable, scrupulous mosquito<br />

bite avoidance is advised.<br />

If traveling to a high risk area, avoid conception during<br />

travel and for 28 days after returning home.<br />

If a pregnant woman returns from areas of high risk,<br />

even she if does not have any symptoms, consider<br />

seeking medical advice to rule out having<br />

acquired the virus.<br />

FINGER<br />

Trigger finger is a common condition<br />

affecting the base of the finger or<br />

thumb in the palm, particularly when<br />

trying to straighten the digit.<br />

It can produce a “click” when the digit is straightened. Tendons<br />

pass through a tunnel in the finger, known as the flexor sheath.<br />

Sometimes this becomes swollen and restricts tendons, causing<br />

them to ‘catch’ and ‘lock’. This catching can cause pain and<br />

restrict movement which results in ‘triggering’.<br />

Trigger finger is more common in women, patients over 60, in<br />

the dominant hand and in those with diabetes or arthritis. It is<br />

also associated with other conditions such as carpal tunnel<br />

syndrome, which is caused by compression of the median<br />

nerve and can cause tingling and numbness in the fingers.<br />

Once the finger starts to lock, it is likely to get worse unless<br />

treated. Within the first six months, prevention of movement<br />

of the finger using a splint for up to six weeks can be effective,<br />

however it can result in some stiffness.<br />

Instead, most specialist Hand Surgeons treat trigger finger with<br />

a steroid injection. This is usually effective and patients can use<br />

the hand immediately, although the triggering may take a<br />

couple of weeks to settle down. Sometimes a repeat injection<br />

is needed six weeks following the first injection.<br />

In severe cases, a small operation may be necessary. This simple<br />

surgery only requires a local anaesthetic, but must be carried<br />

out by a specialist Hand Surgeon – and JDoc365 can refer you.<br />

The procedure is almost 100% successful, is not usually painful<br />

and patients are able to use the hand normally almost<br />
