Spring is in the air…
Emerging from Winter, the brighter days are within touching distance
now. In this edition of The Apple, we want to help you make sure
nothing spoils the new season for you or your family.
Many types of health screening are beneficial. We provide them to help detect, prevent and treat many illnesses and
conditions in JDoc365 patients. Breast cancer screening for example, can be a life saver. Dr. Sylvia Abramov offers
sound advice on the subject in her article on page 6, whilst I share some of my own experience on page 8.
Whilst some children are naturally outgoing, others are held back by anxiety. Emma Citron specialises in child
psychology, and many will find her article on page 4 enlightening. And did you know that your mood can be
influenced by what you eat and drink? Tara Whyand, specialist dietitian and nutritionist, suggests some lifestyle
adjustments that could help put the smile back on your face on page 7.
Planning a holiday? You’ll have read about the Zika virus, so check carefully before travelling anywhere. You may need
travel ‘jabs’ or vaccinations, so get them in plenty of time. We can tell you exactly which ones you’ll
need, and how far in advance of travelling you’ll need them.
And if there’s anything else we can do for you or your family, remember we’re only a phone call way.
Kind regards,
Sixth sense
By Dr. Herschil Lewin
A doctors instinct
can make all the
difference
I feel very privileged to be able to help my patients as a
result of the study and training I have received, and the
experience I have gained throughout my 30 year
medical career.
Whilst constant advances in medical science provide
new insight, and the ability to prevent and treat many
illnesses now, many doctors also posses an invaluable
‘sixth sense’ when examining patients.
Sometimes, an internal alarm bell
will ring, and I will be compelled
to check for something purely on
an inexplicable ‘hunch’
This happened to me not so long ago. A mother
brought her nine month old baby boy to me for a
check-up as he had a slightly raised temperature.
Having conducted all the obvious checks with no cause
for alarm, an inner voice told me that something was
still not right. I referred the baby to hospital to be sure,
and his mother took him straight there.
When the baby was being examined in the hospital, he
suddenly developed a rash which hadn’t been evident
before. It turned out to be bacterial meningitis, so the
appropriate medication was administered without
delay. Thankfully he made a full recovery, thanks in part
to being in the right place at the right time.
Similarly, on another occasion a concerned mother
brought her child to see me as he was struggling with a
nasty cold. Maybe parents also develop a sixth sense
when caring for their children, as suddenly his
demeanour changed and I realised that something
more serious was going on. I put the patient on the
couch, and a moment later he stopped breathing and
his eyes rolled up. Although terrifying for the patient
and mother, we were quickly able to resuscitate him
and return his vital signs to a normal level.
Fortunately cases like this are very rare, but they do
happen. Most of the time, the conditions are selflimiting
and everything settles back to normal with little
additional intervention. Even when more serious
conditions are found, modern medicine can now treat
them effectively. During my career, advances in medical
science have given us access to new drugs and
medication, and a deep understanding of how to
administer them for the greatest effect. Guidelines have
been developed to help us treat and monitor
conditions, and there is a huge bank of research and
information which doctors can access to help provide
the best possible service to our patients.
Having successfully tackled so many conditions, patients
now live longer than ever before. As a direct result,
new diseases like dementia have replaced them, and
our aging population has new and different healthcare
needs compared to previous generations.
The challenge of meeting the needs of the patients of
the future goes on. Further amazing advances in
medical science continue to help, but the doctors sixth
sense will always have an important role to play in
looking after patients.
Anxieties & Phobias in Children
Children, like adults, can experience anxieties and phobias, and in equal
measure. No individual is immune to anxiety across the age ranges.
We see anxiety manifesting itself in children as young
as a few months old in extreme circumstances. They
may go off their food, have sleep disturbance, become
withdrawn or hard to manage when stressed. This is
well recognised now by parents, carers and educators.
Some children are more anxious than others, and we
recognise the child who worries about contracting
SARS or any other ailments they learn about on
posters, at school or via the media. Some may worry
about the safety of their parents if they cycle to work
or travel abroad, or terrorists or plane crashes. We
should remember that children’s anxieties are no more
‘irrational’ than our own.
changes in behaviour, avoidance behaviours or
apparent distress, then assessment by a registered
Clinical Psychologist will be useful. They may consider
cognitive behavioural therapy or other techniques.
Don’t try this yourself, as you may inadvertently make
the anxieties worse, especially if the child perceives that
family members find their worries fanciful or absurd.
Anxiety is useful and can help a child
to perform at their best in a school
show or test
Anxiety prepares a person for ‘fight or flight’ and results
in a surge of adrenalin. However, when worry causes
distress and avoidance of usually enjoyed activities, this
is an issue which is likely to need addressing.
An 8 year old child came to me as he was convinced
that there were snipers in his garden who were
threatening the wellbeing of his loved ones. This may
sound fanciful, but he was aware of well publicised
random acts of violence. He knew about the London
bombings and did not want to use the Underground.
He also knew about Maddie and worried that he, or a
sibling, may be kidnapped.
These occurrences are real, so dismissing the worries as
ridiculous will not help. If your child is significantly
anxious over a period of two or more weeks with any
Emma Citron
Clinical Psychologist
Remember, many of our anxieties or phobias seem
trivial to others. Why would you be afraid of dogs,
spiders, snipers, bombers or the dark? They don’t harm
you, or are so rare so as to be of negligible concern.
Listen attentively to your child, and take their concerns
seriously. Respond to their subjective distress and seek
the help of a qualified professional where necessary.
How do you feel?
20% of breast cancers can be found by self examination
One in eight women will contract breast cancer. It is the commonest
cancer in the UK with 80% of cases recorded in women aged over 50.
by Dr. Sylvia Abramov
85% of patients survive breast cancer for 5 years or
more so early detection is essential. Some risk factors
are inherited, a history of breast cancer or other breast
conditions, an early start of periods or late menopause
also increases the likelihood of breast cancer.
Other factors include the use of hormones in the
contraceptive pill, a late first pregnancy (over 30 years
old), obesity, smoking, excessive drinking and postmenopausal
HRT. No scientific evidence supports the
theory that antiperspirant, tight bras, termination of
pregnancy or breast implants impact on risk. However,
prolonged breastfeeding seems to be protective, as
well as exercise.
Brisk walking for just two hours a
week reduces the risk of breast
cancer by 18%
About 20% of breast cancers are found by physical
examination rather than mammography. Look in the
mirror for uneven breast size or shape, nipple changes
or discharge, or a rash. Check with arms raised too.
Feel your breasts while lying down. Use the pulp of
your fingers to check the opposite breast in small
circular movements. Start from the nipple and move in
larger circles, covering the whole breast to the armpit.
Breasts can feel lumpy, but you are looking for a lump
that feels different from the rest of the glandular tissue,
or a palpable lymph node in the armpit. Regular selfexamination
helps to identify changes early. Not all
lumps are cancer but if in doubt, consult your Doctor.
If a lump or nipple discharge is found, or as a regular
screening tool for women between 50 and 70, a
mammogram may be carried out. MRI scans can be
used for screening high risk women too, or to examine
a suspicious area. If an abnormality is detected, a biopsy
is the only way to tell if cancer really is present.
JDoc365 provides breast screening, advice and, where
appropriate, referral to consultant breast surgeons. Call
0800 043 1492 for an appointment today.
PROFBIOTICS.COM
How much does food
influence your mood?
by Tara Whyand M.Sc, B.Sc, specialist dietitian and nutrition advisor to ProfBiotics.com
If you have been diagnosed with irritable
bowel syndrome (IBS), you may feel like
it’s a never ending battle. You may have
trialled a range of prescribed drugs or
peppermint products but found many of
their effects just do not last. Why is this?
IBS is a functional bowel disorder, meaning bowel
movements are affected. Apparently the cause of
altered movements is down to changes in our gut
bacteria, so unless you address the bacteria directly and
work out what is affecting them, your symptoms will
never go away. These bacteria can produce substances
that cause inflammation, which is another good reason
to nip this problem in the bud. One of the things often
ignored is how our brain affects the bowel.
Our state of mind impacts on the make-up
of bacteria in the bowel, and effects the
function and symptoms experienced.
Stress and lower mood for example can also impact on
the choice of foods and drinks consumed, which can
then go on to make symptoms worse.
Before seeing a Dietitian for any dietary manipulation it
is really important for you to become aware of how
your lifestyle affects your bowel symptoms. For this I
recommend using a food and mood diary for at least 3
days, including 1 weekend day. It will help you see a link
between your mood, and why you choose to eat or
drink something.
People often start the day with overstimulated ‘loose’
bowels which are linked to excess caffeine and stressful
starts. Others miss meals or do not drink enough
because they are rushed, leading to burping,
constipation and bloating.
Of course solving this is easier said than done. Stress
management e.g. relaxation therapy, mindfulness,
meditation, massage and all forms of exercise can help.
Consider regular planned meals, frequent water sips
and limiting to four caffeinated drinks a day too, before
changing what food products you consume.
Screen sensation
One simple check could save your life. Even if you feel
healthy now, you may have inherited health risks or
problems that are not evident, but may be detected
through examination and investigation.
“The Wellbeing Check revealed a well woman, but
breast examination revealed a 2cm lump in the upp
outer quadrant of the left breast.”
By Dr. Walter Abelman
Despite increased awareness of lifestyle and inherited
factors that influence our health, none of us really know
how ‘healthy’ we are. For many going about their busy
lives, subtle signs or bearable symptoms can be
ignored, only for conditions to re-emerge with serious
implications later on. Even when screening or checking
for specific symptoms, it’s easy to miss other vital clues
that something may be wrong.
That’s why a thorough ‘Wellbeing Check’ is
recommended. They are like your own annual MOT,
the perfect opportunity to take stock of your health,
and are a form of health screening which is
preventative, helping to avert the onset of disease early.
Many conditions such as high blood pressure, prediabetic
states, high cholesterol, prostate cancer, heart
problems and stress can be prevented or effectively
treated before your health is compromised.
The JDoc365 Wellbeing Check includes specific tests
based on gender, age and medical history. In most
cases, results are available within 24 hours.
The benefits of a Wellbeing Check are perfectly
illustrated by the case of the 52 year old woman who
booked one purely as a precautionary check. She
arrived looking healthy, having recently had an NHS
screening for breast cancer. Her mammogram
appeared clear and she was given a clean bill of breast
heath. This came as no surprise as the patient checked
her own breasts regularly.
The JDoc365 Wellbeing Check revealed a well woman,
but our breast examination revealed a 2cm lump in the
upper outer quadrant of the left breast.
Without delay, the patient was sent to a breast
surgeon and underwent a thorough assessment
including examination, ultrasound localisation and a
biopsy of the breast. Diagnosis confirmed a grade 2
lobular carcinoma. Although this form of cancer can be
felt, it is not always apparent on mammograms.
Swift and thorough action was taken. All ended well, as
after a further lumpectomy, breast conserving surgery
and radiotherapy, the patient was given the all clear.
If the lump had not been detected, it may have grown
and spread with devastating consequences. Now, five
years on, the patient remains fit, well and very grateful
for the vital discovery that saved her life.
COUNSELLING NORTH LONDON
Fight or Flight?
By Aryeh Sampson
If you’ve ever exploded with anger and
regretted it shortly afterwards or felt
yourself simmering with resentment for
days, you’ll know how harmful these
feelings can be.
Anger can wreak havoc on relationships and have a destructive
effect on yourlife.
Research has shown anger to be
linked to an increased likelihood
of heart disease, stroke, cancer
and the weakening of the
immune system
As a psychotherapist that specialises in anger
management, I have seen three key approaches play a
vital role in helping to control anger.
MANAGING ANGER 1 GAINING CLARITY
In the heat of the moment we often become irrational,
blowing things out of proportion. Once we feel calmer,
it’s important to look rationally and objectively at the
anger-provoking situation. By changing the way we
think about the situation, and seeing it from the other
person’s perspective, we can change our emotional
reaction to it.
MANAGING ANGER 2 COMMUNICATION
When a person feels angry two responses are
common: fight (explode outwardly) or flight (retreat
inwardly). Learning to communicate one’s feelings in an
open, non-confrontational way is very important. It
leads to creating a safe space where both sides can be
understood and work to resolve their differences.
MANAGING ANGER 3 PERSONAL CHANGE
Simple lifestyle changes can often help us to become
less prone to anger; for example getting more sleep,
eating better or being more organised. On a deeper
level, that may mean overcoming insecurities and
confronting unresolved issues from the past.
Zika
What you should know about the latest
tropical virus that’s all over the news…
By Dr. Dalia Bialostocky
Zika was discovered in 1947, although
outbreaks were rare prior to 2015.
Until recently, Zika occurred mainly in Africa, Southeast
Asia and the Pacific Islands. Now however, we see an
increasing number of cases in Brazil, and across Latin
America and the Caribbean.
Zika virus is transmitted primarily by the bite of an
infected Aedes mosquito. Whilst there have also been
cases which were transmitted sexually or from mother
to baby via the placenta, these are rare. Generally, Zika
is NOT passed from person to person.
What are the symptoms?
Zika has an incubation period of 3-12 days. Symptoms
include low grade fever, joint and muscle pains, rash,
conjunctivitis, a headache and eye pain.
Why is Zika such a serious virus?
The World
Health
Organisation
claim that a test
to more easily
diagnose
the Zika
virus could be
available within
weeks, not
years.
When contracted during pregnancy (especially in the early
stages), the Zika virus is associated with microcephaly.
This is a neurological condition in which a baby's head
is significantly smaller than those of other children of
the same age and sex, and this will often lead to
developmental issues. During the Zika outbreak, there
have also been an increased number of cases of
Guillain-Barre Syndrome. This affects the peripheral
nervous system. It occurs when the body's immune
system attacks part of the nervous system.
How can you minimise the risk?
There is currently no vaccine against Zika virus, so
pregnant women should avoid traveling to high risk
areas. If travel is unavoidable, scrupulous mosquito
bite avoidance is advised.
If traveling to a high risk area, avoid conception during
travel and for 28 days after returning home.
If a pregnant woman returns from areas of high risk,
even she if does not have any symptoms, consider
seeking medical advice to rule out having
acquired the virus.
A full range of travel vaccinations
are available from JDoc365,
including Yellow Fever jabs.
TRIGGER
FINGER
By Professor Abhilash Jain
MBBS, MRCS, MSc, PhD, FRCS(Plast)
Associate Professor of Plastic and Hand Surgery,
University of Oxford & Imperial College Trust London
Trigger finger is a common condition
affecting the base of the finger or
thumb in the palm, particularly when
trying to straighten the digit.
It can produce a “click” when the digit is straightened. Tendons
pass through a tunnel in the finger, known as the flexor sheath.
Sometimes this becomes swollen and restricts tendons, causing
them to ‘catch’ and ‘lock’. This catching can cause pain and
restrict movement which results in ‘triggering’.
Trigger finger is more common in women, patients over 60, in
the dominant hand and in those with diabetes or arthritis. It is
also associated with other conditions such as carpal tunnel
syndrome, which is caused by compression of the median
nerve and can cause tingling and numbness in the fingers.
Once the finger starts to lock, it is likely to get worse unless
treated. Within the first six months, prevention of movement
of the finger using a splint for up to six weeks can be effective,
however it can result in some stiffness.
Instead, most specialist Hand Surgeons treat trigger finger with
a steroid injection. This is usually effective and patients can use
the hand immediately, although the triggering may take a
couple of weeks to settle down. Sometimes a repeat injection
is needed six weeks following the first injection.
In severe cases, a small operation may be necessary. This simple
surgery only requires a local anaesthetic, but must be carried
out by a specialist Hand Surgeon – and JDoc365 can refer you.
The procedure is almost 100% successful, is not usually painful
and patients are able to use the hand normally almost
immediately.
