Registration Opens Wednesday, May 20, 2015 Certification Application Postmark/Submission Deadlines Regular Friday, July 31, 2015 $ 995 Late Thursday, October 1, 2015 $1,195 Continued in 2015! 13 th Hour Deadline for those who missed the Regular & Late Deadlines Thursday, October 15, 2015 $2,100 $50 Multimodality Discount Applicants who are current Diplomates of the Certification Board of Cardiovascular Computed Tomography (CBCCT) or those who passed the CBCCT exam and hold Testamur status are eligible for a $50 discount. Cancellation Deadlines & Refund Candidates who provide a written and signed request to cancel by Friday, November 20 are entitled to a refund of $795. Candidates who received a $50 multimodality discount will receive a refund of $745. Scan your SIGNED cancellation request, include as an attachment and email to administration@cbnc.org. Alternatively, you may fax your SIGNED cancellation request to 240-631-8152. Refunds will not be possible after November 20, 2015. Incomplete/Incorrect Application Fee Applications submitted without all required correct documentation are subject to a $75 resubmission fee. Non-Sufficient Funds Fee Checks returned to CBNC for non-sufficient funds are subject to a $35 fee. Rescheduling Fee Prometric has revised their reschedule police and fee structure with cost implications to candidates. Prometric will assess candidates a $25 rescheduling fee for appointments rescheduled less than thirty (30) days of their originally scheduled exam date. IN ADDITION, candidates who reschedule less than five (5) days of their originally scheduled exam date and time will be assessed an ADDITIONAL fee of $165 by CBNC. Individuals who pass the 2015 exam and are enrolled in the ABIM MOC program may claim 20 points of Self-Evaluation of Medical Knowledge (Part II) credit through ABIM. Please provide your 6-digit ABIM ID number on the CBNC application form. We’re Here for You! The CBNC Office is staffed from 7:00 AM to 8:00 PM (Eastern) on exam days and our focus is on you.
