As an employee of (Agency Name), you are a MANDATED REPORTER (per section 15630 of the<br />
Welfare and Institutions Code) and are also required to report ANY REASONABLY SUSPECTED<br />
ABUSE to the ombudsman and/or local police. Failure to report abuse is a misdemeanor, and is<br />
punishable with up to six months in the county jail or by a fine of up $1,000 (or both).<br />
All allegations of Abuse will be investigated and the ”report” will remain confidential (unless<br />
disciplinary action requires this report to become public). Any employee engaging in abuse,<br />
which is substantiated, will be immediately terminated. Any retaliation toward a reporter by<br />
an (Agency Name)’ employee will result in immediate disciplinary action, up to and including<br />
termination from employment.
