About Your Rehabilitation Team Upon your arrival at Henry Ford Macomb, the rehabilitation team evaluates your physical, medical, psychological, vocational and social needs. Working with you and your family, they develop an individualized goal plan. The team then meets weekly to review your progress, revise goals and plan for your successful discharge from the program. Your involvement and the involvement of your family are critical to your progress. In light of this, a team member will always involve you in plan recommendations and changes. If you or a family member has questions about your care, please direct them to a team member or the program manager. The following are descriptions of team members and their roles. You and Your Family You and your family are the most important members of the team and are encouraged to participate in the program by communicating with team members and the physician, and by attending patient/ family conferences, as requested. Your family or caregiver will be scheduled to attend therapy and nursing instruction sessions to aid in understanding your individual needs and prepare for discharge. 2

Physiatrist The physiatrist is a physician who specializes in Physical Medicine and Rehabilitation and is generally responsible for the medical management of patients involved in the rehabilitation program. The physiatrist evaluates, advises and directs the total care of the patient and utilizes the expertise of a variety of professionals. Rehabilitation Nurse Our specially trained nurses provide support and care on a 24-hour basis. They also provide you and your family with education. Physical Therapist The physical therapist addresses areas such as strengthening, balance and coordination, wheelchair activities, walking and stair climbing. 3
