Follow-up Services Henry Ford Macomb Hospitals and Henry Ford Health System offer comprehensive outpatient services to help you continue your recovery. Recommendations and arrangements for many of these services will be made prior to your discharge. Outpatient Therapy Henry Ford Macomb Hospitals offers an extensive array of physical, occupational and speech therapy programs on an outpatient basis. Therapy is offered on the campuses of our Warren physical rehab center and Clinton Township hospital as well as several conveniently located outpatient sites throughout Macomb County. Home Care Henry Ford Home Health Care offers an in-home team of certified health care professionals providing skilled nursing; physical, occupational and speech therapies; home health aide services; medical social work services; and nutritional counseling. Medical Equipment Henry Ford Health Products provides medical equipment, including ambulatory aids and wheelchairs; hospital beds, bedside commodes or shower benches; and traditional products such as blood pressure and diabetes supplies. Stroke Support Group This group meets monthly at the Clinton Township hospital. It offers support and socialization to you and your family as you adjust to lifestyle changes brought on by a stroke. 12

Insurance & Payment Information Medicare (with a secondary insurance) pays for the cost of inpatient rehabilitation provided you meet criteria at admission and during your stay. This criteria includes a medical condition that necessitates acute care in an inpatient rehabilitation program, along with the ability to participate in at least 15 hours of therapy per week. The Medical Directors at each program will make the decision whether you meet criteria, but it is always subject to review from Medicare. Private insurance, Medicaid, and managed care benefits will be verified prior to admission and pre-authorization for admission will be obtained by hospital staff. This is not a guarantee of payment. Throughout your stay, the rehabilitation staff will work with your insurance provider to obtain continued authorization and we will provide information regarding co-pays and deductibles. Private payment and reduced cost arrangements for patients who qualify for financial assistance can be arranged through the billing department of Henry Ford Macomb Hospitals. Third party funding sources such as Worker’s Compensation can be negotiated as needed. You are entitled to and will receive a written disclosure of your specific insurance information and coverage that is available to us at the time of your admission to the inpatient rehabilitation unit. Our admission referral coordinators will help you with this information.
